Please see Troubleshooting Extensions with the Activity Log on how to start Visual Studio with the activity log enabled. The blog posts specifies where to find the ActivityLog.xml, and how to obtain data from it.
Please provide us with the log data when opening an issue, as it will make pinpointing the issue so much easier.
