Record stale-authority claims and algebra receiver conflation specimens

Append located specimens to two existing recurring-failure-mode rows:

• a_live_authority_name_carries_a_superseded_claim: an instrument header repeats a withdrawn scoped-fit claim, and a sibling census note describes transport behavior that had already been repaired. Both resolving citations caused incorrect instructions; reading the actual authorities prevented false defect filings.
• state_space_conflation: algebra receiver instantiation reads structural variant members as nominal generic arguments. The retained resolved-node observation names the producing API, source revision, invocation and content identity; the receipt distinguishes static production reachability from an unmeasured population of affected production expressions. Compiler repair belongs to a separate lane.

No compiler behavior, workflow or gate changes. The carrier implementation is separate in #10600.

Review 61013’s measurement-transcription finding is answered by the written response: the receipt names its measurement producer and retains the qualitative withdrawal without copying its numeric output. The manager adjudicated that response as satisfying the finding.

Composition: integrated main 1ee213e26f60a3741cebf9cbd1a925ad58c8b1d9. The generated-artifact driver refused docs/design-failure-modes.md; resolution took that base ref’s projection verbatim, leaving regeneration to CI heal. An identity-set comparison against that exact base found no added or removed projection rows and no duplicate identities. This checks base-row preservation before heal, not final projection agreement; the healed artifact must still be checked against both updated authority rows.

Validation: scoped actual-node execution is linked in the algebra receipt; no compiler repair or defect discharge is claimed by that observation. Formatting and diff checks passed. CI and review must cover the new head after projection healing.

Review 61013 requested removal of the transcribed memory measurements under DESIGN §6, citation of the measurement producer while retaining the qualitative withdrawal of the scoped-fit claim, and an updated Markdown projection.

Commit 7d63339eb9 changes the second receipt in dag/gunbc/recurring_failure_mode/a_live_authority_name_carries_a_superseded_claim.dag: it removes the copied figures and names clever-tern-899’s scoped gunbc compile control with entry src/v2/std/node.dag, retaining the statement that observed demand exceeded that runner’s reported budget. The linked diff shows the removal and replacement directly.

CI heal commit 0572927237 regenerates docs/design-failure-modes.md with the same corrected receipt. At that head, a local assertion check over both files confirmed the producer citation and qualitative withdrawal are present and the copied figures are absent from the receipt.
