Cleanup: delete three semantic-prose rows left in the heal/credential carriers

Prose-only cleanup following #7766. Deletes three semantic-prose String declarations left in the heal/credential carriers, and repoints the one stale citation that named a deleted symbol.

What is deleted

Each row was pure narration — incident history, review attribution, and operator-ruling restatement — sitting beside the typed structure that already carries the behavior. DESIGN §4c: an ordinary String declaration whose sole purpose is commentary is misplaced or dead data.

No typed carrier is touched. The Scaffold disposition and its dissolution trigger, the executable refusal and remediation structures, and every witness stay exactly as they were. Nothing is replaced with a comment or a new note.

No semantic consumer

A corpus search over *.dag / *.rs found zero references to any of the three names beyond their own declaration sites. The single remaining mention was a prose citation in docs/plans/shell-to-dag-residual-census-and-arc-completion.md naming ci_heal_commit_push_note; deleting the declaration would have left exactly the §3 stale-citation defect that rule exists to prevent, so that row is repointed to the surviving typed carrier ci_spec.dag ci_heal_commit_push_shell_emit_scaffold, which is what actually carries the disposition and its dissolution trigger.

Verification

• Corpus resolve + compile + typecheck green over 403 sources with the deletions applied.
• ci.yml regenerated from this tree and byte-compared against the committed artifact — the deleted rows have no consumer on any emit path, so the generated workflow is unchanged.
• Exact-head CI is the authoritative check.

Deletions only: 3 files, 7 lines removed, 1 citation line rewritten.
