This PR is already merged. Reviewed the full diff — it's a clean documentation-only revert (0 additions, 67 deletions across 4 files) of PR #624.
No blocking issues.
One observation: This reverts the documentation for token-gated approvals but the underlying implementation code from PR #616 (API endpoints, hook scripts, token generation) presumably still exists in the codebase. This creates a docs-code drift where the feature is present but undocumented. If the intent per issue #635 is to replace this approach entirely, consider also removing or deprecating the implementation code in a follow-up.

No agent-mode design concerns. This is a clean documentation revert removing token-gated approval docs in favor of #635. No agent workflows, pre-fetching, structured output requirements, or prompt-level constraints are affected.
