Summary

When a native Claude passthrough stream stalls (bodyStallSec) or exceeds the byte cap (bodyMaxBytes), tapAnthropicSseForLog (src/server/claude-messages.ts) ends the body with an Anthropic error frame. The row, however, is finalized as status 200 with only closeReason: "body_stall" / "body_overflow".

addRequestLog persists failure diagnostics to usage.jsonl only for rows at status ≥ 400 or with a non-completed terminalStatus (src/server/request-log.ts, failureDiagnostics). So these cut-short turns reach usage.jsonl as plain 200 rows with no closeReason and no reason, and everything that reads usage.jsonl counts them as successes. The in-memory /api/logs row still has the closeReason.

This settles the status split raised in the #6235 review ("stall 200 vs reset 502"). The Responses relay already decided it: a stall-timeout incomplete is logged as 502 with terminalStatus: "incomplete". httpStatusForRequestLogTerminal maps only a max_output_tokens incomplete to 200, and its decision log reads "Stall, EOF, and unknown incompletes must remain visible failures" (7fcaa9119).

Change

  • Stall and byte cap (src/server/claude-messages.ts, failBody): finalize with status 502, terminalStatus: "incomplete", the existing closeReason, and the proxy's own message in upstreamError. The message is proxy-owned text with no upstream content (for example anthropic passthrough body stalled: no upstream bytes for 90s). A mid-stream reset stays 502 / failed, so the two now differ only by terminalStatus. The client stream is unchanged.
  • After the turn's own terminal (message_stop or an upstream error event): a stall or overflow is a finished turn. It logs 200 / terminal and closes without an error frame, matching the read-error branch from fix(claude): end a passthrough stream on an upstream reset with an Anthropic error event #6235. Before this PR such a turn also got an error frame appended after message_stop.
  • CRLF / CR streams (CodeRabbit finding, 97254b530): the tap's inspection split frames on \n\n only, so a CRLF- or CR-delimited stream never had its frames seen. Usage was not read, and a finished turn that then stalled would have logged as a failure. The inspection copy is now normalized to LF, with the forwarded bytes untouched. A trailing CR is held until the next chunk, so a CRLF split across chunks stays one line ending. failBody shares the read-error branch's tail flush, and the blank line is restored only when the client has not received one.
  • Managed native lane (src/server/messages-native.ts): no source change. Its streaming finalize already forwards the tap's meta (fix(claude): end a passthrough stream on an upstream reset with an Anthropic error event #6235), so its rows change the same way, and its non-streaming fold keeps forwarding the tap's meta.
  • structure/clients/claude-desktop.md: the "not uniform" sentence added in fix(claude): end a passthrough stream on an upstream reset with an Anthropic error event #6235 now describes a uniform 502. The Responses parity is claimed for the stall only. The byte cap is described as matching the non-stream passthrough's 502 body_overflow, since the Responses side has no byte-cap incomplete.

Effects, checked by reading each consumer:

Consumer Effect
classifyRequestOutcome / Prometheus Unchanged: the outcome is incomplete before and after.
Failure fingerprint / projection Unchanged: incomplete rows are skipped.
Resend permission Unchanged: it needs a failure cause, and incomplete has none.
Account / key / OAuth cooldowns and key failover Unchanged: they act at send time on a non-OK response, never on a finished stream row.
errorCode Becomes upstream_server_error, the code the Responses stall row gets.
Routing health, routing analytics, JEV stats Now count these turns as failed instead of successful. This matches how the Responses stall is treated and is the point of the change.

Verification

Head 97254b530 on dev 64294638a (0 behind), Bun 1.4.0 (package.json).

  • RED first. With src/server/claude-messages.ts reverted to dev, all of the following fail:

    • a stalled native stream logs a 502 incomplete row and keeps its diagnostics in usage.jsonl
    • a native stream over the byte cap logs a 502 incomplete row and keeps its diagnostics in usage.jsonl
    • a relayed stream over the byte cap is a 502 incomplete row, like the passthrough (managed lane)
    • the updated A1/A2 (claude-messages-endpoint.test.ts, which now compare the full finalize meta)

Against the first commit 999713197, two more fail, showing the error frame after message_stop:

    • a turn that sent message_stop and then stalls is finished: 200 terminal, no error frame
    • bytes past the cap after message_stop do not turn a finished turn into a failure

Against abf0b15e5, four CRLF/CR cases fail: CRLF- and CR-delimited turns that then stall (including usage), a CRLF split across chunks, and a CRLF turn followed by bytes past the cap.

