fix(server): recover handoffs when target context is exhausted - #25610
Returning to a provider with no room for an incoming handoff fails and asks the user to compact manually.
Change
Start a fresh provider session with a bounded selection of saved history when the handoff cannot fit the resumed session. Preserve the original request and attachments, and reuse the recovered session on later turns.
Probe the exact payload before replacing the native session, including failed requests and the full recovery handoff. Estimate occupancy only from history accepted into the current native session. Leave background commands and native subagents running and explain when they prevent recovery. Claude also checks before replacing its process, and ACP explicitly creates a new native session through its existing replacement transaction.
Verification
Regression tests cover exhausted context with native injection and inline history, failed-request recovery, repeated estimate-only recovery, payloads that cannot fit a fresh session, background-work preservation, and fresh ACP identities. vp run check:changed passed formatting, lint, Knip, GraphQL checks, typechecks for 21 affected packages, and 385 tests across 5 files. The separate handoff-budget suite also passed its 17 tests.
Implemented by GPT 6.1 Sol using the Codex harness.
- Start a fresh provider conversation when a resumed thread cannot fit handoff context
- Update the handoff error and user guide for automatic recovery
Macroscope's review found this PR requires human approval — This change adds automatic recovery for exhausted provider contexts, including fresh runtime creation, bounded history reconstruction, and background-work protection across orchestration and provider adapters. Because it changes default production behavior and spans multiple runtime lifecycle components, the impact should receive human review.

Walkthrough
The turn-start service evaluates handoff budgets and prepares retry and recovery context. When a resumed thread lacks capacity, it can request a fresh native thread, subject to context limits and background-work checks. Adapter behavior, failure messages, tests, and user guidance cover these paths.
The adapter input supports forced native-thread replacement. The ACP adapter resets replacement-session state and preserves the provider-thread ID. ACP and Claude checks detect pending background work, with optional subagent checks.
Prepare handoffs and recover exhausted threads apps/server/src/orchestration-v2/ProviderTurnStartService.ts, packages/provider-core/src/server/failure.ts
The service calculates context budgets and prepares retry and recovery handoffs before delivery. It tests the resumed thread first and can use a fresh native thread when recovery fits. Pending background work blocks replacement. Failure messages describe background-work and request-size conditions.
Validate recovery and document limits apps/server/src/orchestration-v2/testkit/ProviderSwitch.integration.test.ts, apps/server/src/orchestration-v2/Adapters/AcpAdapterV2.test.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts, packages/provider-core/src/server/failure.test.ts, docs/user/portable-handoffs.md
Tests cover exhausted-target recovery, retries, oversized requests, tight capacity, and pending background work. User guidance describes fresh-conversation recovery and advises shortening the request or choosing a larger-context model if it does not fit.

