fix(provider): preserve ACP process exit diagnostics in failed runs - #25693
An ACP child can exit with an error and useful stderr while the failed run shows only Provider turn failed. The runtime already retains the process status and stderr; the failure formatter and native request logger discard them. Related: #19689.
- Provider turn failed.+ ACP process exited with code 23+ Synthetic configuration failed.
Keep the validated exit code or signal and a redacted stderr tail in the existing failure message and code fields. Record the same diagnostic for failed native ACP requests. The failure message stays within 4,096 characters, retains the latest diagnostic, and never reads an arbitrary cause message.
Before exposing stderr, omit its leading line when the raw tail reaches its cap: that line might begin inside a credential whose prefix was discarded. Extend the existing token redaction to keep underscore-containing API key bodies covered, and cover standalone ghp_ and xox?- tokens in the final failure redactor. For ACP stderr only, withhold whole lines containing credential or key markers, matched case-insensitively after Unicode normalization. This deliberately withholds harmless lines that only mention those markers too. The whole-line rule applies only to ACP stderr. The added standalone-token redactions also apply to other providers’ failure messages. The underscore correction overlaps #24015 and is necessary for this diagnostic to be safe; it can disappear from this diff once that fix lands.
This is one focused fix under CONTRIBUTING's obvious-bug exception: preserve termination information T3 already captures. No automatic retry, provider switch, client contract or application setting changes.
Evidence
Before: a made-up ACP child exits 23 after printing a synthetic configuration error and fake credential. The Antigravity adapter emits a failed terminal event with Provider turn failed. and a null code. Eight targeted regression cases failed with production changes reverted before the Unicode-tail case was added. The prefixed/escaped-credential and whole-line regressions each failed before their respective corrections.
After: the real-child adapter test receives code 23 and the redacted diagnostic. Focused coverage also checks signal exits, nested causes, bounded tails, Unicode boundaries, discarded credential fragments, native logging and synthetic token formats.
Focused command (Node 26.12.1, pnpm 12.14.0, Vite+ 1.4.0):
vp test run \
packages/provider-core/src/server/failure.test.ts \
packages/provider-acp/src/server/nativeLogging.test.ts \
packages/provider-acp/src/server/stderr.test.ts \
apps/server/src/provider/acp/AcpJsonRpcConnection.test.ts \
apps/server/src/orchestration-v2/Adapters/AntigravityAdapterV2.test.ts
62 tests pass, exit 0, using a canonical temporary directory.
Observed synthetic-child result:
class: provider_error
code: "23"
message: ACP process exited with code 23
Synthetic configuration failed.
[REDACTED credential diagnostic]
Required pre-push vp run check:changed upstream/main: all steps pass (format, lint, both knip passes, GraphQL generation check, typecheck of 21 affected/dependent packages, and the same 62 tests), exit 0 at 27c2377ec76d9f91f1bdbbaacdab9930b7679d55. upstream/main is the comparison ref because this branch targets the upstream repository.
pnpm --filter @t3tools/provider-core --filter @t3tools/provider-acp --filter t3 typecheck: exit 0. Changed-file vp lint and vp fmt --check (all ten files): exit 0.
The default macOS temporary-directory alias causes the existing file-containment test to fail identically on untouched main. The focused suite passes with a canonical temporary directory. No file-containment code is changed.
Restatement search: searched docs/ and failure-formatting callers for Provider turn failed, AcpProcessExitedError and stderr. The existing exit error carries the fields; no guidance states that generic process failures are intentional.
Line count: 71 added/removed production lines across four production files; remaining changes are focused tests and the mock child fixture.
Sources: repository CONTRIBUTING.md, AGENTS.md, development and Effect service guidance; the pinned Effect 4.0.5 reference; Vite+ test documentation.
Not verified in a running desktop/mobile client or against a real provider account. All examples, process output and test identifiers are synthetic.
Merge Danger
Door: two-way for code. Reverting cannot recall failure text already stored or delivered.
Blast Radius: Providers
The exit diagnostic and whole-line filtering affect ACP failures. The standalone-token redactions apply to all provider failure messages. Existing failure fields reach all clients. Consequential risk: provider stderr may contain credentials, so the change requires careful redaction and bounded output. The raw buffer remains bounded; the complete leading line can be omitted conservatively at exactly the cap.
Could break: unrecognized formats, including multiline secret values, cookies, passphrases, unlabelled tokens, obfuscated labels and non-HTTP URL credentials, could reach a persisted failed run or native log. Known-pattern redaction cannot guarantee arbitrary provider text is free of secrets. Whole-line withholding also removes useful diagnostics mentioning a key or token, including startup exits that previously showed stderr.
Undo: revert this patch to restore generic failure reporting. Existing run history and native log entries remain. If an unrecognized credential format reaches them, rotate that credential. This patch provides no historical-record deletion or recall mechanism; reverting does not remove those records. No schema migration is needed.
Review dispositions: field-name pattern lists missed a new name shape each round. After reproducing prefixed, suffixed, camelCase and multiword authorization leaks, changed the ACP stderr method to withhold entire credential-labelled lines. Restored the general failure-field redactor, keeping this stricter method ACP-only. Corrected the rollback limit and disclosed unknown bare-token/multiline formats. Exact-cap test advice is skipped: clipped and unterminated real-child cases cover the privacy decision. Retry guidance remains unchanged in scope: a specific exit diagnostic replaces generic wrapper guidance. Required pre-push checks were run and passed at the named exact head. Skipped (optional): the late test-order strengthening, stale domain-error docstring, neutral marker wording and extra server warning logs; they are outside the demonstrated failure. The optional loss of an IPv6 startup hint when its marker falls in a clipped leading line is accepted with the documented conservative clipping tradeoff. The affected-package checker reported its scripts typecheck modified its own cache inputs; all checks still passed. No extra credential-pattern expansion is included; residual formats are disclosed above. Maintainers decide contribution eligibility; no claim of approval is made.

