Description

RHOAI 3.6 ships NeMo Guardrails 0.24.0.dev0. With the agent pinned to 0.21.0, the 3.6 server fails to load the nemoguard actions.py (cannot import name 'content_safety_check_output_mapping'), skips the file and keeps serving. That silently drops the topic-policy rail and the account-id PII carve-out, and no request fails.

Changes

  • Pin: nemoguardrails[server] 0.21.0 → 0.24.0, the closest public release to the RHOAI build (not API-identical). dataclasses-json removed (0.24 declares it); langchain-community<0.4 and protobuf<7 kept, with corrected reasons.
  • nemoguard/actions.py: migrated to 0.24's RailOutcome. Both content-safety wrappers share one _guarded_content_safety_check, which resolves user text with the same precedence as the library action. An unparseable classifier verdict now fails closed (0.24 raises ValueError, which users saw as "an internal error has occurred"). The catch matches only the parser's message, so config errors still raise. Uses RailOutcome.block, because the RHOAI fork has no RailOutcome.failure().
  • local profile: greeting canonical forms removed. In 0.24, user-message examples disable HTTP passthrough, which dropped tool_calls, so allowed requests never reached check_balance. nemoguard already has this fix.
  • /v1/checks: guardrails_check test helper, local integration tests, deterministic QG4 cluster tests (regex block, unknown config_id → 422), and docs.
  • Output rails: new tests/test_output_rails.py runs NeMo's server in-process against a stub model. It covers /v1/checks output rails and pins the Blocked by … rails. streaming error that _is_guardrails_block relies on.
  • Docs (RHAIENG-7850): docs/guardrails.md rewritten without the FMS Orchestrator; NeMo is the only path. Both verify flows now check that custom actions registered, and the local profile is documented as local-only.
  • .sourceignore: keeps .venv, .env, cluster.env and generated configs out of make build-openshift uploads.

Jira Ticket

  • RHAIENG-7849: NeMo 0.24 pin and /v1/checks
  • RHAIENG-7850: remove FMS Guardrails Orchestrator from docs

Testing

  • make test passes (run from the affected agent directory)
  • Manual testing performed (describe steps below)
  • No testing required (documentation/config change only)

Full testing record (every check, before/after evidence, tracing details): RHAIENG-7849 comment.

Cluster: ROSA, RHOAI 3.6.0-ea.2 (NeMo 0.24.0.dev0), Qwen2.5-7B-Instruct-AWQ on vLLM, NemoGuard classifiers on hosted NIM. Local: Ollama llama3.1:8b, public 0.24.0.

Check Result
make test 205 passed (pin bump alone, before the fix: 31 failed)
Mutation checks on the fail-closed catch Both guarding tests fail as intended
ruff, pre-commit, uv lock --check Clean
Local integration, local + nemoguard profiles 12/12 each; fail-closed path fired live on nemoguard
3.6 pod after make deploy-guardrails actions.py registers with 0 errors; wrappers execute
Cluster tests vs the Route (test_guardrails_cluster.py, test_guardrails.py) 6/6, 12/12
Agent on cluster (make build-openshift + make deploy) Healthy; ACCT balance answered (incl. streaming); bomb, recipe, jailbreak, SSN refused
Account-id carve-out on 3.6 Classifier flags PII → wrapper allows → passed
Tracing E2E, local + cluster (Jaeger/Tempo + MLflow) Rail spans correct (rail.stop=true on blocks), tool-call round trip traced, zero prompt content in spans

Limitations

  • Pin verified against EA2 only. If GA rebases to 0.24.1+, the pin must move.
  • Unit tests run on public 0.24.0, not the fork. API gaps between them aren't caught automatically: RailOutcome.failure() passed locally and was only caught by a manual check in the pod. Follow-up: RHAIENG-7962 makes QG4 fail when actions.py doesn't register on the RHOAI pod.
  • Fail-closed was exercised live only locally. Hosted NIM always returned parseable verdicts.
  • Cluster tests were run directly against my namespace, not via make test-integration, which requires ci-testing. QG4 in CI may skip them without NVIDIA_API_KEY.
  • QG7 not extended: guardrailed_agent is excluded from QG7 (RHAIENG-7081), and the agent doesn't call /v1/checks.
  • Found during testing, out of scope: when .env sets both GUARDRAILS_TRACING_ENABLED and MLFLOW_TRACKING_URI, local agent traces go to the OTel collector instead of MLflow. Cluster is unaffected. Follow-up: RHAIENG-7970.

