The
activeCandidates query now fetches every VM row for a billing team with no status pre-filter, including all historical failed and destroyed rows. The old query used inArray(cloudVms.status, ["provisioning", "running", "paused"]) to keep the result set small. This new query runs inside an advisory-locked transaction, so a long-lived team with hundreds of historical VM rows will hold that lock while transferring and filtering all those rows in application code — adding latency that directly blocks concurrent creates for the same team.

Team ID priority in vmCreateAuthOptions is inverted relative to the handler — edge-case resolution failure for multi-team users
vmCreateAuthOptions resolves the billing team ID as header/query → body (line 264), but the handler at line 189 uses body → header/query:
// auth options (line 264): header wins
requestedBillingTeamId: requestedVmTeamIdFromRequest(request) ??
await requestedVmTeamIdFromBody(request)
// handler (line 189): body wins
const requestedBillingTeamId = body.billingTeamId || requestedVmTeamIdFromRequest(request);Concrete failure: a multi-team user whose selectedTeam is teamA sends x-cmux-team-id: teamA (header) and {"billingTeamId": "teamB"} (body).
vmCreateAuthOptionspicks teamA (header wins). SinceteamA === selectedTeam, shouldListTeams = false, souser.teamIds = ["teamA"]only.- The handler picks teamB (body wins).
resolveVmEntitlementschecksuser.teamIds.includes("teamB")→false→VmBillingTeamResolutionErrordespite the user legitimately being in teamB.
Fix: align vmCreateAuthOptions to body-first, matching the handler's semantics for a POST payload. request.clone() already ensures the original body is still readable by the handler.

Use mockRejectedValue instead of mockResolvedValue(Promise.reject(...)).
mockResolvedValue(x) wraps x in Promise.resolve(...). Passing a rejected promise here works only because awaiting the outer promise chains the inner rejection — but it constructs a rejected promise whose handler isn't attached synchronously, which can trip Bun's unhandled-rejection diagnostics and is not the documented way to make a mock reject.

Clarify the free-plan opt-out when a global item is set.
Line 119 currently implies that leaving CMUX_VM_PLAN_FREE_CREATE_CREDIT_ITEM_ID unset is enough to avoid free-plan create credits, but createCreditItemId() still falls back to CMUX_VM_CREATE_CREDIT_ITEM_ID. In an environment that sets the global item for paid plans, free will keep consuming create credits unless the free-plan key is explicitly set to a disabled value. Tightening the wording here would prevent an easy misconfiguration.

Scope legacy null-team rows only to personal-team creates.
The fallback on Lines 152-154 and Lines 256-258 always pulls billing_team_id IS NULL rows for the caller, even when this create is explicitly billed to another Stack team. After requested-team routing, a user’s old personal-scope VMs can now trigger vm_active_limit_exceeded for an otherwise-empty team because both reconciliation and the final count include those rows. Thread enough context to distinguish personal-team creates from selected-team creates, and only apply the null-team fallback in the former case.

Auth and handler disagree on team ID priority
vmCreateAuthOptions resolves requestedBillingTeamId as header/URL first, body second (requestedVmTeamIdFromRequest(request) ?? requestedVmTeamIdFromBody(request)). The handler resolves it as body first, header/URL second (body.billingTeamId || requestedVmTeamIdFromRequest(request)).
When a client sends both x-cmux-team-id: team-A (their selected team) and body billingTeamId: team-B, auth uses team-A and skips team listing (since it matches selectedTeam.id), so user.teamIds = [team-A]. The handler then tries to resolve entitlements for team-B, which isn't in user.teamIds, producing a false 403 vm_billing_team_not_found. The mac app always sends the selected-team header; any call that additionally passes a body billingTeamId for a different team will hit this.

Suspended VMs excluded from active limit check
Medium Severity
FREESTYLE_ACTIVE_STATES only includes "starting", "running", and "suspending", but omits "suspended" and "resuming". When the live reconciliation path runs, VMs in Freestyle's "suspended" state (which map to "paused" in the DB) are excluded from the live active set. In isActiveForLimit, these VMs then fail all checks (not in live set, not "provisioning") and are not counted toward the limit. This means paused VMs no longer count against the active VM cap when reconciliation is active, diverging from the old behavior where "paused" VMs always counted.

Backchannel filter loses frame state across process calls
Medium Severity
TerminalBackchannelFilter.process doesn't track whether it's currently mid-frame (prefix consumed, awaiting terminator). If a backchannel frame is split across two WebSocket messages, the first call consumes the prefix and returns. On the next call, pending contains only payload+terminator data. Since prefixIndex won't find a prefix, the code falls through to the "no prefix found" path and outputs the raw base64 payload as terminal content, failing to deliver the RPC and corrupting terminal output.

