docs: check in the advisor plans and their outcome record - #133

The plans/ directory holds the implementation plans generated by the 2026-07-05 code-health audit (advisor workflow), all executed and merged (PRs #114–#131). plans/README.md is the durable part: the execution record, the findings that were considered and rejected (so a future audit doesn't re-report them), and the direction notes for future features (--fix policy, baseline files, live UI, MCP extensions).
