Initiative descriptions are now markdown documents instead of a text column on initiative. Adds the initiative_description value to the shared document_sub_type enum (DB/sqlx, storage/search/notification OpenAPI, cognition schemas). Initiative SQL shifts from description to description_document_id on insert/select/list/delete/update, including returning the document id on delete/update for cleanup and a query to resolve document share permissions.

On the web app, toSubType centralizes mapping wire subtypes to UI types and drops initiative_description so those docs do not appear as task/snippet/skill blocks in soup, previews, and GraphQL mapping (with tests).

chore(api): regenerate clients after initiative_description

test(initiative): assert RESTRICT as Postgres restrict_violation
