Replace the single leading/trailing quote strip in fm-decision-hold.sh's normalized_blocked_by with a full quote strip, since a validated task-id slug can never contain a double quote. This removes any positional dependence on where the hold's id sits in tasks-axi's quoted multi-entry blocked_by list; regression tests cover first, middle, last, and only positions. Add a --no-action (alias --none) flag to resolve so a captain decision that routes no follow-up work can be recorded through resolve itself, mutually exclusive with --routed-to, instead of requiring at least one routed task.
