Refactored Request Handling in Bifrost

This PR refactors the request handling logic in the Bifrost core to eliminate code duplication across different request types. Key improvements include:

  1. Created unified handleRequest and handleStreamRequest methods that consolidate the common logic for all request types, reducing hundreds of lines of duplicated code.

  2. Added a response stream object pool to improve memory management for streaming requests, similar to the existing pools for other channel types.

  3. Improved error handling with context-aware response sending to prevent deadlocks when clients disconnect.

  4. Extracted fallback handling logic into separate helper methods (shouldTryFallbacks, prepareFallbackRequest, and shouldContinueWithFallbacks).

  5. Fixed potential memory leaks by properly managing plugin pipeline resources for streaming requests.

  6. Added early nil context handling to prevent blocking in request processing.

  7. Enhanced queue transfer during provider concurrency updates with proper goroutine synchronization using WaitGroups.

These changes maintain the same functionality while significantly improving code maintainability and reliability.

# Refactored Request Handling in Bifrost

This PR refactors the request handling logic in the Bifrost core to eliminate code duplication across different request types. Key improvements include:

1. Created unified `handleRequest` and `handleStreamRequest` methods that consolidate the common logic for all request types, reducing hundreds of lines of duplicated code.

2. Added a response stream object pool to improve memory management for streaming requests, similar to the existing pools for other channel types.

3. Improved error handling with context-aware response sending to prevent deadlocks when clients disconnect.

4. Extracted fallback handling logic into separate helper methods (`shouldTryFallbacks`, `prepareFallbackRequest`, and `shouldContinueWithFallbacks`).

5. Fixed potential memory leaks by properly managing plugin pipeline resources for streaming requests.

6. Added early nil context handling to prevent blocking in request processing.

7. Enhanced queue transfer during provider concurrency updates with proper goroutine synchronization using WaitGroups.

These changes maintain the same functionality while significantly improving code maintainability and reliability.
