🔴 Critical orchestration-v2/Orchestrator.ts:1458

After a real worktree handoff, the failed run’s provider session is absent from projection.providerSessions, so failedSessionCwd is undefined and workspaceMovedAway stays false; the queued continuation is then held as though the provider had failed. WorktreeMcpService reaches this path by dispatching thread.metadata.update, which detaches the session before the turn fails, and the projection no longer loads sessions without their bindings. Read the historical session independently of its current binding or persist the handoff signal before detaching.

Await queue promotion before asserting its result.

eventSink.write persists the failure events, but handleRunUpdate runs in a detached stream consumer. Effect.yieldNow does not prove that queue promotion or the test event consumer has completed. Await the queued run's persisted starting update, as the neighboring failure test does, before checking the projection and heldRunIds.

Review comment at @apps/server/src/orchestration-v2/runtimeLayer.test.ts at line
4582:
Replace the `Effect.yieldNow` synchronization in the queue-promotion test with
an awaited signal that the detached event consumer has persisted the queued
run’s `starting` update. Capture that run ID in a dedicated queue from the
`run.updated` stream, await it before reading the projection, and preserve the
existing `heldRunIds` tracking.