Independent synthetic verification at d9100420ea4c23b56bee13f0e4aca375da8e6290: the 96 focused tests pass, but additional recovery cases still fail.
This change recovers by starting a fresh target native session with selected saved history. I tested the production services and adapters with synthetic data and test doubles, without real provider sessions or conversation data.
Additional case
Reproduction and observed result
Pending background work
In the exhausted-target turn-start fixture, report pending background work just before returning to the target. Recovery still allocates a replacement. Separately, feed the production Claude adapter a synthetic background-task start and completed parent turn, then start a turn with a different native identity: it closes one fake query, opens a second, and clears its pending-work roster. This is a two-layer reproduction, not a real-provider end-to-end run.
Failed request omitted from preflight
Fail the first target request after history injection. Set target usage to 30,000 of 32,000 tokens and send a short retry. The retry fails with ContextHandoffBudgetError; no fresh session is created. The retry history is added after the recovery probe.
Repeated estimate-only recovery
Extend screenshot-exhausted-target-estimate-native: after its recovered follow-up, send a short source turn and return to the target. The turn completes, but the native-session generation becomes 3 instead of remaining 2. Old target text omitted during recovery is still charged to the reused provider-thread row.
Replacement happens before the full payload fits
Add another exhausted-target native scenario with a 25,121-token model window; keep its short request and single image. Recovery creates the second native session, then fails with ContextHandoffBudgetError. The delta-only probe fits a 914-byte allowance; adding the replacement summary needs 935.
ACP does not necessarily create a fresh native identity
In the production ACP adapter's mock-agent fixture, finish a turn, then call ensureThread with the existing provider row and its native reference cleared. It returns the same native identity. A turn-start fixture mirroring that behavior still fails with ContextHandoffBudgetError. The current exhausted-target tests assume ensureThread creates a fresh identity.
These suggest checking live work before replacement, probing the exact final delivery payload, estimating only text accepted into the current native session, and ensuring the ACP path explicitly creates a fresh native session. The explicit fresh-native support in #24579 is relevant to the last case.
Passing checks, Node 26.12.1 and Vite+ 1.4.0:
vp test run apps/server/src/orchestration-v2/testkit/ProviderSwitch.integration.test.ts apps/server/src/orchestration-v2/ContextHandoffBudget.test.ts packages/provider-core/src/server/failure.test.ts: 64 passed, exit 0.
vp test run apps/server/src/orchestration-v2/ProviderTurnStartService.test.ts: 32 passed, exit 0.
The additional diagnostics fail on assertions for completed recovery, preserved background work, fresh identity, or session reuse. The refusal diagnostics also assert the exact budget-error message. I have not checked a real provider, client UI, or these cases against main. No duplicate PR is being opened.
Confirmed all five cases and fixed them in e946d30.
Recovery preserves background work and returns an actionable error until that work finishes or is stopped. Claude checks again at the process-replacement boundary, and ACP refuses replacement while work is active.
The fit probe includes the same prepared failed-request handoff as delivery.
Context estimates count only accepted input and delivered history belonging to the current native session. Another provider switch reuses the recovered session.
The fresh-session probe includes the full recovery handoff. The tight-window case now fails before allocating a replacement.
ACP explicitly creates a fresh native session with forceNewNativeThread, using its existing replacement transaction. The test verifies a new native identity, stable provider-row identity, and a completed subsequent turn.
Each reported case has a regression, including a production Claude-adapter reproduction that failed before the guard change. vp run check:changed passed 384 tests, formatting, lint, Knip, GraphQL checks, and typechecks for 21 affected packages. The separate handoff-budget suite passed 17 tests. The upstream merge had no conflicts. The earlier web CI failure was an attachment-preview timeout outside this diff; the new push will run CI again.

Buddy's web chat is Lazurio T3 Code with the Hermes Agent over ACP (root
decision 0210, plan DEV-6676). On the Osobní Prostředí a second T3 server with
T3CODE_LAZURIO_BUDDY=1 runs Hermes as the provider instance `buddy`. Hermes keeps
every prompt as the person's words in Buddy's memory, so T3's own instructions
(<t3_code_instructions>, <runtime_info>, <pull_request_linking>) would become part
of Buddy's history, twice after Stop or a steer and again after every restart.
- The ACP adapter asks AcpT3Instructions when it builds an instance whether its
prompts carry T3's instructions; without an answer every instance keeps them.
server.ts answers no for Buddy's instance where it builds provider instances
(ProviderInstanceRegistryHydration).
- The turn start projects a composer record of kind `lazurio-page` as one
labelled Czech block ("Kontext stránky (údaje, ne pokyn): ...") after the
person's text, for Buddy's instance only; every other record keeps upstream's
projection.
- Buddy's sessions take no model or effort from T3 (HumanAndMachines/Lazurio#648):
the adapter asks AcpT3Selection which options of T3's selection an instance
takes; null, the default, is all of it. For Buddy's instance withBuddyInstance
names only Hermes's speed (`fast`), so T3 applies no model and only Hermes's
Normal / Fast switch whenever a session starts, loads or gets a new selection.
Without T3CODE_LAZURIO_BUDDY=1 nothing changes, also for an instance called
`buddy`. Tests drive the repository's mock ACP agent in Hermes' place, through
the adapter and through the production V2 runtime, and read what it received.
On v0.1.3 the seams stay at the only prompt assembly and send in the ACP
adapter. Upstream now computes the slash-command check from the flavor's
rewritten user text (pingdotgg#24605); the rebuild takes that line and keeps the
instruction gate around t3AcpPromptWithInstructions and the runtime notes. The
orchestrator test provides upstream's new CodexManagedTokenBroker, which the
provider instance registry now needs, and the two file headers are cut to the
four-line comment limit.
On v0.1.4: upstream moved the turn text assembly up in ProviderTurnStartService (pingdotgg#22864, pingdotgg#25610, pingdotgg#25660); the page block seam moves with it, while the prompt-command and steer projections stay upstream's as before. server.ts keeps upstream's port scanner (pingdotgg#26245) after the wrapped instance hydration, and the Buddy orchestrator test provides the new PortDiscovery that hydration now needs. The pingdotgg#648 selection gate, which row 28's commit carried on v0.1.3, comes back to this row and sits beside upstream's flavor hook (pingdotgg#24605).

