fix(server): cancelling a background-only delegated task reports it interrupted - #21676
task_cancel could stop a delegated child's background work after its turn completed while task_status still reported completed and the parent task stayed running. fixes https://git.cafe/pingdotgg/t3code/issues/16739.
cancellation now commits cancelledRunId with the child's stop and interrupt outbox, then settles completed work as interrupted. it preserves published results, failures, and unrelated questions. parent and child locks exclude new delegation during cleanup; direct remote descendants dispatch under the held child lock, and terminal-task cancellation uses the same locking path.
verification: blacksmith vp run check:changed passed format, lint, knip, graphql, typechecks for 32 affected packages, and 136 tests across 10 files. the new remote-descendant case reproduced a lock hang before the fix and passes afterward. real codex and the web client verified a finished intermediate turn with a running nested sleep 3600: cancellation returned interrupted / result_available, retained WAITING_FOR_BACKGROUND_CHILD, and persisted its cancellation run id. cancelling an already-completed task preserved its completed status and published result; the cancelled task remained interrupted.
unverified: live remote environments, claude native background work, electron, mobile, and relay/tunnel connections. existing integration coverage exercises provider background work, refusal, follow-ups, checkpoint races, and restart recovery.
…nterrupted
task_cancel on a delegated task whose turn had ended while its background
shell or native subagent kept running stopped that work but recorded
nothing. task_status then reported the task completed with the child's
interim reply, and the parent's subagent row stayed running because no
later child run ended to finalize it.
task_cancel now records cancelRequestedAt on the task before it stops the
child thread. cancelDelegatedTask writes it under the parent lock, only on
an original task whose result is not yet available, runs the child's Stop
under the same lock, and drops the field if that Stop fails. Every finalize
honors it: a cancelled task settles as interrupted once its turn ends,
without waiting for the work the Stop is ending, and task_status derives
the same status until it settles. A follow-up starts without the field, and
a terminal task keeps its result.

…ss a restart
Settling a background-only task as interrupted left the child run
completed, so the async-question cleanup, which keys on the run's
outcome, kept the task's user_input questions pending. It now uses the
effective outcome of a cancelled original task, with the same run
ownership and restart-continuation checks.
The cancel integration test now runs on a file database. Each case
restarts on it and runs startup recovery, then checks task_status and
the parent row. A new case records the cancel and stops the child, but
exits before anything settles the task, and startup recovery settles
it. The cases also check that the task's own question is dismissed and
an unrelated one stays pending.

…ow-up's
The original task keeps cancelRequestedAt, and question cleanup applied it
to whichever run of the child finished last. A follow-up that completed
after the cancel then had its own async question dismissed.
Cleanup now applies the cancel only to a run that started no later than
the cancel was recorded. A later run uses its own outcome, so a completed
follow-up keeps its question pending. The follow-up test now covers that.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
Matching the stopped run by start time still caught a later follow-up
that started in the same millisecond the cancel was recorded, and missed
a restart continuation of the stopped run that started after it.
cancelDelegatedTask now records the stopped run as cancelledRunId with
the cancel. Question cleanup applies the cancel only when the latest
run is that run or a restart continuation of it. A follow-up starts with
the field cleared. The after-cancel follow-up test now records the cancel
at the follow-up's own start time.
Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
task_cancel recorded the child's last non-queued run, which could be a
monitor notification run. Task progress skips monitor runs, so the task
settled from its own run while question cleanup looked for the monitor
run, and the task's question stayed pending.
The cancelled task now records the run it settles from, which comes from
task progress. Question cleanup matches that run. A row read before the
call that settles it has no run yet and matches the same result run.
Finalize and cleanup use plain task progress again: a cancelled run still
capturing its checkpoint settles when the capture finishes. A new cancel
case runs a monitor notification after the turn.

The task_cancel section said a cancelled background-only task always
settles as interrupted. Only a completed result maps to interrupted; a
failed result stays failed. Say so.

