fix(server): an external Codex account swap releases the running app-server - #29508
An app changing Codex accounts under T3, either by running codex login or by swapping ~/.codex/auth.json, is invisible to a running codex app-server. The app-server caches auth at spawn; its only reloads are guarded by account id and skip on mismatch; there is no watcher. When the old token still works, nothing errors: every turn keeps running on the old account and its limits burn. Issue #23720 is the loud cousin where refresh fails every turn; this silent variant has no open PR, and maintainer PR #28108 scopes external swaps out of its own body. Prior attempts #11147 and #24856 were closed unmerged.
Change
Adapters may expose resolveCredentialFingerprint, a tri-state probe: a string identity, null once the provider surface carries no file credential, undefined when unobservable (torn write, unreadable mode, probe failure). The Adapter contract carries the annotation; null and undefined carrying different release consequences is the point.
CodexAdapterV2 installs it for non-managed setups: a hash of tokens.account_id or the API key from auth.json, never the whole file, which is rewritten on every token refresh.
ProviderSessionManager records the fingerprint at open and, at the cache-reuse seam, releases a live session on any change, with reason manual_shutdown, detail "Provider credentials changed.", and an orchestration-v2.provider-session-released log line (release detail is not persisted for non-error shutdowns). The released session respawns and a fresh app-server re-reads auth.json at spawn, so the next turn runs on the new account and Limits show for it.
Busy turns are kept: the release happens at the next reuse seam, not mid-turn. Background work is kept. There is no retry loop: one probe per reuse decision, and unobservable reads never release.
No collision. #28108 catches settings-driven rebuilds via adapter identity; this catches external credential swaps via the fingerprint. When both land, the seam condition is rebuilt || fingerprintChanged. If #28937 (spare sessions) lands first, it restructures the baseline-capture region; rebase order matters there.
Keyring limitation (verified from source)
Keyring-to-keyring swaps leave no disk signal: the Direct backend (macOS/Linux default) stores nothing on disk, and under Secrets (Windows default) the ciphertext file is re-encrypted on every save, so hashing it tracks writes, not identity; that idea is deferred and must never become fingerprint input. A missing auth.json under keyring|auto maps to null on purpose: that is precisely the value that releases on file-to-keyring and keyring-to-file mode transitions, the only file-visible signal such homes have.
Verification
vpr typecheck: 39/39 packages. Focused suites: 282 passing across 7 files (re-ran after rebasing onto the current main tip). vp run check:changed: format, lint, knip, graphql, typecheck pass. The same single pre-existing, environment-dependent AgentSessionScanner test fails on this stack, proven failing at HEAD.
Established from openai/codex source at the pinned commit, statically and empirically. Repro on main, dev-server local: sign in via a Codex home backed by ~/.codex/auth.json, run a turn, then outside T3 replace auth.json's identity with another account's (or run codex login as the other user) and run another turn. The turn still runs on the old account and its limits, with no error anywhere; deleting auth.json entirely reproduces the same staleness. Source-level why: the app-server caches auth at spawn, its reload paths are guarded by account id (mismatch skips the reload), there is no watcher, and codex logout plus keyring saves both delete the file.
Verified end to end on a dev server: scratch CODEX_HOME, a synthetic auth.json carrying only a fake account id, a real codex binary, and an isolated T3 home with one project. The first turn spawns the app-server against the scratch home and captures the credential baseline. Rewriting auth.json to a second fake account and sending another turn releases the still-live session when the turn's open re-checks. Server log, verbatim: orchestration-v2.provider-session-released { providerSessionId: 'provider-session:provider-instance:prb-probe:shared', reason: 'manual_shutdown', detail: 'Provider credentials changed.' }. A fresh app-server spawns immediately after with the new baseline, and its 401s (missing bearer auth against the synthetic file) confirm both sessions read the scratch home. No real account state is involved anywhere.
Still not exercised: a swap between two real ChatGPT accounts with live limits, and keyring-only homes.
Upstream oddity worth a separate report: the app-server login RPC reads cli_auth_credentials_store_mode from a snapshot that may differ from the auth manager's store mode frozen at construction.
Stacking
Depends on #29507 (refactor/codex-home-env-helper): CodexCredentials.ts calls withCodexEnvironmentHome. Merge that first.
This branch is stacked directly on top of it, so until #29507 merges, this PR's diff includes that commit's six files (home-derivation refactor, no fingerprint logic in them). After #29507 merges, GitHub collapses those commits and this PR shows only its own six files. Rebase target after the merge: current main.
CodexDriver, codexUsage, and AgentSessionScanner each carried their own
copy of the 'environment CODEX_HOME stands in for blank settings homes'
ternary, and CodexAdapterV2 omitted the substitution entirely, so its
settings home could diverge from the home the driver spawns with. All
four now go through withCodexEnvironmentHome in CodexHomeLayout.
The helper assigns the raw env value and trims only for the blank gate,
which the existing padded-path fixture in CodexDriver.test pins. The one
intentional behavior alignment: the adapter now honors env CODEX_HOME
for blank-home instances, matching the routes already honoring it.
codexUsage keeps its managed-mode guard at the call site; the adapter
hoists its construction-time environment merge once so derivation and
the environment slot share it.

