The documentation ignores the second usage of foreach, and only documents the full usage:

foreach EXP as $var (INIT; UPDATE; EXTRACT)

In fact the EXTRACT argument is optional, and the two following forms are equivalent:

foreach EXP as $var (INIT; UPDATE)
foreach EXP as $var (INIT; UPDATE; .)