Opus found the description incorrectly said other providers’ failure messages were unchanged. Corrected it: whole-line withholding is ACP-only; the two standalone-token redactions also affect other providers. The original verdict is preserved. Both reviews found no source defect, credential leak or regression in this focused diff.
Both independently read the complete diff, relevant source/callers, tests, prior findings and verification evidence. They inspected tests rather than executing them. They confirmed the consequential credential-exposure risk and the synthetic-only public examples. Earlier credential findings are resolved. Optional advice and residual-format limitations are disclosed in the description. The required pre-push check passed at this exact source head, including all 62 focused tests and 21 affected/dependent package typechecks. No runtime installation is claimed.

Walkthrough
ACP process exits now produce failure details with validated exit information and sanitized stderr. Request logs include failure data for these exits. The mock agent and Antigravity integration test cover a prompt-triggered exit.
Provider failure construction recognizes ACP process-exit causes, redacts and bounds stderr, and includes a valid exit code or signal. Tests cover wrapped causes, redaction, and truncation.
The mock agent exits with code 23 when enabled and a text prompt contains "crash now". The integration test checks that the failed turn includes the exit code and synthetic stderr while excluding the fixture token.
pingdotgg/t3code#23654: Both changes handle Antigravity ACP process failures; this PR preserves exit details and redacted stderr in failure reporting.
pingdotgg/t3code#24015: Both changes update ACP stderr token redaction, including GitHub token formats.

Add the other GitHub token prefixes to the standalone-token redaction.
The pattern matches only ghp_ and xox?- tokens. Fine-grained PATs (github_pat_) and the gho_, ghu_, ghs_ and ghr_ tokens pass through unchanged.
Line-level withholding in acpProcessExitDiagnostic does not catch them either. A line such as Error: github_pat_11AAAA... contains none of token|secret|pass|credential|authorization|key. The token then reaches the persisted failure message.
SECRET_TOKEN_PATTERN in packages/provider-acp/src/server/stderr.ts (Line 9) has the same gap. Apply the same change there.
Based on learnings: GitHub credential strings use the fixed prefixes gho_, ghp_, github_pat_, ghu_, ghs_ and ghr_. Redaction logic must handle each of them.

Keep raw ACP stderr out of the failure message, or document a narrow exception.
acpProcessExitDiagnostic places redacted child-process stderr lines in the message of the provider failure. makeProviderFailure returns that message after bounding it. nativeLogging.ts also includes the same failure object in the native request-log payload for AcpProcessExitedError.
docs/internals/effect-services.md requires attributes and log annotations to exclude raw output. Its only listed exception is DeviceBootError.diagnostics, and the doc states that other service errors keep the rule. Redaction and the length cap lower the exposure, but the excerpt is still raw stderr. Choose one of these changes:
Keep only the exit code or signal in the failure message and omit the stderr excerpt.
Add a documented ACP exit-diagnostics exception that states its scope, the redaction it depends on, and the length cap.