External account changes (codex login, an app swapping auth.json) were
invisible to a running app-server: it caches auth at spawn, only
same-account reloads are adopted in-process, and the old session kept
serving turns on the swapped-away account whenever the old token still
worked. Issue pingdotgg#23720 is the loud variant where refresh fails every turn;
the silent one has no open PR (pingdotgg#28108 scopes external swaps out).
Adapters may expose resolveCredentialFingerprint, a tri-state probe
(identity string; null while the provider surface carries no file
credential; undefined when unobservable). ProviderSessionManager records
the fingerprint at open and releases a live session on any change, with
reason manual_shutdown, detail 'Provider credentials changed.', and an
orchestration-v2.provider-session-released log since release details are
not persisted; the released session respawns and the fresh app-server
re-reads auth.json at spawn. Keyring-only swaps leave no disk signal,
a documented limitation.

Walkthrough
Codex home resolution now applies effective environment settings. Codex adapters can expose credential fingerprints. The session manager checks fingerprints before reusing sessions and handles changed credentials according to session activity and pending work.

A shared helper applies a nonblank environment home when configured home paths are blank. Home resolution uses it in the Codex driver, usage reader, session scanner, and adapter setup. The scanner resolves relevant secret-backed home variables and uses Windows-specific case-insensitive matching.
Resolve and expose Codex credential identity packages/provider-core/src/server/ProviderAdapter.ts, apps/server/src/provider/Drivers/CodexCredentials.ts, apps/server/src/provider/Drivers/CodexCredentials.test.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts
The adapter contract adds an optional credential fingerprint probe. Codex resolves the effective auth file and hashes an account ID or API key. The Codex adapter exposes the probe outside managed setup mode.
Refresh sessions when credentials change apps/server/src/orchestration-v2/ProviderSessionManager.ts, apps/server/src/orchestration-v2/ProviderSessionManager.test.ts
The session manager records fingerprint baselines and checks them before reusing a runtime. A changed fingerprint can trigger a release; busy sessions and sessions with pending background work are retained.

Explanation
The description follows the required template and includes concrete reproduction steps, a source-level explanation, verification results, and the model and harness. The Scope and approval section does not link a triaged bug issue or maintainer discussion that explicitly approves the direction and scope. It cites issue #23720, prior PRs #11147 and #24856, and maintainer PR #28108, but it does not include a maintainer approval comment. The verification section reports typecheck, focused suites, and an end-to-end dev-server run with a log excerpt. It also identifies unverified cases: swaps between real ChatGPT accounts with live limits and keyring-only homes. The check fails because approval for the external-credential-swap fix is not documented.

