Summary

On 2026-10-01, #16397 merged migration 20261001000000_coderouter_vm_pool_initialization and code that inserts into its table. Vercel deployed main to production at 08:23 UTC. Nobody applied the migration to PlanetScale, and every Cloud VM create failed for 7 hours until #16405. The 2026-09-02 Blaxel enum incident had the same cause. Production migrations never run at deploy, so the "apply before merge" rule depended on memory. This PR adds three layers.

1. Deploy gate (the guarantee). web/tools/migration-deploy-gate.mjs runs first in vercel-build (the script Vercel runs; build also calls it). On the production environment of the cmux and cmux-staging Vercel projects (matched by VERCEL_PROJECT_ID), it reads drizzle.__drizzle_migrations names from the build's own DIRECT_DATABASE_URL/DATABASE_URL inside begin read only, then compares them with web/db/migrations through drizzle's getMigrationsToRun, the name-based rule apply-migrations.mjs uses. Any missing name fails the build, so Vercel keeps the previous deployment live. It applies nothing. Fail-closed cases: unreachable database or unreadable ledger, missing DATABASE_URL, and a Vercel production build with no VERCEL_PROJECT_ID. Previews, the docs projects that build web/ with --prod, CI's web-production-build, and local builds skip it. Output contains migration names and an error code (ECONNREFUSED, a SQLSTATE), never URLs, SQL, or driver messages. The file lives in web/tools/ because the root .vercelignore drops every scripts/ directory from the build.

Break-glass: set CMUX_MIGRATION_GATE_BREAK_GLASS=<deployed commit SHA> (7 to 40 hex) on the project's production environment and redeploy. The gate is skipped for that commit only, with a loud banner. A value left behind does not disable the gate for later commits.

2. PR readiness check (early signal). New Web migration readiness workflow (pull_request_target, GitHub-hosted runner). It reads the PR file list through the API as data, never checks out or runs PR code, and runs the gate from the base revision. When a PR adds a web/db/migrations/<name>/migration.sql, it checks those names read-only against staging and production through repository secrets CMUX_MIGRATION_GATE_STAGING_DATABASE_URL and CMUX_MIGRATION_GATE_PRODUCTION_DATABASE_URL. Until those secrets exist, it requires the db-migrations-applied label as an attestation. labeled/unlabeled re-run it. It is a standalone workflow and not part of the ci-web.yml/web_subareas.py routing, because it must run from trusted base code with secrets and must re-run on label changes. Because pull_request_target reads the workflow from the base branch, it starts running after this PR merges. It is not a required check.

3. Ordering. web/AGENTS.md gets a "Migration order" section: open the PR, apply to staging, apply to production, merge; migrations must be expand-only so the code already on main keeps working once they are applied.

apply-migrations.mjs selects pending migrations by name (getMigrationsToRun filters local names absent from the ledger), so a migration whose timestamp is older than the newest applied one still applies. Verified below.

Operator steps (not done in this PR)

The PR check needs two secrets. Create a role that can only read the migration ledger, on each PlanetScale branch (staging, then main). Run the SQL as an admin role, for example through pscale shell cmux-prod <branch> --org cmux:

CREATE ROLE cmux_migration_ledger_reader NOLOGIN;
GRANT USAGE ON SCHEMA drizzle TO cmux_migration_ledger_reader;
GRANT SELECT ON drizzle.__drizzle_migrations TO cmux_migration_ledger_reader;

Then create the login role and store its URL:

pscale role create cmux-prod staging migration-readiness-ci --org cmux --inherited-roles cmux_migration_ledger_reader -f json
pscale role create cmux-prod main migration-readiness-ci --org cmux --inherited-roles cmux_migration_ledger_reader -f json
gh secret set CMUX_MIGRATION_GATE_STAGING_DATABASE_URL --repo manaflow-ai/cmux # paste the staging database_url
gh secret set CMUX_MIGRATION_GATE_PRODUCTION_DATABASE_URL --repo manaflow-ai/cmux # paste the main database_url

If PlanetScale refuses the custom inherited role, do not fall back to pg_read_all_data in a repository secret: any workflow on any branch can read repository secrets, and that role reads customer data. Use an environment restricted to main instead, and change the workflow to use it.

The deploy gate needs no new secret: it uses each project's existing production DATABASE_URL, whose role can already read drizzle.__drizzle_migrations on both branches (checked read-only).

