Roadmap §1.1 — the spine everything else attaches to. No product behaviour yet: no tenancy CRUD, no engine adapter, no messaging. Those are §1.2–1.5.

What is here

Module Purpose
src/config/env.ts Validated configuration. Every value checked at construction; a present-but-malformed value is an error, never a silent fallback.
src/observability/logger.ts Structured logs, correlation id carried via AsyncLocalStorage, credential redaction by key.
src/db/ SQLite + Drizzle, the tenancy schema, and a migration manager that ships migrations inside the binary.
src/api/server.ts RFC 9457 errors, /health (liveness), /readyz (readiness).

Cold start is one command: no data directory, no database, no server to install — migrations apply and /readyz returns ready.

Two decisions worth reviewing

SQLite, not Postgres — ADR-0005 is amended, not replaced. The original chose Postgres immediately; that turned out to cost more than it bought, because the migration verification logic could only be pinned by synthetic rows while no server existed to test against. Switching produced 8 integration tests that apply real migrations and exercise divergence and rollback detection.

The trigger for moving back is concrete: the moment a second process needs the data. The real cost is stated plainly in the ADR — row-level security is unavailable, so the second tenant-isolation layer in docs/04 is deferred. That matters for §1.3, where repository scoping becomes the only thing between tenants.

Migrations are compiled into the binary. A relative migrations directory does not exist beside an installed single-file build, and the failure is silent: no tables at all, reported as "none found". The SQL is imported as text and materialised at startup. Verified by running a compiled binary from an unrelated directory with no .sql files present.

The applied set is verified as an ordered prefix by hash and timestamp. Checked against sqlite-core/dialect.js: the migrator compares each journal timestamp against the single greatest recorded created_at and never inspects individual hashes, so a database holding [A, C] against a build of [A, B, C] passes a membership check and then skips B permanently.

Already reviewed

Two CodeRabbit CLI passes (12 findings, then 7) are applied in 5080bf9 and 425d491. Real defects found and fixed:

  • /readyz echoed the raw driver error — host, port, database and role — to an unauthenticated caller
  • Log context fields were spread unredacted, so a credential placed in a request context reached the log verbatim
  • Credential redaction matched an exact key set, missing clientSecret; now generalised to substring matching rather than patched with a third alias
  • MIGRATE_STRICT parsed as raw === "true", so "yes" silently became false
  • A build with no embedded migrations reported "up to date" and exited zero
  • runMigrations() applied migrations without checking the database matched the build — init() checked, but db:migrate bypasses it

Deliberately not fixed: a claimed missing mkdir in materialise(). Bun.write creates parent directories, verified against a path whose parents do not exist. Raised twice, so it is now answered by an inline comment, a test, and a .coderabbit.yaml instruction.

Review focus

  • src/db/migration-manager.ts — the ordered-prefix comparison. Subtle, and a wrong version either rejects a legitimate database or lets a build skip a migration silently.
  • src/config/env.ts — the boundary where this project's recurring defect lives: accept input, skip validation, default silently. Found in five forms across six stage-0 passes, and twice more here.
  • Schema portability — ADR-0005 claims the Postgres move is mechanical. Easy to write, hard to keep.

79 tests, CI green. Postgres is not required to run them.

Roadmap §1.1. No product behaviour yet; this is the spine everything else
attaches to.

- src/config/env.ts — validated configuration. Every value is checked at
construction, and a present-but-malformed value is an error rather than a
silent fallback. Six review passes on the stage 0 harness found that same
silent-default mistake in five forms; the boundary is the place to stop it.
- src/observability/logger.ts — structured logs with a correlation id carried
through AsyncLocalStorage. Credential-shaped fields are redacted by key, and
a caller-supplied correlation id is validated before it is echoed, since it
reaches both the logs and the error body.
- src/db/schema.ts — the tenancy spine: projects, environments, api_keys.
Environments own the keys, webhooks and quotas; consent will attach to the
project. Only the Argon2id hash and a display prefix are stored for a key.
- src/api/server.ts — RFC 9457 errors, /health for liveness and /readyz for
readiness. Liveness deliberately touches no dependency: a probe that fails
on a database blip gets the container restarted for a fault a restart cannot
  fix.

