Code Review
This pull request introduces a new diagnostic tool, approval-diagnostic.sh, along with comprehensive unit tests, to clearly identify why a pull request is not approved. It also updates the advisory review gate to log specific dropped bots and integrates the diagnostic output directly into the GitHub step summary. The review feedback provides valuable recommendations to improve the robustness and efficiency of the Bash scripts. These include avoiding command substitutions in conditional lists under set -e, separating local declarations from assignments to preserve exit codes, optimizing multiple jq calls into a single @tsv parsing step, using here-strings to prevent unnecessary subshells, and asserting specific exit codes in tests.

Maintainer decision: lower the advisory quorum to 3 — one more change needed here
The diagnostic (approval-diagnostic.sh) and the AC #2 denominator reconciliation both look right, and making required a projection of the registry is exactly the correct mechanism. Keep all of that.
But reconciling to the registry moves required from 6 to 7, which is the wrong direction for the queue. A maintainer has now decided AC #3: the quorum becomes 3. The full rationale and evidence are in #1894's body under "MAINTAINER DECISION 2026-09-23" — read it there, it is authoritative.
The short version: across eight recent PRs, gemini-code-assist and codeant-ai reviewed all eight, sonarqubecloud participates via a top-level comment (so it is invisible in the reviews API but does signal), and copilot-pull-request-reviewer, chatgpt-codex-connector, qodo-code-review and graphite-app produced zero reviews. Those four are waited on and then timed out on every single PR.
What to change
Set advisory_gate to no for those four rows in scripts/lib/reviewer-sources.tsv. Because this PR already derives the denominator from the registry, that data change alone yields required=3. Do not hardcode 3.
Three copies of the set must move together or they drift — the exact failure the registry exists to prevent:
scripts/lib/reviewer-sources.tsv_advisory_gate_load_fallback_botsinscripts/lib/advisory-review-gate.sh- the hardcoded fallback array added by this PR in
scripts/lib/approval-diagnostic.sh
Please add a test asserting all three agree.
Do not touch
creates_threads and dev_lead_trusted stay exactly as they are on every row — the #1425 invariant is unrelated to advisory_gate, and these four bots remain dev-lead-trusted and on the reviewer scorecard. They are leaving the approval-wait set only. sonarqubecloud stays advisory_gate=yes.
Record in each dropped row's rationale column why it was dropped (zero reviews observed across the eight PRs listed in #1894, 2026-09-23), so re-adding one later is an evidence-based decision.