Testing

  • Red/green: commit 1 adds web/tests/migration-deploy-gate.test.ts and web/tests/migration-deploy-gate-db-behavior.test.ts; bun test tests/migration-deploy-gate.test.ts failed (module missing). After commit 2: 23 pass. Cases: all applied passes; one missing fails with its name; older-timestamp migration missing behind a newer applied one fails; unreachable DB (injected and a real 127.0.0.1:1 connect) fails closed without printing credentials; break-glass for this commit passes with a banner and no DB read; stale or too-short break-glass is ignored; preview, non-Vercel, and docs-project builds skip; missing project id or DATABASE_URL fails; PR-mode name checks and missing secret.
  • Real Postgres (throwaway postgres:16-alpine, migrated with bun scripts/db-migrate.mjs): gate passed (98 of 98); after deleting the 20260930150000_hexclave_mirror ledger row behind newer rows, the gate failed naming it; preview skipped; break-glass passed; unreachable URL failed with ECONNREFUSED. CMUX_DB_TEST=1 bun test tests/migration-deploy-gate-db-behavior.test.ts passed (it runs in the web-db-migrations CI lane).
  • Out-of-order apply: applied all migrations except 20260930120000_team_email_invitations, then applyPendingMigrations on the full folder applied exactly that one.
  • Live read-only view (short-lived pg_read_all_data roles, deleted afterward): production main and staging each have all 98 migrations on this branch, so the gate passes today. Both ledgers also hold 3 names not on main (including the reverted 20261001000000_coderouter_vm_pool_initialization); the gate ignores database-only names.
  • From web/: bun run typecheck clean, bun run lint:complexity no new findings, eslint clean on new files, tests/vercel-build-inputs.test.ts and tests/vercel-ignore-build.test.ts pass.
  • Workflow: actionlint clean; tests/test_ci_actionlint_covers_every_workflow.py, test_ci_fork_runner_routing.py, test_ci_workflow_guards_are_wired.py, test_ci_reusable_workflow_permissions.py, test_ci_merge_queue_required_checks.py, test_ci_required_checks_are_bounded.py, test_ci_workflow_run_sources.py, test_ci_pull_request_caches_are_read_only.py, test_ci_guard_workflow_structure.py, test_runner_label_policy.py pass.
  • Not verified: a real Vercel production build running the gate. The first production deploy after merge is that test; it should print production database has all N required migration(s).

Summary by cubic

Prevents a repeat of the 2026-10-01 incident where a merged migration was never applied to production and every Cloud VM create failed for 7 hours. Production Vercel builds of cmux and cmux-staging now fail when their database is missing any migration from web/db/migrations, so Vercel keeps the previous deployment live.

Deploy gate

  • web/tools/migration-deploy-gate.mjs runs first in vercel-build and compares local migrations against the drizzle.__drizzle_migrations ledger read-only, using drizzle's name-based pending rule so an older-timestamp migration behind newer applied ones is still caught.
  • Unreachable databases, missing DATABASE_URL, and unidentifiable production builds fail closed; previews, docs projects, CI, and local builds skip.
  • CMUX_MIGRATION_GATE_BREAK_GLASS=<deployed commit SHA> skips the gate for that commit only.

PR readiness check and docs

  • New Web migration readiness workflow reads the PR file list via the API and verifies added migrations against staging and production read-only; until the two ledger-reader secrets exist, it requires the db-migrations-applied label instead.
  • The workflow needs CMUX_MIGRATION_GATE_STAGING_DATABASE_URL and CMUX_MIGRATION_GATE_PRODUCTION_DATABASE_URL repository secrets holding read-only PlanetScale roles; create those before relying on the check.
  • web/AGENTS.md documents the order (open PR, apply staging, apply production, merge) and requires expand-only migrations.

Written for commit c7fc0c3. Summary will update on new commits.

Summary by CodeRabbit

  • New Features
    • Production builds now verify that required database migrations are applied before deployment, helping prevent deployments when the database schema is not ready. The check applies to the production and staging projects and reports missing migrations or database access issues.
    • Pull requests that add migrations can be checked against staging and production before merging. If database checks are unavailable, an approval label is required to proceed.
  • Documentation
    • Added guidance on applying migrations before merging and keeping schema changes compatible with deployed versions.

Adds failing tests for a Vercel production build gate that refuses to
deploy when the target database lacks a migration from web/db/migrations,
including the 2026-10-01 out-of-order case (older-timestamp migration
missing behind a newer applied one), DB unreachable, break-glass, and
preview skip.

On 2026-10-01 #16397 merged a migration and code that needed it. Vercel
deployed main to production, nobody applied the migration, and every Cloud
VM create failed for 7 hours.

- tools/migration-deploy-gate.mjs runs first in vercel-build. On the
production environment of cmux and cmux-staging it reads
drizzle.__drizzle_migrations in a read-only transaction and fails the
build, keeping the previous deployment live, when any local migration
name is missing (drizzle getMigrationsToRun rule, so an older-timestamp
migration behind newer applied ones is caught). Unreachable DB fails
closed. Previews, docs projects, CI and local builds skip.
CMUX_MIGRATION_GATE_BREAK_GLASS=<deployed commit SHA> skips it for that
  commit only.
- Web migration readiness (pull_request_target, trusted base code, PR file
list as data) verifies migrations a PR adds against staging and
production through read-only secrets, or requires the
db-migrations-applied label until those secrets exist.
- web/AGENTS.md documents the order: open PR, apply staging, apply
  production, merge, with expand-only migrations.

Merge Risk: 🟡 Moderate · up to c7fc0

The new migration readiness workflow fails the repository's required CI guard because it uses a bare hosted runner instead of the repository runner variable. Fix this before merging; the migration gate logic itself shows no concrete defects in the supplied context.

