When a user clicks "I know the name" then "I don't know the name" when creating a contact's relationship, the form data is not updated; Any entered names are retained, otherwise the form won't submit and will ask for the name(s) to be entered. I have added a function to clear the form fields that are used when "I know the name" option is selected. This function is called when a user clicks the "I don't know the name" radio button. This solves the above issues.
