What happened?
This badge is not showing in the page: https://coveralls.io/repos/github/voxpupuli/puppet-nodejs/badge.svg
and there is a redirection set to: https://s3.amazonaws.com/assets.coveralls.io/badges/coveralls_0.svg
Perhaps CSP should contain both URLs?
What browsers / clients are you seeing the problem on?
