Verify rule Md019 and Md021 #1001

When implementing fixes, noticed a couple of things that needed further investigation. Verify that those things did not cause any problems and fixed them.

Found:

  • code in Md019 to handle weird circumstances and tab characters was not used in Md021
  • used as inspiration for fixes in Md021
  • both added new test cases to cover more interesting cases.
