[gg-rebased] fix: pin same-repository MTP to the target revision

Summary

Forward-port #118 onto dev/gilded-gnosis-rebase at 533b037f35.

When target and MTP draft come from the same repository, the draft now inherits the target weight/code revision and the serialized NextN probe resolves against that effective revision instead of implicit main.

The conflict resolution retains both new upstream behaviors that were absent from the old GG context:

• custom proposer dotted-path detection;
• sequence-parallel MTP full-token-layout restoration.

Explicit draft revisions, separate draft repositories, and local paths remain unchanged.

Port validation

• ruff check: pass
• ruff format --check: pass
• git diff --check: pass
• cold-cache MTP E2E is tracked with the v19 image

Supersedes #118 for the upstream-rebased GG line.