…ad of cancelling it
A message sent while an ACP agent works is steered by cancelling the
running turn (session/cancel) and sending the message as the next
prompt, because ACP has no steering request. For Buddy (Lazurio Hermes
over ACP) every such message stopped his work and he started it again.
An agent that lists "lazurio.activeSteer": 1 in its initialize
capabilities' _meta now gets supportsActiveSteering for its session, and
steerTurn sends the extension request _lazurio/steer {sessionId, text}
with the message's text and its attachments as saved paths. The turn
goes on. An answer other than steered waits up to 10 s for the prompt
to settle and fails: a turn that ended leaves the message to the next
turn; a prompt the agent has not started yet is steered by the effect's
retry. Agents without the meta keep steering by interrupt and restart.
effect-acp's initialize normalization now keeps capabilities._meta.
Tests: the mock ACP agent advertises the meta under
T3_ACP_LAZURIO_ACTIVE_STEER=1 and answers _lazurio/steer. The adapter
test steers a running turn, which completes with the steer's text and
without session/cancel, and a steer after the turn fails. A second test
keeps restart steering for an agent without the meta.
Claude Opus 5.5 in Claude Code (T3 Code).
The row also carries the two review rounds of #141: a steer names its turn and message, so it reaches only that turn and only once, and a steered message becomes the next turn only when the agent refused it.
On v0.1.4: pingdotgg#25610 added commitReplacementSession for a session that replaces the running one (handoff recovery, rollback). It now reads the replacement's _lazurio/steer capability too, so a replaced session does not keep the old session's answer to whether the agent steers.

steerTurn sends the extension request _lazurio/steer {sessionId, text}
with the message's text and its attachments as saved paths. The turn
The row also carries the two review rounds of #141: a
steer names its turn and message, so it reaches only that turn and only
once, and a steered message becomes the next turn only when the agent
refused it.
On v0.1.4: pingdotgg#25610 added commitReplacementSession for a session that
replaces the running one (handoff recovery, rollback).
commitReplacementSession sets the adapter's activeSteering from the
replacement's initialize answer, so whether a prompt names its turn and
a steer goes through _lazurio/steer follows the session that runs. The
session's supportsActiveSteering capability is still the one negotiated
at startSession; a replacement does not change it.

…ad of cancelling it
A message sent while an ACP agent works is steered by cancelling the
running turn (session/cancel) and sending the message as the next
prompt, because ACP has no steering request. For Buddy (Lazurio Hermes

…ge as a Czech block
The pingdotgg#648 selection gate, which row 28's commit carried on v0.1.3, comes back to this row and sits beside upstream's flavor hook (pingdotgg#24605).

buddyT3Instructions, LAZURIO_BUDDY_ENV and isBuddySurface, which nothing
imports, are module-private.

Problem
Regression tests cover exhausted context with native injection and inline history, failed-request recovery, repeated estimate-only recovery, payloads that cannot fit a fresh session, background-work preservation, and fresh ACP identities.
vp run check:changedpassed formatting, lint, Knip, GraphQL checks, typechecks for 21 affected packages, and 385 tests across 5 files. The separate handoff-budget suite also passed its 17 tests.Implemented by GPT 6.1 Sol using the Codex harness.