Also added: a managed-lane streaming stall case that checks terminalStatus and the tap's reason, and a terminalStatus assertion on the existing fold test.

  • Targeted. bun test tests/lab/core-lab-boundary.test.ts tests/claude-integration/claude-native-passthrough.test.ts tests/claude-integration/claude-messages-endpoint.test.ts tests/claude-integration/messages-native.test.ts tests/claude-integration/messages-native-oauth.test.ts tests/claude-integration/messages-native-opaque-state.test.ts → 193 pass, 0 fail.

  • Directories. bun test tests/claude-integration tests/usage → 2124 pass, 1 fail. The failure is POST /v1/messages?beta=true streams an Anthropic-shaped turn end to end, a translated route this does not touch. It fails the same way in a directory run of untouched dev and passes alone (see fix(claude): end a passthrough stream on an upstream reset with an Anthropic error event #6235).

  • File size. claude-messages-endpoint.test.ts stays at 1990 lines: the A1/A2 changes are in place, and the new cases live in the sibling passthrough and managed-lane files.

  • Gates. bun x tsc --noEmit → exit 0. bun run structure:check → passed. bun run privacy:scan → passed.

  • Full suite (bun run test on a Mac mini with the Codex app not running, at 999713197): exit 1 after 643 s with 17 failures, none in this code.

    • 3 of them fail the same way in a full run of untouched dev 64294638a on the same machine: deferToNewerServiceRuntime (2) and turning Codex off.
    • The other 14 are in claude-management-api.test.ts (Claude Desktop apply/PUT) and claude-models-discovery.test.ts (Codex discovery / selectors). They include 30 s timeouts, failed the same way in a loaded dev run during fix(claude): end a passthrough stream on an upstream reset with an Anthropic error event #6235, and pass when rerun at 97254b530: bun test tests/claude-integration/claude-management-api.test.ts tests/claude-integration/claude-models-discovery.test.ts tests/claude-integration/claude-native-passthrough.test.ts tests/claude-integration/messages-native.test.ts tests/claude-integration/claude-messages-endpoint.test.ts → 203 pass, 0 fail.

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

  • Required local validation passed; commands, results, and any full-suite exception are documented.

  • I pushed my PR to a recent dev commit (at most 10 behind; a maintainer may still ask for the exact tip before merge).

  • I resolved all correct Codex and CodeRabbit findings.

  • My PR is ready for review.

Summary by CodeRabbit

  • Bug Fixes
    • Responses that stall or exceed size limits before completing now report an error and are marked incomplete, rather than appearing successful.
    • Streams that finish a turn before stalling or exceeding limits remain marked as completed and close without an error message.
    • Terminal events are recognized across different line endings, helping preserve completed responses and their usage details.

…complete row

tapAnthropicSseForLog ended a body stall or byte-cap overflow with an
Anthropic error frame but finalized the row as status 200 with only
closeReason body_stall / body_overflow. usage.jsonl persists failure
diagnostics only for rows at status >= 400 or with a non-completed
terminalStatus, so these cut-short turns reached it as plain 200 rows with
no closeReason and no reason: anything reading usage.jsonl counted them as
successes.

The Responses relay logs the same events (a stall-timeout incomplete) as
502 with terminalStatus "incomplete"; httpStatusForRequestLogTerminal maps
only a max_output_tokens incomplete to 200. The tap now does the same: 502,
terminalStatus "incomplete", the existing closeReason, and the proxy's own
message in upstreamError. A mid-stream reset stays 502 / failed, so the two
differ only in terminalStatus. The client stream is unchanged.

This settles the status split raised in the lidge-jun#6235 review.
…ed turn

Review follow-up. failBody did not check whether the turn's own terminal
(message_stop or an upstream error event) had already gone through, unlike
the read-error branch. An upstream that sent message_stop and then idled
past bodyStallSec, or kept sending past the byte cap, got an error frame
appended after message_stop and, with the 502 row, would have counted as a
failed turn the client had fully received. It now logs 200 / terminal and
closes without a frame.

