• Xcode: 26.0 beta 7 (17A5305k)
• macOS: 26.0 Beta (25A5351b)

Steps to reproduce

1. Use agent mode
2. Normally use.
3. Encounter this issue very frequently, I had to continue many times for one prompt.

Same here. With different models. Switching models sometimes seems to help for a short while, but then the issue happens again.

the same issue has been occurring very frequently recently, and it has already seriously affected usage.