.coderabbit.yaml gains rules for the boundaries that now have code behind
them — tenant scoping, credential handling, SSRF on tenant-supplied webhook
URLs, and the silent-default habit itself.

50 tests. Postgres is not required to run them.

I had wired drizzle-kit directly and skipped the MigrationManager directive in
the Bun guides. That would have shipped broken: a compiled binary is a single
file, so a relative migrations folder does not exist beside it, and the failure
is silent — the manager reports "none found" and creates no tables at all.

- scripts/embed-migrations.ts generates a manifest importing each .sql with
`type: "text"`, so the SQL is compiled into the binary. It fails on drift in
either direction and on an empty journal, both of which would otherwise build
cleanly and produce a binary that lies about what it can run.
- MigrationManager materialises the embedded SQL at startup and at the CLI entry
point, pruning files this build does not carry so a rollback cannot leave a
  newer build's migration on disk.
- Applied migrations are verified as an ordered prefix by hash *and* timestamp.
Verified against pg-core/dialect.js: the migrator selects work with
`order by created_at desc limit 1` and never inspects individual hashes, so a
  database holding [A, C] against a build of [A, B, C] passes a membership check
and then skips B permanently. Sequence comparison catches rollback,
divergence, gaps and byte-identical duplicates in one rule.
- Migration failure no longer dumps a bundled stack to stderr, and the process
refuses to serve on a schema it cannot verify.

Confirmed empirically: the compiled binary, run from an unrelated directory with
no .sql files present, wrote its own schema out of itself. CI fails if the
manifest drifts from the journal.

…uccess

Eleven of twelve findings applied; one rejected as incorrect.

Real defects:
- /readyz echoed the raw driver error to an unauthenticated caller. The message
names host, port, database and role. It now goes only to the log.
- Log context fields were spread without redaction, so a credential placed in a
request context — where one is most likely to be put — reached the log
verbatim while a directly-logged one was masked. Aliases (accessToken,
cookie, refreshToken and others) added too.
- MIGRATE_STRICT parsed as `raw === "true"`, so "yes" and "1" silently became
false. That is the exact silent-default habit this config module was written
to prevent, sitting inside the fix for it.
- A build with no embedded migrations reported "schema is up to date" and
exited zero, because zero embedded means zero pending. assertPackaged now
runs inside inspect(), so a packaging fault fails at every entry point.

Also: a precise boundary type for the tracking-table rows rather than unknown
plus a double assertion, Bun.fileURLToPath in the generator so a checkout path
containing a space resolves, an explicit Config type at the entry point, and
rationale TSDoc on the exported logging and database helpers.

Rejected: the claim that materialise() must mkdir before writing. Bun.write
creates parent directories, verified against a clean path.

Each defect has a regression test. 66 tests.

Stage 1 has one process, so nothing yet needs what Postgres was chosen for.
Deciding early cost more than it bought: the migration verification logic — the
ordered-prefix comparison that took eight documented traps to get right — could
only be pinned by synthetic rows, because standing up a server was a
prerequisite nobody had met.

Switching immediately produced 8 integration tests that apply real migrations to
a real file, assert the tracking table matches buildSequence(), and exercise
divergence and rollback detection. That is the argument, not the convenience.

Verified against sqlite-core/dialect.js: the tracking table is unqualified where
Postgres puts it in a `drizzle` schema, created_at is `numeric` rather than
bigint, and the selector is the same `ORDER BY created_at DESC LIMIT 1` — so the
trap-7 comparison ports unchanged.

The schema is written to keep the eventual move mechanical: epoch-millisecond
timestamps, JSON as text typed in Drizzle, uuid string ids rather than rowid,
enums as constrained text. Connection PRAGMAs are explicit — WAL, foreign_keys
ON (off by default in SQLite, and the schema relies on cascades), busy_timeout.

DATABASE_PATH replaces DATABASE_URL: a bare path, a file: URL or :memory:, with
a postgres:// value rejected rather than silently creating a file named after
the connection string.

ADR-0005 amended rather than replaced, and states the real cost plainly: row
level security is unavailable, so the second tenant-isolation layer in docs/04
is deferred until the move.

Cold start verified: no data directory, no database, one command — migrations
apply and /readyz returns ready. 75 tests.

…try point

Five of seven findings applied, two rejected with the reasons recorded in code.