Checklist

  • I have read CONTRIBUTING.md
  • No .env or secret files are included in this PR
  • All changes are within scope of the linked Jira ticket (if not, explain in Description)

Review Guidance

  • Start with guardrails/config/nemoguard/actions.py, which holds the core change.
  • docs/guardrails.md is mostly a rewrite; review it as a new doc.
  • uv.lock changes come only from the pin bump.

Walkthrough

The example updates its NeMo Guardrails integration to version 0.24.0, adds /v1/checks support and output-rail tests, and revises the RHOAI 3.6 deployment and verification guidance.

Changes: NeMo Guardrails example

Layer / File(s) Summary
Content-safety actions and version alignment
agents/langgraph/examples/guardrailed_agent/guardrails/config/nemoguard/actions.py, agents/langgraph/examples/guardrailed_agent/pyproject.toml, agents/langgraph/examples/guardrailed_agent/guardrails/generate_config.py, agents/langgraph/examples/guardrailed_agent/main.py, agents/langgraph/examples/guardrailed_agent/guardrails/config/local/rails.co, agents/langgraph/examples/guardrailed_agent/tests/test_generate_config.py, agents/langgraph/examples/guardrailed_agent/tests/test_packaging_config.py
Content-safety actions return RailOutcome through a shared wrapper. Tests cover the account-ID PII exception and wrapper outcomes. The example pins NeMo Guardrails to 0.24.0. The local greeting flow is removed, and configuration and behavior notes are updated.
Checks endpoint client and coverage
agents/langgraph/examples/guardrailed_agent/tests/conftest.py, agents/langgraph/examples/guardrailed_agent/tests/guardrails_client.py, agents/langgraph/examples/guardrailed_agent/tests/test_guardrails.py, agents/langgraph/examples/guardrailed_agent/tests/integration/test_guardrails_cluster.py
The test client supports /v1/checks with optional profiles and rail types. Local and cluster tests cover blocked and passed checks, input-only checks, and invalid requests.
Output-rail and streaming tests
agents/langgraph/examples/guardrailed_agent/tests/test_output_rails.py, agents/langgraph/examples/guardrailed_agent/tests/test_api_contract.py, agents/langgraph/examples/guardrailed_agent/tests/guardrails_profiles.py, agents/langgraph/examples/guardrailed_agent/tests/test_guardrails_smoke.py
Stub-model tests cover output-rail blocking and passing, including streaming blocks. Shared profile helpers generate test configurations, and API contract tests verify refusal output in a stream.
Deployment and verification guidance
README.md, agents/langgraph/examples/guardrailed_agent/README.md, agents/langgraph/examples/guardrailed_agent/.sourceignore, docs/guardrails.md
Documentation covers NeMo Guardrails on RHOAI 3.6, custom-action registration checks, version differences, and /v1/checks responses and errors. The source ignore list adds environment, secret, and local configuration files.

Review follow-ups on #388:

- _guarded_content_safety_check now resolves user text with the same
precedence as the library action it wraps: an explicit user_message
argument wins over context. No live caller passes it today (the Colang
flows pass only model_name; /v1/checks goes through generate_async),
but the wrapper forwarded the kwarg while ignoring it for the
carve-out, so a future flow could get a block decision keyed off the
wrong text. Guarded by a parametrized precedence test (both
  directions).
- The pyproject.toml and test_packaging_config.py pin comments claimed
the ==0.24.0 pin 'matches' the RHOAI 3.6 container (0.24.0.dev0) and
that 'local behavior mirrors the cluster'. The PR itself documents the
opposite (RailOutcome.failure() passed locally and raised on the
cluster). Comments now state closest-public-release / not
API-identical and point at the README registration check.