Provider errors during reconciliation surface as unhandled 500s
reconcileActiveProviderVmIdsForLimit throws VmProviderOperationError when any getInfo call fails for a reason other than 404 (e.g. transient Freestyle API unavailability). That error is not caught anywhere in createVm or the route handler, so teams at or above their active VM limit during a Freestyle API disruption will get 500 instead of a deterministic 402.
The same risk applies to the retry path inside catchAll: if the forced reconciliation also throws VmProviderOperationError, it escapes the catchAll and the user still sees 500 rather than the original 402 they would have received before this PR.
Adding .pipe(Effect.orElseSucceed(() => null)) (or equivalent) to both reconcileActiveProviderVmIdsForLimit call-sites would silently fall back to DB-only counting on provider failure, preserving the previous 402 behaviour.

Blocking socket RPC stalls the websocket receive loop for up to ~3s per frame.
handleTerminalBackchannel runs inline on the receive thread and performs a synchronous SocketClient.connect() + sendV2(... responseTimeout: 3). While that call is in flight, receiveSync cannot pull the next message, so PTY output (including subsequent terminal data and any further backchannel frames) stalls. Under a misbehaving local socket peer the loop sits idle for the full 3s timeout per frame, and bursts of cmux notify calls serialize end-to-end — visible as terminal-output lag.
Dispatch the RPC onto a dedicated serial queue (one in-flight request preserves ordering) and return immediately so the receive loop keeps draining.
🔧 Suggested fix: hand off the RPC to a serial queue

Don't send reply-oriented commands over a fire-and-forget backchannel.
This path still forwards rpc and every browser subcommand even though the websocket terminal backchannel has no response channel. Commands like cmux rpc system.capabilities or cmux browser get-url will now exit successfully with no output instead of falling back to a socket relay or failing clearly.

Make /usr/local/bin first, not merely present.
If the inherited PATH already contains /usr/local/bin later in the list, this helper returns it unchanged, so another cmux earlier in PATH can still win. That breaks the new bundled-CLI assumption for websocket terminals.

Bound the requested-event write before provider finalization.
Effect.all waits for recordCreateRequestedEvents and provision. The live recordUsageEvents path awaits db.insert through dbEffect, which adds no timeout and does not bind the query to currentCloudDbQuerySignal. If that insert remains pending after provision returns a handle, execution cannot reach markCreateRunning or its rollbackProviderCreate cleanup path. The provider VM can remain running without a persisted providerVmId, and a retry returns VmCreateInProgressError. Add a bounded audit-write path and ensure interruption after provider creation destroys the provider resource before returning.

Remove the real-time ceiling from this correctness test.
Effect.timeout("1 second") is a hard absolute latency ceiling. A paused or slow CI worker can fail this test although the Deferred completion signal establishes the required behavior. Use an Effect test clock for any deadline, or wait only on the Deferred-based signal.
As per coding guidelines, a test must not use “a hard absolute latency ceiling on shared CI.” As per path instructions, the same rule applies to web/tests.

Touches VM auth/billing/limit enforcement and adds a new terminal backchannel transport, so regressions could impact who can create VMs or how commands are relayed. Changes are scoped with tests but affect core provisioning paths.
Overview
Optimizes Cloud VM API auth and billing selection by letting routes pass
requestedBillingTeamIdand limiting Stack team listing (includeTeams: "selected"), including accepting abillingTeamId/teamIdfrom the VM create body and mapping active-limit failures to402 vm_active_limit_exceeded.Tightens free-tier behavior: raises the default free active-VM limit to 5, removes the implicit free-plan create-credit default (credits become config-only), and reconciles Freestyle active-VM limit checks against live provider state (with a retry path) while running provider create and pre-audit events concurrently.
Adds a WebSocket terminal "backchannel" RPC path: the Swift CLI injects a per-session
backchannel_tokenplus workspace/surface IDs into the WS auth frame, filters terminal output forcmux-rpcescape frames, and forwards validated RPCs to the local socket; the remote daemon seeds the token/context into the PTY env, the bundled VM images now bake acmuxsymlink tocmuxd-remote, and the Go CLI can emit fire-and-forget RPC escape frames when running in WS terminals.Reviewed by Cursor Bugbot for commit beb7439. Summary by cubic
Speeds up Cloud VM creation by recording requested events alongside guarded model-plane and provider provisioning instead of waiting for the audit first. The request still waits for the audit and preserves provisioning failure cleanup.
vm_active_limit_exceededwhen exceeded.billingTeamIdorteamId; membership is validated and invalid selections return 403.cmux; WebSocket sessions pass workspace, surface, and backchannel token context so VM shells can trigger local v2 RPCs.CMUX_REMOTE_TRANSPORT=ws, including fire-and-forget browser relay and selected v2 commands.Written for commit 929069d. Summary will update on new commits.
