fix(opencode): show a readable message when a provider error is <none> - #24734
An OpenCode turn failed, and the thread showed a provider error whose text was <none>.
When a provider answers with a body the AI SDK cannot read, such as an HTML error page, OpenCode stores APIError with message: "<none>", an HTTP status, and that body. T3 used only the message, so the thread said <none>.
Change
When OpenCode's error message is the placeholder <none>, the failure shown in the thread uses the HTML page title and the HTTP status. A real provider message is left as it is. The same reading is used for the OpenCode 1 session error and for OpenCode 2 execution failures and retries.
Scope and approval
Very small fix of an obvious bug: the failure text was the placeholder <none> even though the event already carried an HTTP status and a response body. No new behavior besides replacing that placeholder. No prior issue.
Verification
CI gates for this diff are lint, desktop build, web tests, server tests, and tests for @t3tools/mobile, @t3tools/provider-opencode, and @t3tools/scripts. Rust, release smoke, and mobile native lint are skipped.
vp check: 0 errors, 366 existing warnings. knip:check and both GraphQL --check commands passed.
Web: 330 files, 3912 tests passed.
vp run build:desktop passed. The preload verifier passes when the platform is linux, which is what CI uses.
@t3tools/scripts: passed except build-npm-launcher, which compares a /var temp path with the /private/var path macOS returns. The desktop-artifact Windows probe passes when ELECTRON_RUN_AS_NODE is unset.
Server: 4468 passed, 17 failed, 4 skipped. The failures are in untouched files. They are macOS /var versus /private/var paths, a locally installed Antigravity app, or directory order. The OpenCode suites passed. Current main CI on Ubuntu is green.

Walkthrough
The OpenCode provider adapters now use a shared formatter for errors. It uses available message, HTTP status, and response-body details. Session updates and turn finalization use the same constructed failure.
Adds fallback handling for empty or <none> messages. The formatter can use a normalized HTML title or numeric HTTP status, and can return a generic message when needed.
Adapter error handling and tests packages/provider-opencode/src/server/adapter.ts, packages/provider-opencode/src/server/v2/adapter.ts, packages/provider-opencode/src/server/adapter.test.ts
Both adapters pass available error details to the shared formatter. Session updates and turn finalization use the same failure. Tests cover formatted messages and fallback cases. The v2 execution path retains its existing usage-limit classification.

Full details: Title check
Explanation
The title names the main change: when a provider error message is the placeholder <none>, OpenCode now shows a readable message. It matches the summary's change to readableProviderErrorMessage and the fallback to the HTML title and HTTP status. It uses the conventional commit format and fits the 70-character length.
Full details: Description check
Explanation
The description covers Problem, Change, Scope and approval, and Verification. The Problem and Change sections are clear. The Scope section says the fix is very small with no prior issue, as the template allows. The Verification section lists focused checks with observed results, including the 117 provider-opencode tests. It also states what was not checked, such as the macOS path failures and the skipped Rust and release checks. The template asks for a conventional commit title, and the title meets that. The description has no screenshots, but the change is not a UI change. The author names the model and harness at the end, as the template asks. Reviewer comments in the objectives say a live OpenCode 2 path and native clients were not verified, but the description does not claim otherwise. The description is mostly complete, so it passes.

Reuse the sanitized failure message for the legacy session update.
The V2 paths sanitize the title through makeProviderFailure. The legacy path sends the raw title to updateProviderSession first. That emits an unbounded lastError in a provider-session.updated event.

