fix(server): keep newer Claude wakes active during replay - #22998
A Claude wake can lose its active run while it is still working. An earlier wake leaves a result in the buffer, a success or an error. A queued prompt runs, and Claude opens a newer root system/init before the continuation attaches. Replay then settles the continuation with the older result, so delegation calls in the newer wake are refused with parent_not_active.
Change
During replay, a result before the last root system/init must not settle a successor that is still running. Keep the earlier wake's billed usage and error notices. Its text comes from its own assistant frames. Settle with the newer result when it arrives, including when the whole successor is already buffered. An earlier usage-limit error therefore becomes a notice, and the newer wake's result decides the run.
Keep the existing fallback to the last non-empty result when the latest buffered wake contains only empty zero-turn task-notification successes. A selected earlier result is billed once, at settlement. Earlier empty zero-turn task-notification successes are not billed, as on base. Live result handling, the settling result's text fallback and compaction keep their existing behavior.
Tests use the incident's init, assistant, result order and cover older success and error results, several older wakes, a fully buffered successor without an assistant frame, the last non-empty result fallback, lone buffered results, empty-wake billing, and a live empty success after a buffered rate-limit assistant error. Caller ownership checks are unchanged.
Scope and approval
Refs #16808. #16808 is not triaged, so this uses the obvious-bug exception: the fix changes only how the Claude adapter picks the replayed result, with no contract, setting or default change. No maintainer approval is claimed.
This fixes one buffered-result sequence that can cause the reported symptoms. It does not establish a fix for every case in #16808, or allow a native subagent that outlives its parent turn to create delegated work.
Against base 066c8fc0cd, with identical tests: all nine replay cases pass on head; four pass and five fail on base. The failures expose early settlement, missing successor output, a missing earlier error notice and omitted earlier usage. Existing quota and compaction tests, the lone-result cases, empty-wake billing and the live empty-success case pass on both.
The broader adapter, continuation-service and MCP run passes 432 of 434 tests on head. Base has 427 passed and seven failed: the five replay regressions and the same two fixture failures. On Windows, the two failures (reports distinct child results and reports identical child results) also fail on base: the fixture reader converts a file URL to an invalid path for codex_transcript.ndjson.
Server typecheck, changed-file lint (one existing unused-layer warning), both knip checks, format checks and diff checks pass. The branch merges cleanly with current main.
An open-PR search for wake, num_turns, continuation, 16808, replay and usage limit found no matching per-result replay fix among the returned results; some searches were capped at 100 results. A live Claude timing reproduction was not run.

An older buffered result can otherwise settle a continuation whose newer
wake is still running. Let the last root init own replay settlement.
Keep earlier billed usage and error notices once, while leaving text to
the assistant frames of each wake. Empty zero-turn task successes retain
base billing, and the settling result keeps the existing text fallback.
Cover the incident frame order, queued prompt, several wakes, buffered
successors, empty-result fallback, lone results and live empty success.

Macroscope's review found this PR requires human approval — This is a focused Claude replay fix with tests covering buffered wake ordering, failure notices, settlement, and usage aggregation. Because it changes production terminal behavior and token metering, the change warrants human review.

Walkthrough
Claude wake replay now selects a settling result based on the latest turn start. It records usage and provider failures from earlier results, excludes empty task-notification successes from earlier-result accounting, and adds tests for replay scenarios and interrupted-turn usage.

#23553 lets a native background subagent that outlives its root turn call delegate_task, by attaching the delegation to the latest settled run while that subagent is still pending. This PR covers a different trigger of the same parent_not_active symptom: a Claude wake whose continuation run is settled too early by an older buffered result (an earlier wake leaves its result in the buffer, a queued prompt runs, and Claude opens a newer root system/init before the continuation attaches). There the caller is the root agent in the newer wake, and no native subagent has to be pending, so the admission change in #23553 does not apply.
The two changes do not share files: #23553 changes the MCP service, the orchestrator and the shared pending-work helper, and this PR changes only replay settlement in ClaudeAdapterV2. They can land in either order.
Since #23553 will close #16808, I can open a separate issue for this wake-replay sequence, with the frame order from the incident, and point this PR at it. Let me know if that helps triage.
merged current main into the original branch without rebasing. resolved the token-usage conflict using main's aggregation and validation, which already retain earlier usage when the successor stops without a result. preserved the replay settlement guard, earlier error notices, approval ownership, and empty-success fallback. the pr now changes three files (385 additions, 28 deletions).

Associate buffered results with their wake before selecting settlement.
An older result can arrive after a newer root system/init on the same accepted querySession. Continuations reuse the existing query, and querySession.messages passes frames to the adapter in stream order. Query replacement rejects a different query, but it does not enforce result-before-next-init within the current query.
When replay drains a buffer containing that sequence, replayResultIndex searches the full buffer. The older result can therefore settle the newer wake, while the newer result is removed without handling. Track wake ownership when buffering, then select only the latest wake's result. Preserve earlier non-empty results through recordEarlierWakeResult for usage and failure notices.