Macroscope's review found this PR requires human approval — This production fix changes cancellation semantics across MCP status reporting, persisted orchestration state, parent/child locking, finalization, and restart recovery. Although the change is well-covered by focused integration tests and uses additive schema fields, its cross-layer runtime impact merits human review.

Cancelling a delegated task in an uncommon state, where the child run is already being interrupted and Stop has nothing further to do, may still report the task as completed rather than interrupted. The common cancellation paths are covered by the new tests. The fix is a one-line guard, and the risk is bounded.
Security architecture risk: Moderate · up to af606
Cancellation remains restricted to owned tasks and permitted execution modes. However, partial cancellation can now settle a task as interrupted and suppress its completion notification before all delegated work has been stopped, weakening failure containment.
Retained concerns
Medium · reliability · inferred: For an eligible original task whose child turn has ended, cancelDelegatedTask can finalize the task as interrupted and dispose its completion delivery before recursive descendant stopping executes. If a descendant stop subsequently fails, including refusal after a concurrent execution-mode increase, task_cancel returns an error but the original task can remain terminal with no completion wake while descendant work remains active. Before this PR, explicit delivery disposal followed successful recursive stopping, and ordinary original-task finalization waited for dependent work. The new ordering weakens cancellation failure containment without bypassing the descendant's authority checks.
Security review details
Security Blast Radius
inferred — The independently reachable cancellation scope remains the caller thread's app-owned delegated task and its delegated subtree. The added metadata and settlement path do not establish broader tenant, environment, credential, or infrastructure authority. Partial cleanup can nevertheless leave already-authorized descendant execution active after the selected task becomes terminal.
Security Findings and Attack Paths
inferred — No verified unauthorized cancellation path was established. The supported failure scenario requires an authorized cancellation of an eligible original task followed by descendant cleanup failure; its consequence is inconsistent cancellation visibility and notification suppression, not demonstrated privilege escalation.
Trust Boundaries and Controls
observed — Thread-scoped invocation and orchestration capability are required before task lookup. A supplied task ID must belong to that exact parent thread. Cancellation recursively checks execution modes before stopping and carries DispatchModeLimit into dispatch to refuse concurrent mode escalation. Cleanup refusal is returned as an error rather than acknowledged as successful cancellation.
Resilience and Maintainability Implications
observed — Run-scoped cancellation cleanup preserves unrelated pending questions, and restart tests cover stable interrupted settlement. The mode-refusal unit test substitutes a pass-through cancellation helper, so it does not establish that real cancellation metadata and delivery state remain consistent when descendant cleanup is refused.

A follow-up whose turn ended while its background work ran was rejected by
the cancel guard, so it settled `completed` with its interim reply and kept
its async question pending. The guard now records the cancel on any task
whose own result is not yet available, and question cleanup binds to the
task that owns the settled result run. cancelDelegatedTask gets a named
Effect.fn span.

read its starting sequence after the provider events were already
committed, so it never saw them. Read the cursor before forking.