thanks @ricardo-rp. your shared fallback keeps this fix focused across both opencode versions. i preserved your commits, merged current main, and pushed 58215a0c414bbf9a5e342c470e65e5cf1e63c3d2 to this branch. the pr is now out of draft.
the only production adjustment to your implementation reuses the sanitized turn failure for the legacy session error. this fixes the review finding about credential-bearing titles and avoids constructing the failure twice. all original fallback paths remain.
verification:
five cases in the existing adapter test cover html titles, http-only errors, missing details, real messages, and url credential redaction. four failed on main; the real-message control passed. all 37 adapter tests pass with the fix.
blacksmith vp run check:changed passed format, lint, knip, five affected package typechecks, and tests.
real opencode 1.19.4, a controlled http endpoint, and the actual web client reproduced <none> before and Provider access denied (HTTP 418). after. matching banner crops below use the same thread, prompt, status, and html body. stored historical failures stay unchanged.
two independent gpt-6.1-sol reviews approved this exact head. the original sanitation finding is fixed and resolved. the automatic review on this head reports a rate limit.
unverified: live opencode 2, native desktop/mobile, remote/tunnel transports. blank-message and v2 retry/terminal entry points lack direct regression cases; these are nonblocking coverage limits.
merge is still blocked outside the patch: github has not started ci for this head, and the prior fork ci run requires approval. the required Check, Test, server shards, and other contexts have not reported. reported metadata checks pass; this is not a claim that ci is green. no labels were changed manually.
verification is incomplete for the opaque-error fallback. no source changes or pushes were made.
Verified in a real dev environment
inspected the description, discussion, diff, and authors. the pr is open; none of its commits is by juliusmarminge or t3dotgg. tested exact head 58215a0c414bbf9a5e342c470e65e5cf1e63c3d2.
installed and ran github/main (c0f9e36184) in /home/user/main first, then stopped its captured process group before running the pr. used isolated .t3 homes, vp i, and background vp run dev --no-seed. main used server/web ports 16733/8693; the pr used 13773/5733.
drove the actual web client with repository playwright and /usr/bin/google-chrome. added the real checkout as a project through the app, added opencode through provider settings, and sent real provider turns. no mocks, fixtures, demo data, or response interception.
codex gpt-6.1-sol returned PR24734_CODEX_OK on both revisions through the local proxy.
on the pr, opencode 1.19.4 using @ai-sdk/openai-compatible against https://api.openai.com/v1 with the deliberately invalid key invalid displayed the original authentication message in the banner and thread. it remained readable after reload and a second failed turn. the fallback did not overwrite a real provider message.
switched that same failed thread back to codex gpt-6.1-sol; it completed with PR24734_RECOVERY_OK. the app reported 1m 43s and provider retries. the 16-second recording omits the middle wait and labels that edit; it makes no performance claim.
blacksmith ran the existing packages/provider-opencode/src/server/adapter.test.ts: 37 passed. these tests cover the opaque message/title/status and sanitation cases, but do not replace live verification.
short transcript from the running pr server after browser-driven recovery:
the original <none> bug was not reproduced on current github/main. real requests through opencode to the chatgpt web endpoint returned Not Found: {"detail":"Not Found"}. a misconfigured provider pointed at https://openai.com retried and ultimately returned Forbidden: request was blocked by a gateway or proxy.... neither emitted <none>. therefore the html-title/http-status fallback, its credential sanitation, and before/after text change remain unverified through a real provider. i did not substitute a controlled error endpoint because this pass prohibits mocks.
opencode 2 execution failures/retries, native electron/mobile clients, and remote/relay/tunnel connections were not exercised on this linux web pass.
github currently reports passing metadata contexts and skipped unaffected jobs, including a rate-limited review marked successful. the main check/test/server suites have not reported for this head; this is not a claim of green substantive ci. check:changed was not run because this pass made no changes.
result: unverifiable for the central opaque-error change, with the neighboring real-provider behavior above verified. this does not establish that the patch is broken.

Bound the title scan before processing OpenCode error events.
OpenCode 1 forwards responseBody as an unrestricted string, and OpenCode 2 accepts response.body as a string with a 16 MiB SSE-event limit. A roughly 100 KB body with repeated <title prefixes and no > can therefore reach this synchronous regex in either adapter. On the supported Node server, this pattern can block the event loop for about two seconds and delay other work in the same process.

codex gpt-6.1-sol through the local proxy returned PR24734_CODEX_OK on both revisions. after the opencode failure, returned to the separate codex thread and received PR24734_RECOVERY_OK. this pr head requires a new thread to switch providers.
configured real opencode 1.19.5 with @ai-sdk/openai-compatible, a deliberately incorrect base url https://example.com/v1, and an unused key. its real http 405 html response produced Method Not Allowed on main and the pr. the pr preserved that readable provider message in both the banner and thread, after reload, and on a second failed turn. the 11-second recording shows that second turn and its failure without a cut inside the interaction.
blacksmith ran the existing packages/provider-opencode/src/server/adapter.test.ts: 42 passed, including the pr sentinel/title/status/sanitation cases. these tests are supporting checks, not live proof of the fallback. substantive github build/typecheck, lint, test, server shards, and web checks currently pass; unaffected jobs are skipped. check:changed was not run because this pass made no changes.

the screenshots below are the unchanged readable-error control on main and the pr, not a reproduction of the original opaque error.
Not verified
the central <none> fallback remains unverified through a real provider. the original bug was not reproduced on current github/main: the real html response emitted Method Not Allowed, not <none>. source inspection explains that opencode/ai sdk preserves the nonempty http status text. therefore this pass does not prove the replacement title/status text or credential sanitation in the running app. no controlled error endpoint was substituted.
opencode 2 execution failures and retries, native electron/ios/android clients, and remote/relay/tunnel connections were not exercised in this linux web pass.
result: unverifiable for the exact opaque-error change, with the neighboring live behavior above verified. this is not a finding that the fix is broken. the evidence was inspected; the uploaded video played to completion and seeking worked in t3 preview.