Tests: A1/A2 compare the full finalize meta; the managed lane gains a
streaming stall case that checks terminalStatus and the tap's reason, and
its fold test pins terminalStatus. The structure doc limits the Responses
parity claim to the stall and describes the byte cap as matching the
non-stream passthrough's 502.

📝 Walkthrough

Walkthrough

Stalled or oversized streams that have not emitted an Anthropic terminal event now finalize with status 502 and terminal status incomplete. If a terminal event has already been seen, the stream closes as a completed turn without an error frame. Tests and documentation reflect these outcomes.

Priority: ➖ Normal

Estimated code review effort: 3 (Moderate) | ~20 minutes

Change: Bug fix

Suggested reviewers: lidge-jun

Merge Risk: ⚪ Minimal · up to 97254

Incomplete Claude streams are logged as 502 while completed turns remain successful without an appended error frame. No actionable merge-blocking risk is established from the supplied evidence; normal checks should still pass before merging.

Security Architecture Review

Security architecture risk: 🔵 Low · up to 97254

The change improves reporting of interrupted streams while preserving existing limits and cancellation controls. No expanded access or privilege was identified. Incomplete validation of downstream behavior leaves low residual risk.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The inspected change affects existing native SSE requests and consumers of their request-log and usage records. The base-to-head stream-tap change does not add endpoint reachability, credential forwarding, or cross-request authority.

Trust Boundaries and Controls

  • observed — Upstream SSE data controls usage and terminal-event detection, but not the newly stored stall/overflow diagnostic text. The cumulative byte check runs before inspection or forwarding of an over-cap chunk; terminal detection does not disable upstream cancellation.

Resilience and Maintainability Implications

  • observed — Terminal paths use a settled guard, with another check after awaited reads, to prevent competing completion paths from finalizing the same stream. Stall, overflow, abort, and cancellation paths stop relevant timers/listeners and cancel the upstream reader. The shared request finalizer independently permits only the first logging call.

Comment thread src/server/claude-messages.ts
CodeRabbit follow-up. The tap's inspection split frames on "\n\n" only, so
a CRLF- or CR-delimited stream never had its frames seen: usage was not
read, and with this PR's terminal check a finished turn that then stalled
or overflowed would be logged as a 502 incomplete with an error frame after
message_stop. The inspection copy is now normalized to LF (forwarded bytes
untouched), holding a trailing CR until the next chunk so a CRLF split
across chunks stays one line ending.

The tail flush the read-error branch already did is now shared with
failBody, so a terminal block still in the buffer (an upstream that went
quiet right after it, or a held CR) counts there too; the blank line is
restored only when the client has not received one.

Reviewed 97254b5 against dev 6429463, including the terminal/tail/cancellation paths and both native consumers. My isolated Bun 1.4.0 run of tests/claude-integration/claude-native-passthrough.test.ts and tests/claude-integration/messages-native.test.ts passed 80 tests / 721 assertions under CPUQuota=75%, MemoryMax=1536M and fresh application homes. The new stall/overflow rows, post-terminal completion, CR/CRLF and split-delimiter cases pass; no new P0-P2 found in this scoped review. Exact-head hosted CI is now authorized. The author has now marked this same head ready and attested the four readiness boxes. Approval remains pending the in-progress current-head required CI; no merge approval or full-suite success is claimed. The old EOF path remains outside this change, and I did not rerun the broader baseline failures described by the author. No security scan or live account/service change was performed. This overlaps the native helper/document area of #6356; after either lands, retain both terminal diagnostics and the rate-limit header regression when validating the other.

Approved exact head 97254b5. The previous scoped review and resource-bounded 80-test / 721-assertion run cover both native lanes, stall/overflow persistence, terminal-first completion, CR/CRLF and split delimiters. The valid delimiter finding is resolved; current-head Cross-platform CI 36818324318 and required checks are successful, and readiness is complete. Trusted dev has since advanced two commits to a6114b6; the role/state-snapshot changes do not change these native stream sections. No unchanged test rerun, security scan, live account operation or merge was performed. Preserve the header-family regression in #6356 when integrating either overlapping change.
