fix(logs): keep recognized bare upstream error classes in synthesized terminals - #6889
Conversation
… terminals A bare upstream error that ended the WebSocket exchange was turned into a synthesized response.failed carrying upstream_server_error, and captureTerminalHttpStatus ignored bare errors, so structured rate-limit and overload codes were recorded as a generic 502. Preserve recognized rate-limit and overload classes in both relay modes, record them as 429/503 provisionally, let a genuine terminal supersede them, and record a bounded upstreamErrorType. Terminal-status diagnostics move to request-log-terminal-status.ts to keep request-log.ts under the size gate. No replay is added. Refs #6740.

Preserve recognized codes when the error message is absent
When an upstream emits a valid bare error containing only a recognized type/code, such as {"type":"error","error":{"type":"rate_limit_error","code":"rate_limit_exceeded"}}, these new arguments never carry that classification: boundedBareUpstreamError returns undefined whenever both the message and refusal code are absent, so upstreamError() remains undefined and EOF takes the adapterEofIncompleteFrame branch instead. Both relay modes consequently expose a generic incomplete terminal rather than the intended 429/503 failed terminal; treat a recognized type/code as sufficient evidence and synthesize a bounded fallback message.

Persist the new upstream error type with failure rows
For failed requests this adds upstreamErrorType only to the live in-memory row. addRequestLog rebuilds the durable usage row from a whitelist whose failure diagnostics still omit this field, PersistedUsageEntry has no corresponding property, and requestLogEntryFromPersistedUsage therefore cannot restore it. After the ring is evicted or the proxy restarts, /api/logs silently loses the newly advertised diagnostic, undermining its incident-diagnostics purpose; thread the bounded field through persistence, normalization, and hydration.

Unrecognized bare errors recorded a provisional 502, which overrode explicit status fields that combo stream preflight reads (status/http_status on the event), and a completed terminal recorded 200. Record a provisional status only for recognized rate-limit and overload classes, and let response.completed only retire provisional evidence.

Preserve explicit http_status before provisional classification.
When a bare error contains code: "rate_limit_exceeded" and http_status: 503, captureTerminalHttpStatus records provisional 429 before combo preflight reads the explicit field. The projection then uses 429, so the synthesized failure and final failed request-log row can report the wrong status.
Only record the recognized provisional status when the bare event has no valid explicit status.

Summary
Request logs and synthesized terminals now keep the class of a bare upstream
errorevent when it is a recognized rate limit or overload, instead of flattening it to a generic 502.On the Codex upstream WebSocket path, a bare
errorframe (notresponse.failed) ends the exchange, and the SSE relay synthesizesresponse.failedat EOF. That synthesized terminal replaced every non-refusal structured type/code withupstream_error/upstream_server_error, andcaptureTerminalHttpStatusignored bareerrorevents, so a structuredrate_limit_exceededorserver_is_overloadedwas recorded as an HTTP 502upstream_server_error. In #6740 that made a burst of upstream rejections on a pooled ChatGPT route look like opaque server faults and hid which class of upstream refusal was happening. Now:
response.failedand preserve only recognized classes: rate limits and explicit overloads. Unknown codes still fall back toupstream_server_error, and replay-refusal precedence is unchanged.response.failed(src/lib/errors.ts). A later genuine terminal supersedes it, so a bare rate limit followed by a real overload ends as 503, and its pool health input is transient overload, not quota evidence.upstreamErrorTypeis recorded next to the existingupstreamErrorCodeandupstreamRequestIdwith the same token validation.Terminal-status classification and the diagnostic fields move into
src/server/request-log-terminal-status.ts, which leavesrequest-log.ts105 lines shorter and under the 2000-line size gate. The management API reference and the owning structure docs describe the new field and status mapping. This PR does not explain or cure the 502 surge reported in #6740. The reporter's latest data points to upstream admission and overload, and the next step is better evidence from this logging, so the issue stays open.