Real defects:
- Credential redaction matched an exact key set, so `clientSecret` and
`client_secret` reached the log verbatim. It had already been patched once
for `accessToken`. Adding a third alias would be fixing instances of a class,
so matching is now by normalised substring, with `keyPrefix` and
  `correlationId` pinned as deliberately visible.
- runMigrations() applied migrations without checking the database matches this
build. init() checked first, but `bun run db:migrate` and any direct caller
bypass it, which is precisely how an older build writes to a newer schema.

Rejected, with an inline comment at each site so the reason travels with the
code rather than living in a review thread:
- A missing mkdir in materialise(): Bun.write creates parent directories,
including the nested meta/ one. Raised twice now, so it is also pinned by a
test against a path whose parents do not exist, and noted in .coderabbit.yaml
so a third pass does not spend a slot on it.
- The .env.example header was stale rather than wrong about requiredness; the
wording is corrected to say defaults apply but present values must be valid.

Also: class-level TSDoc on MigrationManager, per-level TSDoc on the log methods
explaining why error takes the error separately, and a typed test double.

79 tests.

The current head adds tenant-scoped APIs, delivery processing, migrations, and engine integration but still permits concrete cross-tenant access paths, unsafe webhook resolution, invalid persisted rules, migration-policy bypasses, and delivery/runtime failures. Merge should be held until these security, correctness, availability, and deployment risks are fixed or explicitly accepted by the owners.

Harden the exit paths.

Three gaps in the process exit contract:

  • If server.stop(false) rejects, the void discards the rejection and the process never exits. The orchestrator then waits for its kill timeout.
  • A second SIGTERM calls shutdown again, so stop() runs twice and two process.exit(0) calls race.
  • await main() has no catch. Any rejection that MigrationManager.init() does not handle itself prints a raw stack and exits with the default code, not a documented one.

Type the fake handle instead of casting to never.

Do not call process.exit from a shared static helper.

assertPackaged() runs inside inspect() and runMigrations(). Both are public and are called from tests, from the CLI, and from startup. process.exit(78) ends the process from library code, so no caller can report context, flush logs, or close the database. Under bun test it terminates the whole run.

Move the exit decision to the entry points, which already own exit codes.

Narrow the TypeScript range to ~7.0.2 or pin 7.0.2.

Add schema-generation validation to CI

bun run db:embed reads only the migration journal and SQL files. It does not run drizzle-kit generate or inspect src/db/schema.ts. A schema change without a generated migration can therefore pass this check. Run bun run db:generate in CI and fail if generated files change.

Nine findings across the PR bot and the CLI. Both independently flagged that
error text leaks credentials, which is the strongest signal either has given.

Real defects:
- The correlation id differed between the log and the response. createServer
minted one for the log context, then derive() minted another because
app.handle() received the original request: header 76e70717…, log 983a86cb.
A customer quoting the id from an error body would find nothing. The resolved
id is now written onto the request before it is handed over.
- Caller fields were spread before the canonical ones, so a field named `level`
or `message` silently overwrote the real value — log forgery by an unlucky
field name. Canonical fields are assigned last.
- Redaction matched field names only, so `{ url: "postgres://u:pw@h/db" }` and
any driver error quoting its connection string logged the password verbatim.
String values, error messages and stacks are now scrubbed for embedded
credentials, bearer tokens and key=value pairs.
- withContext stored an unvalidated correlation id. It is sanitised at the HTTP
edge, but the function is exported and reachable directly.
- CI ran db:embed, which only reads the journal, so a schema.ts change with no
generated migration passed. CI now runs db:generate and fails on a diff.
- A journal entry with no embedded SQL hashed "" and became pending forever:
materialise wrote no file, migrate applied nothing, inspect kept counting it,
and strict mode exited 75 on every start naming nothing. It now throws.

Also: TSDoc on the config predicates and problem(), and precise exported types
in tests rather than unknown.

87 tests. Correlation id verified end to end: header and log agree.

Fifth consecutive review pass finding a defect in logger.ts, and the second was
me repeating the previous fix's mistake: `level`, `time` and `message` were
reserved against caller override, `correlationId` was not, so it stayed
overwritable by the very next call. Patching instances is what produced the
run of five.