…awns
CodeRabbit flagged a startup race: the baseline was probed after
adapter.openSession returned, so an auth.json swap between spawn and probe
recorded the new identity while the running process still held the old one,
and every later reuse then matched and never released — the exact staleness
this PR fixes. Probing before the spawn pins the baseline to be at most as
new as the process identity, so the same swap now releases on the next open.

apps/server/src/orchestration-v2/ProviderSessionManager.ts:1190-1261 Minor · Data Integrity & Integration · Quick win
Do not adopt a readable fingerprint when the runtime baseline is unknown.
If the pre-spawn read fails, the app-server can load account A before auth.json changes to account B. The first readable reuse probe then records B and keeps the A runtime. Later probes compare against B, so the stale runtime is not released.
Keep the current behavior when the current probe is undefined. When the current probe is readable and the baseline is unknown, route it through the existing release path instead of adopting it.

…ion discovery
On Windows the spawn merge resolves provider instance variable names
case-insensitively and skips unresolved secret values, but session discovery
matched exact spellings only. An instance configured with e.g. Codex_Home
started Codex in the instance home while discovery looked at the host home,
so sessions written there never showed up for import. The lookup now mirrors
the merge: last string-valued match, case-insensitive on Windows. A scanner
test pins the mismatched spelling against a win32 host platform.

Only makeCodexAdapterV2 was under test, so the derivation createCodexAdapterV2
performs had no coverage. Four cases now capture the client-factory open input
through the real create path and assert the settings home and environment
CODEX_HOME handed to the app-server spawn: blank deference, host CODEX_HOME
adoption, instance-over-host precedence, and settings-over-environment
precedence.
CodeRabbit (Major) on PR pingdotgg#29507.

The open-time fingerprint probe can fail, which leaves the stored baseline
unknown. The change sweep then adopted the first readable fingerprint, so a
runtime started before an external account swap kept serving after it. A
readable probe now releases that session; the next open spawns a fresh one.
Tests cover both the unknown-baseline release and a swap landing inside the
startup window.
Fixed in 3d81e20, on top of the merged refactor branch. The adoption branch is deleted. When the open-time probe fails, the baseline stays unknown, and the first readable reuse probe now goes through the existing release path instead of being recorded, so the runtime started before an account swap is released and the next open spawns a fresh one. An unreadable current probe still keeps the session.
Tests: the old pinning test now asserts the release on the first readable fingerprint (a later change still recycles), and a new case puts the swap inside adapter openSession to confirm the pre-spawn baseline does not absorb it.
The Test Web failure on this head is #22324's known runner flake, not this branch: the one failing test is the MessagesTimeline layout check 'keeps the worked-for heading in place when expanded at the bottom', whose log shows the ResizeObserver undelivered-notification signature from that issue, and this diff touches two server files only. Everything else passed: 8 server shards, unit tests, build, typecheck, lint. A rerun from the repository side should go green.
3955-4006: Trivial · Maintainability & Code Quality · Low value
Delete or fold this test into the "unchanged" test.
This test sets credentialFingerprint to "fp-alice", which is the value it already holds. The setup and assertions match the test at lines 3850-3898: two open calls, the same runtime returned, openCount of 1, and closeCount of 0. The extra Ref.set changes nothing the manager can observe, so the test adds no regression coverage.
Remove this test. Alternatively, add the Ref.set(credentialFingerprint, "fp-alice") step to the first test.
As per path instructions: "Request tests only as those rules allow." As per coding guidelines: "Extend before you add. When an existing test or table covers the behavior, add a row or an assertion there."

Reviewed and fixed in b7a2596: the re-read-to-same-value test is deleted. It set the Ref to the value it already held, so the probe the manager sees is byte-identical to the unchanged test's; the deleted case added no matchmaking the unchanged test misses.
Merged current main without conflicts in c072045. The PR's 13-file diff is unchanged. Confirmed the existing fixes for pre-spawn credential capture and release-only logging, and resolved the already-addressed logging thread.
Blacksmith vp run check:changed github/main passed: 327 tests across 8 files, typechecks across 25 packages, lint, format, knip, and GraphQL checks. Two independent scoped code reviews approved this head. The previous CI failure was the unrelated MessagesTimeline browser test with pointer interception and ResizeObserver errors, consistent with the open runner-flake issue #22324; no unrelated web code was changed.
A live Codex account swap through the client remains unverified. Macroscope's human approval requirement remains for maintainer review.