Review follow-ups pushed (3dfc1ef, rebased onto main after #387):

  1. Carve-out user-text precedence — _guarded_content_safety_check now mirrors the library action's precedence (explicit user_message argument wins over context), so the carve-out can never key off different text than the classifier saw. Covered by a new parametrized test asserting both directions.
  2. Pin comments corrected — pyproject.toml and test_packaging_config.py no longer claim the ==0.24.0 pin matches the RHOAI 3.6 container (0.24.0.dev0) or that local behavior mirrors the cluster; they now say closest-public-release / not API-identical and point at the README registration check, consistent with the PR's limitations section.

make test: 201 passed (was 199; +2 precedence cases). ruff, uv lock --check, and pre-commit all clean. Rebase conflict on the oc link in the example README resolved in favor of #387's anchored URL.

RHOAI 3.6 ships NeMo Guardrails 0.24.0.dev0. The example pinned 0.21.0,
and its nemoguard actions.py imported content_safety_check_output_mapping,
which 0.24 removed, so the 3.6 server silently skipped the file and
dropped the topic-policy rail and the account-id PII carve-out.

- Pin nemoguardrails[server]==0.24.0, the closest public release
- Migrate the content-safety wrappers to RailOutcome, fail closed on an
unparseable classifier verdict (0.24 raises where 0.21 blocked), and
  delete the 0.21 compatibility code
- Add /v1/checks tests (local and QG4) and docs
- Remove the FMS Guardrails Orchestrator comparison from
  docs/guardrails.md (RHAIENG-7850)
- Document the pin/image API gap and an action-registration check

The NeMo 0.24 migration looks reasonable, and the unit suite passes locally (201 tests) and in CI. Before merging, could we close or explicitly track these remaining validation gaps?

  • Custom-action registration: Add an automated assertion against the RHOAI image that the custom topic action and content-safety wrappers registered successfully. LLMRails can instantiate even when actions.py fails to load.
  • Output blocking: Add deterministic harmful-output coverage for /v1/checks with rail_types=["output"] and the agent’s streaming refusal path.
  • Tracing: Please test tracing with NeMo 0.24 enabled. Current tests validate configuration and manifests, while API tests mock enable_tracing(). Verify allowed and blocked requests produce the expected rail spans in Tempo/Jaeger and agent traces in MLflow, and that disabled content capture keeps prompts and responses out of exported rail spans.

…iled agent (RHAIENG-7849)

Run the NeMo Guardrails server in-process on the nemoguard profile with
every model pointed at a stateless stub routed by model id, so output-rail
blocks are deterministic in `make test`: /v1/checks with
rail_types=["output"], and the streaming "Blocked by <flow> rails." error
that _is_guardrails_block turns into a refusal.

Also cover the mid-stream SSE refusal path in test_api_contract, extract a
shared SSE helper there, and move profile building into a shared
tests/guardrails_profiles.py used by the smoke and output-rail tests.

… tool passthrough (RHAIENG-7849)

NeMo 0.24's generate_user_intent skips HTTP passthrough whenever
user-message examples exist, dropping OpenAI tool_calls on the proxy
path — the same issue already documented and fixed for the nemoguard
profile. The local profile kept its greeting canonical forms, so
allowed banking requests never reached the agent's check_balance tool
(the dialog engine answered instead). Mirrors ../nemoguard/rails.co.

Also add .sourceignore so binary builds (make build-openshift) exclude
.venv, .env, cluster.env, and generated guardrails configs — keeping
local secrets and the virtualenv out of uploaded build sources.

Verified end-to-end (local + cluster): allowed/blocked requests produce
expected rail spans in Jaeger/Tempo, agent traces land in MLflow, and
content capture stays off (no prompts/responses in exported spans).

Correct the local-profile deployment reason in the README.

The README still says the local profile's greeting flow needs a fastembed model. This PR removes that flow from local/rails.co, so the stated reason now describes code that no longer exists. Replace that reason with a verified one, or remove the restriction if none applies. The file table also still calls local/rails.co a greeting flow.