Security Architecture Review

Security architecture risk: 🔵 Low · up to c7fc0

The change strengthens deployment safety and keeps pull-request code away from database credentials. Remaining uncertainty concerns effective credential restrictions and production rollout behavior. Emergency bypasses are commit-scoped but require operator cleanup.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The new privileged integration reaches the staging and production migration ledgers from a repository workflow. Its intended authority is ledger-only SELECT access, but effective role grants, branch binding, and secret-access scope were not established. Broader customer-data access is therefore neither demonstrated nor ruled out by configuration evidence.

Security Findings and Attack Paths

  • observed — The inspected PR-input path does not select executable PR code or construct database SQL. Migration names are validated and quoted, checkout uses the base SHA without persisted credentials, and database access uses a fixed query. These controls counter the candidate PR-code execution and SQL-injection paths; effective credential permissions remain a separate proof gap.

Trust Boundaries and Controls

  • observed — The workflow declares read-only repository permissions, pins checkout and Bun setup actions, installs dependencies from the trusted checkout, and scopes database URLs to the verification step. Label fallback changes readiness status but does not execute PR code. With both secrets configured, a label cannot override a failed database check.

Resilience and Maintainability Implications

  • observed — Connected database sessions use BEGIN READ ONLY, rollback, and client closure. Transient read failures receive bounded retries; final read failure rejects admission and emits a sanitized code rather than a driver message or URL. The gate does not mutate shared database state. Connection establishment occurs before the cleanup block, so interruption and failed-connect cleanup are not fully established by this source.

Hardening Proposals

  • proposed — Before enabling credential-backed verification, confirm ledger-only grants and the intended branch and secret scope. Consider retaining project-identification checks during break-glass and recording removal of the bypass after deployment, including same-commit redeploys. These are assurance and hardening proposals, not verified vulnerabilities.

Explanation

Docstring coverage is 30.00% which is insufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff. Analyzed 20 functions across 3 files. (3 skipped: 3 unsupported.)

Full details: Cmux No Hacky Sleeps

Explanation

The new production build/runtime script adds a fixed network retry delay at web/tools/migration-deploy-gate.mjs:121: setTimeout(resolve, retryDelayMs) with a default 2-second backoff. The retry loop has bounded attempts and connection/query timeouts, but it has no cancellation signal or cancellation-aware scheduler. The added tests only cover a single-attempt connection failure and do not test retry or cancellation. This matches the rule's failure condition for fixed backoff in runtime network readiness logic. The workflow timeout is out of scope, and the test-only files do not cause the failure.

Resolution

Replace the raw setTimeout retry delay with a dedicated cancellation-aware retry/timeout abstraction that accepts an abort signal or explicit deadline, stops promptly when the build is cancelled, and preserves bounded total retry time. Restrict retries to verified transient connection failures. Add tests for transient retry, final failure, bounded deadline, and cancellation.

CI fast guards failed on c7fc0c3c97 (https://github.com/manaflow-ai/cmux/actions/runs/36949387305). It does not block the merge; a red guard merged into main breaks it for every open PR.

Run canonical CMUX CI guard profile

these jobs use a bare GitHub-hosted runner; route them through vars.LINUX_RUNNER / vars.MACOS_RUNNER_IOS so Blacksmith<->overflow stays a repo-variable flip:

Fix: the assertion above names what the guard expects; change the tree to match it.
Reproduce in seconds, no build: scripts/ci/guards-local.sh --step 'Run canonical CMUX CI guard profile'

Agents: python3 scripts/ci/guard_attribution.py fix applies the mechanical fixes locally. This comment is updated in place on each push.

📐 Maintainability & Code Quality | 🟠 Major | ⚡ Quick win

Route the job through the repository runner variable to pass CI fast guards.

The canonical CMUX CI guard profile fails on Line 31. The job uses the bare GitHub-hosted runner ubuntu-24.04, and the guard requires vars.LINUX_RUNNER. The comment on Lines 29-30 conflicts with this repository rule. Keep the ephemeral-runner requirement in one of two ways. Option 1: confirm that vars.LINUX_RUNNER resolves to an ephemeral hosted runner. Option 2: add a sanctioned guard exemption for this secrets-bearing pull_request_target job. Either way, the workflow must not merge while the guard fails.

P2: spawnSync has no timeout, so a slow or unreachable DB lets the gate burn through its full retry path (3 × 15 s connect timeout + 2 s delays, ~49 s) while the test blocks. The second runGate() call runs after the ledger row is deleted but before the finally restore, and the harness default per-test timeout is 30 s (CMUX_DB_TEST_TIMEOUT_MS); a kill of the run in that window strands the deleted row in the persistent test DB (the db:test volume is preserved), so the next bun db:test re-applies that migration and fails because its objects already exist. Pass an explicit timeout to spawnSync so the child is terminated, the code-1 assertion fails, and the finally restore always runs.

P3: The real-folder test hardcodes 20260425062520_keen_kronos and uses the live web/db/migrations folder as its fixture with applied: []. When that specific migration folder is later removed or renamed by the routine migration cleanup this repo performs (AGENTS.md: "Remove old schema later"), this test fails CI for an unrelated PR even though the gate is unaffected. Derive the expectation from the folder instead of pinning a mutable migration name.
