A follow-up prompt sent to an ACP session can fail with the raw “Agent is already processing. Use steer() or followUp()” error. Fixes #22660.
Change
Queue and retry only the exact recorded ACP busy rejection, preserving the original payload. Backoff increases from one second and caps at 30 seconds; the prompt stays queued until delivery or cancellation. The existing provider retry item shows the queued state. Stop prevents redispatch even when a retry is in flight; old agent output does not prematurely settle the notice. Other errors keep their existing behavior. No unsupported steering extension is sent.
105 tests pass across the scoped registry/shared ACP adapter and connection suites. Targeted lint and server/provider scoped typechecks pass. Two independent review rounds found three bugs in round 1; all were fixed and round 2 found no confirmed code defect. Final proof audit returned SUFFICIENT; an independent R5 rebuild at the fixture commit had zero normalized semantic differences. Final head adds only Knip entry declarations for the committed scripts; server-scoped unused-code verification passes.
Six before/after pairs run through the real isolated T3 server, authenticated public RPC, persisted projections, and a synthetic ACP v1 stdio provider (not actual oh-my-pi). Linux x86_64, Node v24.21.0. Before: 6a6c84f990. After runtime: eb303fe4b6. Head 27a924b004 adds the committed fixture/driver and guard tests; production source is identical to the after runtime SHA. The index includes source/script hashes and exact reproduction commands. No client render code changed; pixel captures do not apply.

R1 Recovery
2; completed → failed
4 including fresh turn; completed → completed → completed
3 including fresh turn; completed → interrupted → completed
R3 paired transcript. No outstanding prompt at Stop; cancel emitted; notice interrupted with no turn failure; count held at 2 for 60.002 s, then fresh independent turn.
4 including fresh turn; completed → interrupted → completed
R4 paired transcript. Third prompt held; cancel precedes its exact busy response; no resurrection; count held at 3 for 60.005 s, then fresh independent turn.
R5 Old root thought
2; follow-up failed; late thought emitted and not attached to failed run
3; both turns completed
R5 paired transcript. After queued notice persists, retry is held before acceptance. Old root thought produces a running reasoning node while retry notice stays running. Gate snapshot shows no premature terminal/recovery. Final getTurnItem returns exact thought text; reply completes and notice settles.
R6 Unrelated error
2; completed → failed
2; completed → failed
R6 paired transcript. Another internal error retained; no retry item or retransmission through the 2.2 s observation window.
Observed differences: busy errors queue instead of failing, the same payload reaches eventual acceptance, retry notices stay active through prior output and settle on completion/Stop, and stopped threads accept fresh prompts. Prior background reasoning now remains attached to the queued follow-up, where the failed baseline did not attach it. Unrelated errors still fail without retry.
Limits: real oh-my-pi lifecycle and Windows desktop remain unverified. Client rendering, other ACP providers, remote/relay/tunnel topology, and restart durability were not exercised. The queue is in memory. The retry notice can remain visible while an accepted prompt answers because ACP v1 has no acceptance signal. Stop forwards the existing native cancellation, including to previous background work.
All capture-owned processes stopped and the temporary before checkout was removed; cleanup record is in the evidence index.

With a relative --output such as evidence, the fixture writes its wire log relative to the session workspace instead of the driver's evidence directory, so verification fails because that directory is missing. commandArgs inherits the relative path, and the ACP runtime launches the fixture with the workspace as its cwd; resolve values.output before deriving paths from it.

The ACP session runtime retries prompts after exact busy errors. The adapter reports retry state and preserves busy-queue retries through root-session output. Replay tests and a live verifier cover retry, cancellation, recovery, and output timing.
Changes: ACP Busy-Prompt Retry
Layer / File(s)
Summary
Runtime retry and cancellation packages/provider-acp/src/server/AcpSessionRuntime.ts
The runtime retries matching busy errors with delays capped at 30 seconds. It reports retry details through onBusy and stops queued retries when the prompt is cancelled.
The adapter reports busy-queue retries with retry metadata and does not complete them through the root-session output path.
Retry and cancellation tests apps/server/src/orchestration-v2/Adapters/AcpRegistryAdapterV2.test.ts
Replay-session tests cover retry timing, cancellation, old output, prompt payloads, and errors that do not match the busy condition.
Live busy-lifecycle verification apps/server/scripts/fixtures/acp-busy/agent.mjs, apps/server/scripts/verify-acp-busy-live.mjs, knip.jsonc
The fixture provides configurable busy-prompt behavior. The verifier exercises retry, cancellation, recovery, output timing, and final run outcomes. Knip lists both scripts as server workspace entry points.
The busy-prompt retry change does not compile: a duplicate import in the ACP session runtime breaks the server build, lint, and tests. It needs that one-line fix before it can merge. A narrow timing issue also remains. Stopping a queued follow-up just as a retry begins can still send that prompt to the agent.

Full details: Linked Issues check
Explanation
Issue [#22660] requires the ACP registry provider path to preserve a normal message when the agent returns the busy rejection. AcpSessionRuntime retries only code -32603 with the exact recorded error message. It reuses the same session/prompt payload and applies 1, 2, 4, 8, 16, then 30 second delays. It keeps the prompt queued until delivery or cancellation. adapter.ts exposes the queued retry state and prevents earlier root output from completing the notice. Stop cancels queued and in-flight retries. The scoped tests cover recovery, long busy periods, cancellation, prior output, payload equality, and unrelated errors. The implementation does not send an unsupported steering extension.
Full details: Out of Scope Changes check
Explanation
The production changes remain within issue [#22660]. AcpSessionRuntime.ts and adapter.ts implement busy rejection retry, queue state, cancellation, and notice handling. The replay tests, synthetic ACP fixture, live verifier, and Knip declarations provide coverage and tooling for this behavior. No unrelated production feature is established by the reviewed changes.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win
Check cancelRequested immediately before each session/prompt write.
cancelRequested is checked only after a busy rejection. A Stop can still arrive in one narrow window, and the retry then sends the prompt after the Stop.
The failing sequence:
The backoff sleep ends.
The fiber sees phase !== "cancelling" and sets phase = "dispatching".
The fiber yields inside runLoggedRequest, which runs logRequest before it calls acp.agent.prompt.
cancel runs during that yield. It sees "dispatching", so it skips the queued path.
The adapter passes interruptPromptOnCancel: false, so cancel sends session/cancel and does not interrupt the fiber.
The retried session/prompt can reach the agent after session/cancel.
If the agent became idle in the meantime, it accepts and runs the user's message after Stop. The PR objective says a stopped prompt must never be dispatched again.
The window is small. Each backoff cycle opens it again, and the loop has no attempt limit.
cancel sets cancelRequested synchronously before it writes session/cancel. A lazy check right before the RPC call therefore covers every interleaving:
If the check runs after cancel set the flag, the prompt is never sent.
If the check runs first, the prompt is queued ahead of the cancel, and the agent cancels it.

Remove the duplicate Result import. It breaks build, lint, and test.
Line 20 adds import * as Result from "effect/Result". The file already declares Result at line 13. The parser reports "Identifier Result has already been declared". As a result, CI build, lint, and every test shard that loads this module fail.
🐛 Proposed fix
-import * as Result from "effect/Result";
As per the pipeline failures, CI / Build and Typecheck, Lint, and Test all fail on this duplicate declaration.
