#26745 Reduce the amount of markup and images required in the installation

Added three more minor changes.

1. Images now use versioning parameters to avoid the browser cache flush otherwise needed during testing. Unfortunately this only works then moving to the branch. A cache flush is still required following testing to revert.
2. The Prev/Next/Refresh buttons now look ok in IE9 without the gradient fill.
3. Slight positioning adjustment to align refresh with the prev/next buttons.
   Ready for a final test. See tracker 26745.