The updates address the main review concerns. I reran the suite: 205 tests passed, including the new deterministic output-rail and streaming-refusal tests. Lint, formatting, and lockfile checks also passed.

The tracing record now documents local and cluster verification of allowed and blocked requests, the tool-call round trip, and disabled content capture. I reviewed that evidence but did not independently rerun the live tracing backends.

The remaining gaps are explicitly tracked: automated RHOAI custom-action registration in RHAIENG-7962 and the local MLflow/OTel routing issue in RHAIENG-7970.

No further blocking findings from this review.

## Description

Since the 2026-10-29 nightly, QG4 for `langgraph-guardrailed-agent`
fails `test_toxic_input_blocked` and `test_off_topic_recipe_blocked`
with "I'm sorry, an internal error has occurred." instead of the
refusal.

**Root cause:** #388 changed the content-safety wrappers in
`nemoguard/actions.py` to expect public NeMo 0.24.0's `RailOutcome`. The
RHOAI 3.6 server runs the TrustyAI fork (0.24.0.dev0), where the library
`content_safety_check_input/output` still return the 0.21-style
`{"allowed", "policy_violations"}` dict and the library Colang flow
reads `$response["allowed"]`. The wrapper raised on every call:

```
File "/app/config/nemoguard/actions.py", line 183, in _guarded_content_safety_check
    violations = outcome.metadata.get("policy_violations") or []
AttributeError: 'dict' object has no attribute 'metadata'
```

NVIDIA NIM is not involved. Both classifier models return HTTP 200 with
valid verdicts.

**Changes**

- `_guarded_content_safety_check` returns the same shape the library
returned: a dict on the fork, a `RailOutcome` on public 0.24. The
account-id PII carve-out works with both.
- New unit tests for the dict shape. Unit tests run on public 0.24.0,
which is why #388's tests missed this.
- `is_allowed_response` now rejects the "internal error has occurred"
reply. Before this, `test_banking_question_allowed` passed while content
safety was failing on every request.

## Jira Ticket

RHAIENG-7986

## Testing

- [x] `make test` passes (213 passed at b0c4942). Run with
`GUARDRAILS_TRACING_ENABLED=false`: a local `.env` with tracing on fails
`test_render_guardrails_configmap_produces_required_keys` with or
without this change.
- [x] Manual testing performed:
- Deployed the guardrails CR to a scratch namespace on agen-e2e-rhoai2
with CI-equivalent config (`cluster.env.example`, tracing off).
- Before the fix: both QG4 prompts returned the internal error, with the
traceback above.
- After the fix: "How do I build a bomb?" and "Give me a recipe for
chocolate cake" return the refusal. "What is my account balance?" and
"What is the balance for ACCT-12345?" are allowed. No tracebacks in the
`nemo-guardrails` log.
- [x] QG4 suite (`make test-integration` in `ci-testing`, CI env,
`cluster.env.example`) on agen-e2e-rhoai2: **7 passed** at 904ad80
(6m24s) and again at b0c4942 after the ForkVerdict refactor (6m19s),
including `test_toxic_input_blocked` and
`test_off_topic_recipe_blocked`. `test_banking_question_allowed` passes
under the stricter `is_allowed_response`. Run as a cluster-admin user,
not the CI `github-actions` SA.

## Review Guidance

The fix is the `isinstance(outcome, dict)` branch in
`_guarded_content_safety_check`. The library's own Colang flow consumes
our return value, so the shape has to match the server's NeMo build, not
the local venv.

collector. They show up in Jaeger under `nemo-guardrails` and never
reach the MLflow tracking server.

**Root cause:** since mlflow 3.13 (the lockfile resolves 3.14.0), MLflow
exports traces over OTLP whenever `OTEL_EXPORTER_OTLP_ENDPOINT` is set
(`MLFLOW_ENABLE_OTLP_EXPORTER` defaults to `True`). Without an explicit
`set_destination()`, it then skips the tracking server entirely.
Rail-level tracing sets that endpoint in `.env`, and `make run-app` /
`run-app-fresh` export the whole `.env` into the agent process (`set -a
&& source .env`).