Review — fix requested (cycle 3/3)The automated review identified the following issues. Please address each one: Findings to fixAutomated review — NEEDS HUMAN REVIEWRisk: MEDIUM SummaryNo new commits since the cycle-2 cascade review (head is still f530385), so every prior finding carries forward. The PR adds an approval diagnostic plus advisory-gate changes. The diagnostic is read-only, but it still diverges from the runtime gates it says it mirrors. Two review threads (cubic P1, graphite) are unresolved and current, and I re-confirmed both against the code at head. The PR is labelled Linked issue analysisRefs #1894 (not a closing reference, which is correct per the maintainer's direction). The description says it delivers AC #1–#3 (diagnostic, reconciled denominator, quorum lowered to 4) and leaves AC #4–#6 open. AC #2 (the reconciled denominator) is undermined by the unsupported-regex divergence below: the diagnostic can count a different set of participating bots than the gate. …chain; add the why-not-approved skill The first version of this PR re-implemented every approval gate in scripts/lib/approval-diagnostic.sh (~550 lines). The copy drifted from the real gates, and every review cycle found new divergences: - advisory-bot comments were skipped where the real gate blocks on them; - an UNSUPPORTED regex that the gate does not have; - no check-run participation (#2005); - the thread-gate fail-closed path was skipped; - an approval shortcut that ignored advisory evidence. A diagnostic that can disagree with the gate it explains is worse than none. Rebuilt so drift is impossible: - review-one-pr.sh gains a diagnose mode (PR_REVIEW_DIAGNOSE=true). It runs the real gate chain on the live PR, in order, and stops before any model runs. It is read-only (DRY_RUN) and never forced (FORCE_REVIEW and FORCE_RE_REVIEW are off), so it reports what a normal run would do. It checks the per-PR automation budget read-only and ends with proceed / gates-clear when nothing holds the PR. The answer is the run's existing emit_verdict line (decision, reason, would_change, #1552). - emit_verdict also appends that verdict to the step summary, so every pr-review run answers "why not approved?" where an operator looks. - scripts/pr-approval-diagnostic.sh <pr-url> [--json] wraps the diagnose run and adds the PR facts: review decision, approvals at the head, hold labels and merge state. - .claude/skills/why-not-approved/SKILL.md tells any Claude session to run the diagnostic and how to explain each reason. A test keeps its reason table in sync with the reasons review-one-pr.sh emits. Removed: scripts/lib/approval-diagnostic.sh, its tests and its review-one-pr hook. Kept: the advisory gate naming the bots that drop out of the reduced denominator (AC #2). Tests (tests/test_pr_approval_diagnostic.bats, 11): gates-clear, ci-failing, ci-pending without polling, FORCE_REVIEW ignored, human-escalated, already-reviewed-at-head, step-summary line, Markdown and JSON reports, URL validation, and skill/reason sync. Each review-one-pr case asserts no GitHub write and no engine run. Four tests fail on main's review-one-pr.sh. …an ignored .claude skill .gitignore deliberately excludes .claude/skills/, which is a vendor-specific projection. So the skill from the previous commit was never added. This repo's org-wide equivalent is a custom agent profile in agents/, invocable from GitHub.com, VS Code and JetBrains, so the content now lives in agents/why-not-approved.md: - frontmatter has name, description and tools, and passes the validate-agent-profiles job; - it explains how to get the script when running outside a .github-private checkout. AGENTS.md points Claude sessions at the script and the profile. The sync test now checks the profile's reason table and frontmatter. `! grep` is exempt from errexit and was not the helper's last command, so a write call in the diagnose run would not have failed the test. Fail explicitly and print the gh log. …om unreadable or stale facts #1902 review: - review-one-pr.sh diagnose mode wraps `gh` so any write (pr/issue/label writes, `gh api` with a non-GET method in any spelling or with a body, a GraphQL mutation) is refused and logged. DRY_RUN alone missed the advisory rate-limited marker post. - An unreadable budget-event list reports `automation-budget-unreadable` instead of falling through to gates-clear. - pr-approval-diagnostic.sh exits 2 when the PR facts cannot be read or the head moved since the gate run; the hold row reads the shared hold-gate list and is labelled as dev-lead's holds (pr-review gates only on needs-human-review). - Agent profile: gate order includes standing change requests, `gh repo clone` for the private repo, and the no-script fallback no longer promises a verdict line for runs that reached the model review. The AGENTS.md pointer moves to the Scripts section. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com> Claude-Session: https://claude.ai/code/session_019K6UsD7r25pAcRjJL3G6Kx

…ry-gate lines in the report #1902 review: - When GitHub still shows the PR approved but a gate would now hold it (skip / escalate / error), the report leads with that gate instead of "Approved."; a no-op or proceed verdict keeps the approval heading. - The gate log keeps the advisory gate's `[advisory-gate]` lines (e.g. which unavailable bots reduced the required set), with colour codes stripped. #1902 review: only listed read-only subcommands pass through (pr/issue view-list-status-diff-checks, GET api, non-mutation GraphQL, auth status, search, …); anything else — gist, codespace, an extension — is refused. The read-only test pattern also covers `issue close/create/reopen` and gist, and the classifier test exercises them. The agent profile tells users to read the run log when a model-reaching run posted no review. Summary
Rebuilt (7dd295e, 103ee82) at the maintainer's request. The first version re-implemented every approval gate in
scripts/lib/approval-diagnostic.sh(~550 lines). That copy drifted from the real gates, and every pr-review cycle found new divergences: advisory-bot comments were handled differently, an UNSUPPORTED regex the gate doesn't have, no check-run participation, a skipped fail-closed thread path, and an approval shortcut. This version makes drift impossible by running the gates themselves.scripts/review-one-pr.sh(PR_REVIEW_DIAGNOSE=true).FORCE_REVIEWandFORCE_RE_REVIEWare off), so it reports what a normal run would do.emit_verdictline (decision,reason,would_change, [Phase 4] pr-review no-ops report success with reasons buried in logs — clean PR #1531 sat unreviewed 100 min; root-cause + structured decline logging + convergence backstop #1552). When nothing holds the PR it ends withproceed/gates-clear.scripts/pr-approval-diagnostic.sh <pr-url> [--json]wraps a diagnose run. It adds the PR facts (review decision, approvals at the head, hold labels, merge state) and renders a Markdown or JSON report.agents/why-not-approved.md, the "skill" you asked about.
