feat(providers): refresh Fable, Cursor, and Devin catalogs

Summary

• add Claude Fable 5.1 to the verified Anthropic, Claude OAuth/Web, Bedrock, Vertex, and Cursor catalogs with native 1M context, adaptive thinking, five effort levels, 128K output, pricing, parameter normalization, and family fallback
• replace Cursor's broad static list with the curated operator-selected families, ordered quality-first, while preserving thinking/reasoning, Fast, and supported 1M variants
• translate Cursor's synthetic 1M IDs to the canonical wire model plus complete context/effort/fast parameters and keep live-catalog reconciliation authoritative without inventing empty base rows
• replace Devin's broad list with the curated live catalog and provider-bound pricing, including distinct fast-tier rates and output limits
• repair the empty Cursor discovery path so synthetic auto entries do not incorrectly mark every static model unavailable
• refactor the touched Cursor protobuf/catalog decoders so the new-code complexity and dead-code ratchets remain at zero
• repair the current base's rerank-provider route import so the API-route typecheck resolves the canonical DB read-cache module instead of the removed localDb barrel

The branch was rebuilt from the current release/v3.8.51 head and squashed to one commit. The former four-commit PR head remains recoverable locally as codex/archive-12367-before-rebuild-20260902.

Validation

• focused Fable/Cursor/Devin Node regressions: 141 passed
• focused rerank/API-typecheck regressions: 10 passed

npm run test:unit was also attempted locally. It progressed without a reported failure for 17 minutes, then was interrupted because the unchanged warmupScheduler/circuitBreakerFactory Redis-failure tests remained indefinitely blocked on their fake Redis handshake. The PR CI shards are the authoritative full-unit result.
