Incorrect formatting in history tables distorts rendering

Description

When the meta.security member was introduced in #197 the new entry in the version history table in the .md files had one column too many:

Note spurious | right before the issue reference. This results in the issue reference being omitted when the .md file is rendered.

Motivation

Having an issue reference is useful and we don't want to lose that information.
