Tech Debt: Replace RuleBuilder.patientExportOnPatient() with an accumulator #4990

This issue asks to replace RuleBuilder.patientExportOnPatient() with an accumulator like RuleBuilderRuleOp that accumulates for rules for as FHIR_READ.
Further, this issue requests that existing functionality be maintained such as for the bugs the above two PRs fixed.