thanks @only21mil. the original correctly identified that a finished turn can still own background work, and that cancellation must settle the parent task and its questions. i kept your commits and merged main, then simplified cancellation to one durable cancelledRunId field.
cancellation now accepts descendant stops before finalizing, commits the marker with the root stop events and interrupt outbox, and propagates persistence errors. an empty stop plan still commits the marker. descendant enumeration and the root stop share the child lock, preventing new delegation from escaping cancellation. the run is selected under that lock so a continuation finishing after stop still reports interrupted. published results and unrelated questions remain intact.
verification: reproduced completed on main and interrupted with the fix through live codex, task_cancel/task_status, and the web client. focused tests cover native subagents, shell work, checkpoint/turn races, monitor runs, follow-ups, descendant refusal, and restart before the stop outbox runs. vp run check:changed passed through blacksmith (format, lint, knip, affected typechecks and 123 tests). final-head ci is green across all 27 reported checks after rerunning an unrelated attachment-footnote browser timeout.
unverified: live claude/native-shell adapters, desktop, mobile and remote connections. adapter-shaped cancellation paths are covered by integration tests.
known limit: parent dispatch can wait behind a busy child lock. workflow stopping already has its existing 15-second bound. i left out a second cancellation timeout because it could abandon the operation after some descendants have stopped. this path queues provider interruption without waiting for provider shutdown.
review: one final reviewer approved the prior head; the other identified the descendant-intake race. the final two-line lock adjustment addresses it. the requested review-round limit is reached, so that adjustment has local checks and live verification but no further independent review.
independently verified head 2405ed056e55c5d2ad9e18ba0e678eeb6fbd3e6f as-is on this linux boat vm. no code changes or pushes.
installed github/main (570d4b03eef879bd60be3d74444046858c94dacd) in ../main with vp i, then ran its real dev server in the background. checked out the exact pr head, ran vp i, and repeated against its isolated dev state.
used the repo's playwright and /usr/bin/google-chrome to create a project pointing at a real git repository, send messages, and run real codex gpt-6.1-sol turns through the web client and server's mcp tools. no mocks, fixtures, or demo data.
reproduced the target boundary with an intermediate delegated task whose turn completed with WAITING_FOR_BACKGROUND_CHILD while its nested codex task was actively waiting on a real sleep 600. before cancellation, task_status reported running / waiting_for_children, with latestTerminalStatus: completed.
on main, task_cancel stopped the descendant command but the settled task reported completed / result_available with the interim reply.
on this pr, the same cancellation reported interrupted / result_available. read-only inspection of the dev database confirmed completedAt, the original cancelledRunId, disposed delivery, and an interrupted descendant command. after restarting the dev server, a fresh codex turn still read the original task as interrupted.
neighboring behavior: a follow-up to the cancelled child completed with FOLLOWUP_OK; cancelling that already-completed follow-up preserved completed / result_available / FOLLOWUP_OK. the original task stayed interrupted. main's already-completed task control also preserved its result.
blacksmith: all 124 tests passed across TaskCancelNativeSubagent.integration.test.ts, DelegatedCompletionDelivery.test.ts, OrchestratorMcpService.test.ts, BackgroundWorkStop.integration.test.ts, and ThreadStop.test.ts. the existing head has 27 successful ci checks. no new tests or source edits.
live claude/native background-subagent and provider pendingBackgroundTasks paths, async-question dismissal, checkpoint/continuation races, and stop-refusal races were not exercised with live providers. the focused integration suites cover these cases. a standalone codex background-shell probe remained running after its turn, but was already terminal to delegation, so it could only serve as a terminal-result control; the successful changed-path verification above used real nested delegated work.
native ios/android, electron desktop, macos/windows, and remote/relay/tunnel connections were not exercised on this linux vm. no performance claim. the uploaded clip was inspected, played, and seeked in the t3 preview player. dev servers and task-owned browser processes are stopped after verification.
merged current main into the original branch without rebasing. resolved the dispatch argument conflict by preserving main's side-chat deletion argument and passing the cancellation marker separately. the change remains 10 files, +675/-61 against main.
blacksmith vp run check:changed passed: format, lint, knip, graphql generation, 35 affected package typechecks and 144 tests. two independent reviewers approve faea42d14600087fc2e9fad8fe395b6fb6a7cfba.
live codex verification reached running / waiting_for_children after the intermediate turn completed, then cancellation returned interrupted / result_available with INTERIM_BACKGROUND. cancelling an already-completed control preserved completed / result_available / TERMINAL_OK. the real web client rendered both results.
the old timeout finding does not apply to the current stop path: provider interruption is queued through the outbox, and native workflow stopping already has a 15-second bound. adding another timeout could abandon cancellation after stopping only some descendants. resolved that thread after inspecting the current code.
live claude/native background work, desktop/mobile, and remote/relay paths remain unverified. existing integration tests cover the adapter-shaped cases. task-owned dev servers and blacksmith worker are stopped.
