…em index Red regression test for #21578: getRuntimeRecoveryProjection's provider-thread read looks up unfinished turn items without a type filter, so it falls back to turn_items_thread_run_idx and scans the thread's completed history. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
…shed work getRuntimeRecoveryProjection's provider-thread read looks up unfinished turn items without a type filter, so the type-filtered recovery index could not serve it and SQLite scanned the thread's whole item history. Add a status-only partial index on thread_id in a new migration so existing and fresh databases both get it. Fixes #21578 Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
ApprovabilityVerdict: Approved at Macroscope's review found this PR approvable — This is a narrowly scoped server performance fix that adds a partial index for existing unfinished-item recovery queries, with tests covering both query planning and migration registration. It does not alter product behavior, defaults, APIs, or stored data semantics. Notes:

itemLookups to capture both SEARCH and SCAN access for item, then assert that
each unfinished-item lookup uses a status-filtered index so a table scan cannot

Dismissing prior approval to re-evaluate 3aeff01
|
Fixed in 3aeff01: the plan check now includes both SEARCH and SCAN item entries, so a table scan fails the existing partial-index assertion. All 34 projection tests pass; independent review passed both affected files (39 tests). |

Closing: current main already covers this as of 624e70ecdd. If you still see it on a build that includes that commit, reply and we'll reopen. |
Problem
Starting a turn in a long thread repeatedly scans completed turn items while finding unfinished work, delaying other server requests. Reproduced on current main with 17,517 synthetic completed command items and a real Codex turn.
Change
Add a partial index on the thread ID for pending, running, and waiting turn items of every type. Register it as migration 68 so existing and fresh databases receive it. Keep recovery queries and results unchanged.
Scope and approval
Fixes #21578
GitCafe: https://git.cafe/pingdotgg/t3code/issues/21578
Triaged bug via the automated issue flow: #21578. Only the unfinished-item index and its regression coverage are changed.
Verification
Final test-only follow-up 3aeff01 also rejects table scans in the plan assertion. All 34 projection tests and targeted lint/format passed; independent review passed both affected files (39 tests), with both reviewers returning MERGE. Runtime measurements below remain from 521eeb4; production code is identical in the final head.
vp test run apps/server/src/orchestration-v2/ProjectionStore.test.ts -t "selects only threads with runtime state that needs recovery"fails on main becauseSEARCH item USING INDEX orchestration_v2_projection_turn_items_thread_run_idx (thread_id=?)has no partial status predicate. The same test passes with the migration.vp test run apps/server/src/orchestration-v2/ProjectionStore.test.ts apps/server/src/persistence/reconcileV2PreviewMigration.test.ts apps/server/src/persistence/Migrations: 64 tests in 19 files passed.vpr typecheckinapps/serverpassed.okbefore and after. Migration 68 applied successfully to the existing fixture database.Sanitized trace transcript, runtime proof, trace extractor.
This is a server-only migration with no changed render paths; the visual fixture matrix is empty. Measurements are indicative under shared VM load. Synthetic projection rows are performance fixtures, not event-replay proof; the after run reuses the thread containing the preceding successful turn. The reporter’s 15.8 GB database and exact latency were not reproduced. Creating the index incurs a one-time table scan during upgrade.