Structural instead: CANONICAL_FIELDS declares what the logger owns, one helper
strips those keys from both context and caller fields, and the logger's values
are assigned last. Two guards on purpose — the strip stops shadowing, the
ordering holds even if the strip is weakened. Anything added to that list is
protected everywhere, and the forgery tests iterate it, so a new canonical
field is covered the moment it is declared.

Writing those tests surfaced a distinction worth keeping: caller fields must
never set correlationId, but the context is exactly where it is set, and
withContext validates it on the way in. The test excludes that case explicitly
rather than asserting something false.

Also removed the minimum-length thresholds in scrubValue. "Bearer abc" and
"token=xy" were left intact because eight and four characters were guesses at
what looks token-shaped; a short credential is still a credential.

Projects, environments and API keys behind Repository stores, with the
project-facing API authenticated by key.

The design point: the tenant is derived from the credential and never read from
the request. No project or environment id is accepted from a body, query or
header, so a handler cannot be tricked into acting on another tenant's data.
Requests that try — x-project-id and friends — are rejected with 400 rather
than ignored, because ignoring them would be safe while leaving someone free to
build on the assumption that they work.

Every store method that reaches an environment or a key takes the owning
project id. With SQLite there is no row-level security behind these predicates
(ADR-0005), so they are the only guard, and the tests cross two real tenants to
prove each one holds: A cannot read, mutate, list, revoke or mint against B.

API keys are `bw_{live|test}_{slug}_{secret}` — readable so a leak is
identifiable at a glance and bw_test_ cannot be mistaken for bw_live_ in a
hurry. 256 bits of entropy, Argon2id at rest, plaintext returned once. Lookup
is by the non-secret prefix so verification hashes exactly one candidate.
Revoked, expired, unknown and malformed all resolve to null and render an
identical 401, so probing cannot tell a key that was turned off from one that
never existed. A suspended project stops its environments serving, checked by
join rather than on the environment row alone.

The admin API can mint keys and has no authentication yet, so it is not mounted
unless ADMIN_API_ENABLED is set — an unauthenticated key-minting endpoint is not
something to leave to a reverse proxy's configuration.

Store errors carry types rather than message text, so the HTTP layer maps them
without matching strings that drift. NotFound renders 404 even when the row
exists but belongs to another tenant: distinguishing them leaks its existence.

Verified end to end: mint a key over the admin API, authenticate with it, and
grep the logs — the key appears zero times.

The event envelope, a persistent queue, retry with backoff, dead-lettering,
circuit breaking, replay, and signed webhooks.

Durable by construction: an event is written before it is acknowledged, so a
crash between accepting and delivering loses nothing. gowa forwards
fire-and-forget with no retry and no record (docs/01) — defensible for a
single-tenant tool, not for a proxy other businesses depend on. Enqueue is
idempotent on (eventId, environmentId), so the same event offered twice does
not become two deliveries.

Signatures are timestamped: `t=<unix>,v1=<hmac of "t.body">`. gowa signs the
body alone, so a captured payload replays forever; putting the timestamp inside
the signed material is what stops that, and the verifier is exported for
consumers to copy — a scheme nobody can verify easily is one everybody skips.

Webhook targets are tenant-supplied, so every delivery is an outbound request
to an address a customer controls. Validated at configuration time and again
before each send, with the resolved address checked after DNS to cover
rebinding. Writing the IPv6 test found a bypass in my own guard: WHATWG URL
rewrites ::ffff:127.0.0.1 to ::ffff:7f00:1, so matching only the dotted form
let the hex spelling through to loopback.

Then the end-to-end run found a worse one. The policy was
`allowInsecure: !isProduction`, so https://169.254.169.254/ was accepted
outright in development — and an unset or mistyped NODE_ENV would carry that
into a deployment. It is now its own switch, defaulting to false everywhere,
and the config refuses to start if it is true in production. A security control
must not be disabled by an environment name.

call.offer is excluded from the default filter: bunwa never answers or rejects
a call, so a project that never considered them should not start receiving them
because a device rang.

Verified end to end: metadata address 422, localhost 422, valid https 200,
missing scope 403, and the webhook secret is absent from every response.

The half of §1.3 skipped earlier: system-owned devices, consent granted per
(device, project), and virtual devices as the routing unit.

The claim has three outcomes and the middle one is the product — a customer
already paired for a project gets a second environment with nothing asked of
them, while a different project must ask the phone holder first.

