This PR expands the MCP Server integration documentation to better distinguish local versus remote client setups, especially for third-party MCP clients that connect to Home Assistant in different ways.

Proposed change
Update the MCP Server instructions to clarify many different scenarios and use cases where users were getting confused about local and remote server setups and requirements. Adds or improves detailed instructions for Claude Connectors, Claude Desktop, Chat GPT, Claude Code, Cursor, etc.
This is a partial revert of #42168 which removed instructions for a very common use case.
