Disable rule at false value.

{
    "doctype-first": true,
    "doctype-html5": false
}

I want to explicitly set the false.
Compatibility with jshint and csslint.

I suggest:

for (var id in ruleset){
    rule = rules[id];
    if (rule !== undefined && rule !== false){
        rule.init(parser, reporter, ruleset[id]);

The build error seems to be unrelated to my changes:

npm ERR! Error: No compatible version found: minimatch@'^0.3.0'

👍 Caused me quite some headache to get around - yeah, I didn't read that line in the Readme.
