Problem

When OpenCode's workspace command lookup fails, the workspace scan still reports its commands as complete. On OpenCode 1.x, a failed or timed-out command.list is turned into []. On OpenCode 2, a scan that times out returns no commands at all, though a finished scan always lists at least the built-ins. Either way the registry stores an empty command list as the workspace's real inventory, so its / commands disappear from the composer and nothing rescans for them.

Change

A failed lookup now marks the workspace's commands as pending (slashCommandsPending) instead of complete. The registry already handles pending entries: it keeps the last known commands and rescans on the next request, the same way it does for Claude. Skills from the same scan are still published.

  • OpenCode 1.x: the command lookup's failure or 10-second timeout becomes "pending" instead of [].
  • OpenCode 2: a scan that returns no commands is treated as timed out and marked pending.

Only packages/provider-opencode/src/server/driver.ts and its test change.

Scope and approval

A small, focused fix for an obvious bug: a failed lookup is recorded as an empty inventory. Split out of #18309, which keeps workspace skills when a provider's health check fails. That PR no longer changes failed OpenCode lookups. The two PRs touch neighbouring lines in the same driver, so whichever lands second needs a small rebase.

Verification

  • Two new driver tests: an OpenCode 1.x scan whose command.list rejects, and an OpenCode 2 scan whose command list stays empty until the 10-second timeout. Both still return the workspace's skills and report the commands as pending. Both fail on main (742c229957) and pass here.
  • vp test run packages/provider-opencode/src/server/driver.test.ts apps/server/src/provider/ProviderRegistry.test.ts: 70 passed.
  • tsc --noEmit in packages/provider-opencode passes. Targeted lint and formatting pass.
  • Not checked against a live OpenCode server. No client changes, so no UI captures.

Approvability

Verdict: Approved at 960bc73

Macroscope's review found this PR approvable — This is a narrowly scoped OpenCode bug fix that preserves previously discovered commands when discovery fails or times out, using the existing pending/rescan mechanism. Both OpenCode runtime paths receive targeted test coverage, and no product defaults or static-analysis suppressions are changed.

📝 Walkthrough

OpenCode workspace snapshots now mark slash commands pending when command lookup fails or returns no commands. Tests cover the OpenCode 2 timeout path and the OpenCode 1 lookup failure path.

Changes: OpenCode command scan state

Layer / File(s) Summary
Track command lookup failures
packages/provider-opencode/src/server/driver.ts
loadWorkspaceInventory now distinguishes command lookup failures from successful results. On failure, it returns an empty command list and marks slash commands pending.
Propagate pending state to snapshots
packages/provider-opencode/src/server/driver.ts, packages/provider-opencode/src/server/driver.test.ts
The OpenCode 2 snapshot marks an empty command result as pending. The OpenCode 1 snapshot forwards the inventory’s pending state. Tests cover both cases and verify that the discovered skill remains in the snapshot.

Change: Bug fix

Merge Risk: 🔵 Low · up to 960bc

When a workspace has no commands, commands removed earlier may remain visible. Track scan timeout separately from an empty result before merging, or accept this bounded risk.

Security architecture risk: 🔵 Low · up to 960bc

The change preserves previously discovered commands during failed scans without changing command-execution privileges. Remaining uncertainty concerns whether OpenCode 2 can successfully return an empty inventory, which would leave stale suggestions pending rather than clear them.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — A failed or empty lookup can now prolong stale command inventory for affected OpenCode workspaces. The inspected retention path remains provider-instance and cwd scoped; retained entries do not expand native command authority.

Security Findings and Attack Paths

  • inferred — The examined attack path—retaining a removed command name and submitting it to obtain native execution—is not supported. Both unchanged adapters require the name to appear in a fresh provider lookup. This conclusion is limited to the inspected native-command submission paths.

Trust Boundaries and Controls

  • observed — The registry owns cached inventory and refresh scheduling, while provider adapters own live command admission. Snapshot retention does not replace the provider lookup at the execution boundary.

Resilience and Maintainability Implications

  • observed — Existing refresh controls suppress overlapping ordinary scans for the same instance and cwd, reject results from replaced instances or changed workspace snapshots, and release claimed refresh ownership on termination. Failed outer scans do not publish a replacement through this path.

Thanks for this. I'm closing it as part of PR triage. Another PR already covers the same fix.

From the review: This duplicates #18309, which already preserves OpenCode workspace commands after failed or unfinished command lookups by keeping slash commands pending.

If you think this was a mistake, message me on Twitter at @maria_rcks and I'll take another look.