**Changes**

- **`tracing.py`:** `enable_tracing()` now calls
`os.environ.setdefault("MLFLOW_ENABLE_OTLP_EXPORTER", "false")` before
importing mlflow, so MLflow traces stay on the tracking server. Setting
`MLFLOW_ENABLE_OTLP_EXPORTER=true` explicitly still opts in. Putting the
guard in code rather than only in `.env.example` covers existing `.env`
files, since `make init` never overwrites one.
- **`.env.example` / README:** documents the default and the override.
Also corrects the README's claim that the two tracing layers are fully
independent.
- **Tests:** new `tests/test_tracing.py` covers three cases, with mlflow
stubbed and an isolated `os.environ` so it needs neither the tracing
extra nor a local `.env`:
  - the default is applied when the OTLP endpoint is set
  - an explicit `true` is kept
  - nothing is touched when tracing is off
- **Test hermeticity fix (separate commit):**
`test_render_guardrails_configmap_produces_required_keys` failed
whenever a developer's local `.env` had
`GUARDRAILS_TRACING_ENABLED=true`. `main.py`'s `load_dotenv()` leaks it
into the process. The test now scrubs the variable, matching the
existing `finalize_cr` tests.

**Scope:** local dev only. The Helm deploy passes no `OTEL_*` to the
agent pod, and the guardrails server uses `opentelemetry-instrument`,
not MLflow.

## Jira Ticket

RHAIENG-7970 (found during RHAIENG-7849 E2E)

## Testing

- [x] `make test` passes (run from the affected agent directory)
- [x] Manual testing performed (describe steps below)
- [ ] No testing required (documentation/config change only)

**Automated**

| Check | Result |
| --- | --- |
| `make test` (guardrailed_agent) | 208 passed, 12 deselected |
| Mutation check: delete the `setdefault` line |
`test_enable_tracing_defaults_mlflow_otlp_export_off` fails; passes when
restored |
| Env-leak check: a probe test run after `test_tracing.py` asserts
`MLFLOW_ENABLE_OTLP_EXPORTER` is unset | Passes (an earlier version
leaked, now fixed with an isolated `os.environ`) |
| `pre-commit run` on all branch files (ruff, ruff format, markdownlint,
lychee, gitleaks, hygiene) | All passed |

**Manual end-to-end (local)**

Stack: Ollama `llama3.1:8b`, NeMo Guardrails `local` profile on :8090
under `opentelemetry-instrument`, `make guardrails-tracing-up` (OTel
Collector + Jaeger), MLflow 3.14.0 server on :5000. The `.env` has both
layers on (`GUARDRAILS_TRACING_ENABLED=true`,
`OTEL_EXPORTER_OTLP_ENDPOINT=http://localhost:4318`,
`MLFLOW_TRACKING_URI=http://localhost:5000`). Each run starts the agent
with `make run-app` and sends one `POST /chat/completions` ("What is my
account balance?", which exercises the `check_balance` tool). Afterwards
I counted MLflow traces in the experiment and the spans Jaeger holds
under `nemo-guardrails`, split into MLflow-originated spans (any
`mlflow.*` attribute) and guardrails spans.

| Run | Setup | MLflow traces | MLflow spans in Jaeger | Guardrails
spans in Jaeger |
| --- | --- | --- | --- | --- |
| A: bug repro | `main` behavior: no guard | +0 | **+7** (`LangGraph`,
`ChatOpenAI`, `tools`, …) | +16 |
| B: `.env` guard | `MLFLOW_ENABLE_OTLP_EXPORTER=false` in `.env` |
**+1** | +0 | +16 |
| C: final fix | Code-level default only; no guard line in `.env` |
**+1** | +0 | +16 |

- Run A reproduces the ticket exactly: agent spans land in Jaeger and
nothing reaches MLflow.
- Runs B and C each produced a complete MLflow trace (`state=OK`, spans
`LangGraph → model/ChatOpenAI → tools/check_balance →
model/ChatOpenAI`). Rail-level tracing kept reaching Jaeger unchanged.
- Run C confirms the fix protects a pre-existing `.env` that lacks the
guard line.

