Ground authorization requests on one access kernel

Summary

Adopts the authorization-grounding implementation from #7578 onto current main:

• enriches std.access.AccessRequest, keeps AccessPolicy as the single policy-to-decision seam, and keeps decision_meet as the conjunction;
• defines exact typed authorization profiles that only project into the kernel, including a shape-only PublicationAdmissionRequest with no publisher policy or admit/deny fold;
• grounds OIDC and POSIX principals in retained external evidence, with failed, empty, and hermetic POSIX observations refusing closed;
• dissolves the provisional Reference, Publish, AudienceScopeTree, and EffectAdmission carriers, routing effect consumers through AccessPolicy;
• hardens the Rust host-effect boundary to require both the AccessDecision carrier and its Permit arm, including a wrong-carrier Permit RED control.

Operator re-scope

The publication placement gate, roster/cutover, and PublicationPlacementGate CI variant were ordered deleted and are absent from main via #7591. This PR therefore carries no publisher model, publication policy, gate evidence plumbing, or pathname census. The former WET placement-gate receipt and eight gate probes retired with that deleted consumer and are not acceptance criteria for this kernel PR.

Carrier retirement does not reject publication or reference control as product capabilities. Publication remains required, but no replacement ontology is selected pending operator design review. Reference-edge admission is a separate deferred capability awaiting a concrete consumer. The generated DESIGN authority records both dispositions and removes the unapproved PublicationPolicy / PublicProjectionPlan / server-check restoration bar.

Current main was integrated by merge commit. Final pushed head: 4d5fcf560a4462a72ea7dec1accf3b5719203dc7.

Validation receipts

Authorization behavior receipt at 320cb2ed1a363940b74bc9120d36179e8a3b797d:

• focused hermetic authorization matrix: 34/34 passed — authorization profiles 3, principal projection 4, effect grant 7, build transport/scaffold 8, build workspace 6, effect I/O 5, and typed-argv POSIX fail-closed 1;
• broader migrated effect-consumer hermetic witnesses: 21/21 passed;
• broader migrated v2 hermetic witnesses: 18/18 passed;
• current merged head 4d5fcf560a4462a72ea7dec1accf3b5719203dc7: 2/2 passed for the direct hermetic POSIX read and its host-effect application, with the latter now requiring the located NotConverged refusal; current main also supplies Repair main red: interpreter primitive surface census #7614’s primitive-surface census repair for the other two prior floor reds;
• worktree clean and local HEAD equals pushed head.

#7578 is superseded by this adopted implementation PR.
