fix(open-sse): replay title-shaped insufficient_quota refusals once

Summary

A 403 or 451 title refusal carrying insufficient_quota was returned as-is instead of probing the other tool shape. The shape replay now recognizes the token and tries once more before surfacing exhausted credits.

• Behavior: a 403 or 451 title refusal carrying the quota token now probes the other tool shape once before surfacing exhausted credits; no compaction-specific code path added or changed.
• Merge: fix(opencode): only a refusal naming the model clears borrowed tools #15475 lands after fix(open-sse): replay title-shaped insufficient_quota refusals once #15451; rebase on the release tip if it lands first.
• Follow-up: non-official clients with the quota token stay for a later change.