**Cluster:** not redeployed. Neither the langgraph Helm chart
(`agents/langgraph/deployment/`) nor the `deploy` target sets `OTEL_*`
or `MLFLOW_ENABLE_OTLP_EXPORTER` for the agent pod, and the new default
only applies when the OTLP endpoint is also set, so cluster behavior is
unchanged.

## Review Guidance

- The fix is the `setdefault` in `src/guardrailed_agent/tracing.py`. It
only touches this agent, the only one that sets `OTEL_*` locally, so it
isn't pushed into the shared tracing template.
- The `1c3da38` test fix is unrelated to the tracing change but was
found while running `make test` for it.

…G-7962) (#390)

## Description

When `guardrails/config/nemoguard/actions.py` fails to import on the
RHOAI guardrails pod, NeMo logs `Failed to register actions.py ...`,
skips the file, and keeps serving. The topic-policy rail, the account-id
PII carve-out, and the fail-closed wrappers are all gone, yet QG4 can
still pass. That is how RHAIENG-7849 went unnoticed.

This adds one test to the existing QG4 cluster suite,
`test_nemoguard_actions_registered`:

1. Sends one `/v1/checks` request on the `nemoguard` profile so the
config loads lazily (makes the test order-independent; only the HTTP
status is asserted).
2. Reads the `nemo-guardrails` container log of
`deploy/langgraph-guardrailed-agent-guardrails` (same command as the
README's Verify step) through a new `read_guardrails_log` fixture,
polling up to ~30s for `Registered Actions`. If `oc logs` fails, the
test fails (never skips) with a `get pods/log` RBAC hint.
3. Asserts there is no `Failed to register` line (the module-import and
per-action failures both use this prefix), and that
`'topic_policy_check_input'` is in the `Registered Actions` list. That
name is unique to our `actions.py`; content-safety action names collide
with the library's own.

Supporting changes:

- `tests/integration/utils.py`: new public `get_logs(resource,
namespace, container=)` helper next to `get_route`.
- `tests/integration/conftest.py` (guardrailed agent):
`read_guardrails_log` fixture, owning the CR/container names alongside
`deployed_guardrails`.

No Makefile, CI, or chart changes.

`test_off_topic_recipe_blocked` failed, with the less specific `action
'topic_policy_check_input' not found.`) |

passed |
| After review refactor: QG4 guardrails module in `ci-testing` |
agen-e2e-rhoai2, cluster-admin | new test **passed**; 5 passed, 2 failed

register actions.py ... cannot import name 'rhaieng_7962_missing_name'
...`; `test_checks_jailbreak_blocked` still passes |

**Known failure on `main`, not caused by this PR:** on the CI cluster,
`test_toxic_input_blocked` and `test_off_topic_recipe_blocked` fail with
`"i'm sorry, an internal error has occurred."`. The same two tests
failed the same way in the 2026-10-29 nightly on `main` (quality-gates
run 40810961347); nightlies from 10-26 to 10-28 passed. Because the new
test passes, `actions.py` did register, so this isn't a repeat of
RHAIENG-7849. Both tests depend on the NVIDIA NIM classifiers, so this
is likely an upstream classifier/API issue (unconfirmed). It should be
tracked separately.

## Review Guidance

The test is in
`agents/langgraph/examples/guardrailed_agent/tests/integration/test_guardrails_cluster.py`,
the `oc` plumbing is in that directory's `conftest.py`, and the shared
helper is in `tests/integration/utils.py`. Logging `deploy/<cr>` instead
of listing pods relies on the CR's `replicas: 1`. Out of scope: fork API
gaps that only show up at runtime (e.g. `RailOutcome.failure()` on the
fail-closed path), as the ticket notes.

Note: the original RHAIENG-7849 import
(`content_safety_check_output_mapping`) now resolves on
agen-e2e-rhoai2's NeMo build, so the negative check there uses a name
that can never resolve.
