Motivation

Inline base64 images are decoded by Bun''''s native codec during request normalization. The decoded-pixel ceiling was 100 MP (~400 MB of RGBA per surface), pass-through validation forced a full decode of every candidate, and decode concurrency was bounded only per request — so simultaneous requests could multiply native decode allocations without limit, and queued decode work kept running after the client disconnected.

Description

  • Lower MAX_INPUT_PIXELS from 100M to 16M so one decoded RGBA surface stays below 64 MiB.
  • Confirm dimensions from codec metadata before any pixel decode in bunImageEncode and bunImageValidate; oversized or undimensioned inputs are rejected instead of decoded.
  • Admit native decode work through a process-wide gate (enterImageDecode) shared across requests, with abort-aware waiters.
  • Propagate the request abortSignal through normalizeImageTargets, normalizeOpenAIChatImages, the openai-chat adapter, and the Responses dispatch buildRequest calls so cancellation stops queued and subsequent normalization work.
  • Update the structure docs for the new bound.

Tests

  • bun test tests/adapters/anthropic/anthropic-image-normalize.test.ts tests/adapters/openai/openai-chat-image-normalization.test.ts — 43 pass, 0 fail. New cases: the decode gate is shared across simultaneous requests, a header-only 100 MP PNG is rejected before reaching the decoder, and an already-cancelled build rejects without starting normalization.
  • bun x tsc --noEmit — clean.

Checklist

  • Scope stays focused and avoids unrelated cleanup.
  • Docs or release notes were updated when needed.
  • Security-sensitive changes were reviewed for secrets, auth, and unsafe defaults.
  • Tests were added or updated to cover the change.

Summary by CodeRabbit

  • Bug Fixes

    • Image processing now rejects images exceeding a safe decoded-pixel limit before full decoding.
    • Image normalization now respects request cancellation and stops queued work promptly.
    • Native image decoding is capped to prevent excessive concurrent processing.
    • Cancelled or oversized image requests now fail without unnecessary encoding work.
  • Documentation

    • Updated technical documentation to describe pixel limits, decode concurrency controls, cancellation behavior, and byte accounting.
  • Tests

    • Added coverage for shared decode limits, oversized compressed images, and pre-cancelled requests.

Review readiness checklist

This PR stays in draft until every box below is ticked. Tick all four boxes once the requirements are met:

📝 Walkthrough

Walkthrough

The change adds a 16-million-pixel decode ceiling, bounds native image decoding to four concurrent operations, and propagates request cancellation through translated Chat image normalization. Codec validation, adapter wiring, tests, and documentation now cover these behaviors.

Changes

Image decode safety and cancellation

Layer / File(s) Summary
Codec dimension guards
src/adapters/anthropic-image-codec.ts
At lines 44–46, MAX_INPUT_PIXELS changes to 16_000_000. At lines 280–282 and 298–305, encoding and validation inspect metadata first and throw "image dimensions exceed the safe decode limit" for invalid or oversized dimensions. Validation performs the full decode only after the check.
Bounded and cancellable normalization
src/adapters/anthropic-image-normalize.ts, tests/adapters/anthropic/anthropic-image-normalize.test.ts
At lines 35–65, the normalizer adds a process-wide decode semaphore. At lines 114–154, it adds abortSignal handling and abort-aware processing. Lines 216 and 258 route both processing paths through the helper. Lines 401–421 test the shared concurrency limit across simultaneous requests.
Adapter propagation and validation
src/adapters/openai-chat-images.ts, src/adapters/openai-chat.ts, src/server/responses/adapter-dispatch.ts, tests/adapters/openai/openai-chat-image-normalization.test.ts, structure/**/*.md
The adapter option and both request-build paths now carry cancellation signals. buildRequest rethrows cancellation errors. Tests cover oversized compressed inputs and pre-aborted requests. Documentation records the pixel ceiling, process-wide decode cap, cancellation behavior, and retained-byte accounting.

Check cancellation after decode-gate admission.

When admit() resolves a queued waiter, it removes the abort listener before the await continuation resumes. A queued microtask can abort the signal during this gap. The existing check runs only after processAt, so cancelled requests can still perform the decode. Add the check inside try; finally calls leave() and releases the transferred permit.

An incomplete client-tool stream is fail-closed for the current turn: `finalizeTurnEvents` emits the prefix owned by `CURSOR_INCOMPLETE_TOOL_CALL_MESSAGE_PREFIX` and does not retry that send. After the error is streamed, eligible non-isolated turns remint the Cursor conversation id, persist the thread override, and invalidate the inherited checkpoint so the next turn does not resume a conversation left waiting for `mcpResult`. `src/adapters/cursor/thread-continuity.ts` permits three such rotations per retained identity-scoped thread owner in a separate bounded counter; exhaustion keeps reusing the conversation and records an `incomplete-tool-remint-exhausted` diagnostic, while a clean completed turn clears that scope's counter. This allowance never consumes or replenishes the overflow resend budget. Isolated helper and compaction turns neither remint nor change the parent's allowance or checkpoint. Native Composer replay synthesizes `[missing tool_result for this tool_use in history]` for unpaired `toolCallStep` history; external wire models skip native `mcpToolCall` replay, so conversation remint is their recovery path.

