Fixes the gate-repo stale-origin PR misroute (fm-nm-gate-repo-origin) and adds a bounded fail-loud guard (typed exit 70, strict invocation caps) against fm-merge-execute/fm-gh-shim mutual recursion under stale gate origin or mid-flight home update - the defect that drove the host past 4,000 processes on 2026-08-09.
Validation: focused recursion regression, shim, and merge suites green at exact head c8f59be, including valid one-pass cross-home merges. The no-mistakes pipeline was deliberately skipped for this change because its own gate repo carries the stale-origin defect this fixes: the run rebased onto forbidden upstream history (58 ahead/91 behind) and was cancelled at a safe gate. Delivered by direct push + main-deck PR per firstmate direction.
