fix(server): Codex generated images show in chat, including on Android with a Windows host - #19066
Conversation
ApprovabilityVerdict: Approved at Macroscope's review found this PR approvable — Displays Codex-generated images in chat and keeps runs open for images that finish late. The change is limited to generated-output presentation and lifecycle tracking, with no obvious material operational or access-policy risk. Format generated-image embeds packages/shared/src/generatedImageMarkdown.ts, packages/shared/package.json, packages/shared/src/generatedImageMarkdown.test.ts |
The shared helper compares saved paths with existing Markdown images, formats embeds for missing paths, and closes certain unclosed trailing blocks. Tests cover duplicate and existing images, Markdown contexts, and path formats. |
Track and project generated images apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts |
The adapter tracks running image items, saved paths, and final-answer text by native turn. It emits updated assistant artifacts when images are projected. Tests cover successful and excluded image results, late completion, and Stop cleanup. |
Keep ingestion open for pending image work packages/provider-core/src/server/ProviderAdapter.ts, apps/server/src/orchestration-v2/RunExecutionService.ts, apps/server/src/orchestration-v2/ProviderEventIngestor.ts, apps/server/src/orchestration-v2/RunExecutionService.test.ts |
The provider event contract reports pending work by turn. Run execution tracks pending work for owned turns and continues ingestion while work remains. The ingestor omits these events from normalized output. A regression test covers queued events through root finalization. |

Possibly related issues
- pingdotgg/t3code#15420: The issue describes native Codex image generation being omitted from a text-only final reply; this PR appends saved-image embeds.
- pingdotgg/t3code#19059: This issue concerns generated images returned through
functions.exec, an output path not covered by this PR.
Possibly related PRs

Late-generated images may still be lost if the provider session closes before they are projected. Confirm the session-lifetime behavior before merging.
Security architecture risk:
Low · up to 5702a
-
Generated images can now update a conversation after its main response finishes. Updates remain associated with their originating turn, and local images use existing file-access permissions. No new exploitable boundary bypass was established, but the origin of provider-reported paths and actual Windows network-share behavior remain unverified.
Retained concerns
No architecture-level concerns identified.Security review details
-
Security Blast Radius
- inferred — The filesystem capability exercised by projection is environment-host media access under an authorized filesystem-read session, potentially including host-accessible network shares where supported. It is not restricted to the conversation workspace. Arbitrary Markdown filesystem images already exercised this capability before the PR; no new cross-environment authority was established.
Security Findings and Attack Paths
- observed — Path provenance remains a bounded uncertainty, not a verified attack path. Notifications arrive through the spawned provider process and are schema-decoded, but AbsolutePathBuf is implemented as Schema.String. Neither that codec nor the new adapter admission check establishes workspace ownership, file creation provenance, or local-only origin. Whether an external attacker can control savedPath was not established.
Trust Boundaries and Controls
- observed — Websocket upgrades authenticate the session, and media-file URL creation requires filesystem-read scope. Absolute media issuance canonicalizes the target, validates its preview extension, and binds expiring claims to file identity. Asset resolution verifies signed claims and identity. Projection does not replace these controls.
Resilience and Maintainability Implications
- observed — Provider-turn ownership limits late-event routing, and owner-aware gates protect provider-thread writes after root finalization. Stop marks turns as interrupting, clears retained image work, and removes contexts so later callbacks cannot continue image mutation through that context. These controls contain cross-turn state leakage and cancellation-related retention.

Taking over this one for batch codex-08. I’ll keep the generated-image behavior and Windows/UNC support, trim the regression tests, rebuild against current main, and run independent reviews. I’ll preserve your commits and push maintainer fixes here; no merge is planned. |

Re #19066 (comment): fixed in 5cf00d5. The helper no longer rewrites embeds the reply already has. It finds the images the reply shows with the chat renderer's own |
@juliusmarminge Your takeover never got pushed, so I pushed what you planned to a99427a. It keeps the generated-image behavior and the Windows and UNC support, and the tests are down from 544 added lines to 363, each failing on main. The helper no longer rewrites existing embeds; it only appends the missing images, which also fixes the Macroscope finding. The description shows the evidence and what was not tested. |

Rebased onto current main as one commit (the original 18 commits conflicted with main's reshaped adapter and run-execution tests). Behavior is unchanged: saved imageGeneration results are appended to the final answer, late completions keep ingestion open through background_work.updated, and failed or unsaved results are dropped. What's fixed
When Codex made an image with its image tool, chat showed only the text reply, even though Codex had saved the PNG. The server now adds each saved image to Codex's final reply, skips any the reply already shows, and writes Windows and UNC paths so web and mobile can load them. It also keeps the run open until images that finish after the turn are in the reply. Codex 0.166 runs the image tool inside
exec, the path in the issue, and still reports a native image item, so that path is covered too.Evidence
before.mp4
Linux web before, real generation with gpt-6-luna: Codex saves the PNG, but the reply says "Here's the image" and no image shows.
after.mp4
Linux web after, same steps: while the image generates (about 15s) the timeline shows Codex's commentary with "Thinking"; when the turn finishes the image shows under the reply in one step, with no duplicate or broken image. The red banner is from the test setup's placeholder ChatGPT login and is dismissed during the run.
Not shown: an image that finishes after the turn ends (Codex awaited the image in every run; unit tests cover it), Windows or UNC hosts, Android, iOS, desktop.