Translated Chat request construction uses the [inline-image budget](../transports/streaming-health.md#translated-chat-inline-image-budget); the shared normalizer counts retained bytes even when a wire-specific drop callback keeps the image attached.
Translated Chat request construction uses the [inline-image budget](../transports/streaming-health.md#translated-chat-inline-image-budget); the shared normalizer counts retained bytes even when a wire-specific drop callback keeps the image attached, rejects inputs above the safe decoded-pixel ceiling, caps native decode work process-wide, and stops queued work when the request is cancelled.

Propagate cancellation through the Anthropic normalization path.

At src/adapters/anthropic.ts:948, pass abortSignal: incoming?.abortSignal to normalizeAnthropicImages. Extend NormalizeOptions with abortSignal so the signal reaches normalizeImageTargets, which uses it to stop queued and admitted image normalization work. Add a focused cancellation regression test.

베이스는 dev입니다. types.ts와 config.ts는 안 건드립니다.

src/server/claude-messages.ts:423 - Claude 메시지 입구도 normalizeAnthropicImages(body.messages)만 부릅니다.

tests/adapters/openai/openai-chat-image-normalization.test.ts:258 - "디코더에 안 간다"는 테스트는 헤더 검사에서 끝납니다.

* refactor(oauth): move in-flight login state into its own module

src/oauth/index.ts reached 2009 lines after #5090 and is not in the
file-size baseline, so it tripped NEW_OVERSIZED at the 2000-line
threshold and failed the gate for every branch whose merge ref includes
current dev.

Raising the cap is not available: updateBaseline only lowers, and
tests/ci-workflows/file-size-ratchet.test.ts asserts it. Trimming the
comments that #5090 added to explain why a raw paste carrying an
explicit state suffix is state-bearing would have fit the budget and is
the wrong trade.

src/oauth/login-flow-state.ts takes the bookkeeping for a login that has
started and not yet settled: the per-provider state, abort and manual
paste maps, the slot helpers, the flow-state reconciliation, and
submitManualLoginCode. The cut follows a seam rather than a line count.
Nothing moved reads or writes a stored credential, and nothing left
behind touches an in-flight slot. index.ts re-exports
reconcileOAuthFlowState and submitManualLoginCode, so the management
routes and the Codex login flow that import them are unchanged.

The raw code#state validation from #5090 moves with submitManualLoginCode
unchanged, comments included.

index.ts ends at 1894 lines.

No local suite, focused test, typecheck or build was run. Exact-head
hosted CI is the execution evidence.

* fix(adapters): keep the bounded image decode inside the file's size cap

src/adapters/openai-chat.ts reached 823 lines against a committed cap of
822 after #5208, so the ratchet reports GREW and the gate fails for every
branch whose merge ref includes current dev.

The overage is one line out of a six-line addition, which does not justify
extracting a module. Naming the options object instead of spreading it
across three lines expresses the same call with the same arguments and
leaves the file at 821, one line under the cap. No comment was removed and
no behaviour changed: normalizeOpenAIChatImages receives the same
tierBias and abortSignal, and the rejection handler still rethrows on a
real abort and otherwise finishes.

No local suite, focused test, typecheck or build was run. Exact-head
hosted CI is the execution evidence.

* test(layout): register the client runtime test in both inventories

tests/clients/client-runtime.test.ts arrived with #5144 and was recorded
in neither scripts/test-layout/layout.json explicit nor
tests/fixtures/test-layout-expected.json, so the layout guards fail for
every branch whose merge ref includes current dev.

The file already sits in the domain directory its name implies, so the
registration is the whole fix: clients in both inventories, matching the
other tests/clients entries. No guard was relaxed and no seed regex was
widened to make the file resolve implicitly.

Reported by the contract-campaign task, which verified the omission by
static blob lookup on dev before raising it.

No local suite, focused test, typecheck or build was run. Exact-head
hosted CI is the execution evidence.

#5226)

Completes review findings left open when #5208 merged: normalizeAnthropicImages now receives the request abort signal at the anthropic adapter and claude-messages passthrough call sites (abortSignal moves onto NormalizeOptions so both entry points accept it), and the decode gate re-checks aborted after admission so a request cancelled while queued never starts a native decode. Adds regression coverage for the post-admission abort and for rejecting an image whose dimensions header cannot be sniffed.
