Keep the second file's preview when a legacy entry has no recorded change.

When the first recorded change is an add or delete, toolCallPatch in apps/web/src/lib/toolCallDiff.ts returns the patch unchanged only if every parsed file matches a recorded change. The adapter records changes only from entries that have a changes array, so a legacy oldText/newText entry adds no recorded change. Its file therefore fails the match. The consumer then rewrites the whole two-file patch as content of the first file, and the second file's preview is lost.

The fix belongs in the consumer's match logic or in the producer's change recording. Either change must keep the existing protection for single-file patches whose paths do not match. Add a test for this order: an add or delete entry first, followed by a legacy entry.

Review comment at @packages/provider-acp/src/server/adapter.ts at line 1150:
Update the change-recording or patch-match logic used by the adapter and
toolCallPatch so a leading add or delete followed by a legacy oldText/newText
entry preserves the second file’s preview when that legacy entry has no recorded
change. Keep the safeguard that rejects a single-file patch whose path does not
match, and add a test for the specified entry order.
