fix(claude): preview images read from the workspace - #9119

💭 I'm scanning the diff for Effect service convention violations, checking whether the changes to threadActivity.ts, ActivityPayloadProjection.ts, and ClaudeAdapter.ts stay within pure helper functions and presentation logic, and confirming the new import of isWorkspaceImagePreviewPath is just a named pure helper. The remaining changes in session-logic.ts and work-log/presentation.ts also look like pure logic, and there's no sign of new Effect service definitions, errors, or ManagedRuntime usage anywhere. I'm now double-checking the ClaudeAdapter.test.ts imports for Random, Fiber, Effect, and Stream to confirm they're already established in the file and that the new Schema.encodeSync usage is properly imported.
