fix(server): persist late Claude tool results after turn finalize - #7192

💭 Looking at the in-scope files for UI consistency, I'm checking whether the session-logic changes are correct — specifically verifying that toolCallId exists on DerivedWorkLogEntry in the web version, since mobile added this field but the web diff doesn't show it being added to the entry type or extraction logic. The web implementation already has the toolCallId infrastructure in place, so these are purely logic changes without any UI or styling modifications. There's a minor detail where the merge preserves the original createdAt but uses the later entry's ID, though that's not a UI concern. The schema requires JSON output, so I'll structure the response with a success state and put "All clear" in the summary field rather than as a standalone line.