Writing the tests found three flaws in my own logic, each narrower than the
last and each caught only because two real tenants were exercised:

- The first branch keyed on `device.state === "unpaired"`. A device stays
unpaired until an engine pairs it, so a second project claiming the same
number in that window was also treated as the first — granted implicit
consent and access with nobody asked.
- Keying instead on "no other project has consent" still let the same project's
second environment grant itself what its first was still waiting to be given.
The question is whether the device is new to the system, not new to a project.
- A second environment claiming while a request was pending minted a fresh
challenge token, invalidating a message the phone holder may already have
been looking at and asking them the same question twice. A live request is
now reused: one outstanding question per project.

Consent is per project throughout: granting releases every environment of that
project at once, and revoking one project leaves the device's others untouched.
Every decision is appended to an immutable trail with its evidence, because
"prove this customer agreed" needs the message id of their reply, not a boolean.

Phone numbers are normalised to one E.164 form before storage — two spellings
becoming two devices would silently break the lookup the whole flow rests on.

…registry

The abstraction ADR-0002 rests on, which I had skipped — and skipping it is
what let the claim-flow bug hide: a device never leaves `unpaired` without an
engine, so the window where a second project could slip in was permanent.

- DeviceEngine constrained to what an HTTP client to gowa and an in-process
Baileys socket can both express. That constraint is the point: it stops
gowa-shaped assumptions leaking into the control plane.
- Status is two booleans, not a string, because gowa's /devices list `state`
was measured disagreeing with /devices/{id}/status (docs/12).
- SendResult carries acceptedAt and says plainly that acceptance is not
delivery — gowa reported connected for 203 seconds after a silent drop.
- InboundMessage has no senderDisplayName field at all. gowa forwards it and it
is the device owner's private contact naming, not the sender's published
name; the contract has nowhere to put it so it cannot be forwarded by
  accident.
- The conformance suite is written now and run against the fake first: a suite
that has never passed anything proves nothing about the next thing it judges.
Engines that cannot pair unattended report those tests as skipped rather than
passing, so a partial pass is visible.
- A fake in-memory engine, so the control plane can be driven end to end
without gowa, a container or a phone.
- An engine registry with bounded pool capacity, because one process holding
every device is the blast radius ADR-0003 exists to avoid.

…e gowa

Completes the engine half of §1.2. The adapter speaks gowa's HTTP API against
an unmodified image and manufactures everything gowa does not provide.

The reconciler is the substance, not the /ws bridge. gowa forwards no lifecycle
event by webhook at all — proven end to end in docs/12 — so every
device.logged_out a tenant ever receives is produced by comparing polled status
against remembered state. It is a pure function over (previous, observed) and
tested exhaustively: a socket drop with credentials intact is a disconnect, the
same booleans with credentials gone is a logout, a failed poll is neither, and
nothing is emitted when nothing changed.

Writing the polling test found a real defect: the reconciler needs a known JID
to tell a logout from a device that never paired, and /devices/{id}/status
returns only the two booleans. The adapter never learned a JID, so the headline
event would have silently never fired. It now overlays JIDs from the list
endpoint — one call per pass rather than one per device.

Error classification defaults to retryable. gowa panics on error paths and
renders one opaque 500 with prose, so there is no taxonomy to map; a wrong
"fatal" loses a message a retry would have delivered.

Verified against a real container: the adapter passes every unattended test in
the shared conformance suite. The seven needing a paired phone report as
skipped, naming themselves, so a partial pass stays visible. Runbook added,
including the port trap — a dev server on 3000 answers /health with a 200 and
the suite would test the wrong process.

Pin the gowa image and wait for readiness before running the suite.

Two points about this block:

  1. The image uses the :latest tag. A conformance run then measures whichever build was published that day, and a later run cannot reproduce it. Pin a digest or a release tag, so a recorded result stays meaningful.
  2. docker run -d returns as soon as the container is created, not when gowa listens on port 3000. The bun test command that follows can start before the server accepts connections. The adapter then fails every request and the whole suite reports errors that look like adapter defects. Add a readiness wait between the two commands.

Write the attempt row and the delivery state in one transaction.

recordAttempt runs two independent statements. If the process stops between Line 114 and Line 116, the attempt row persists but the delivery keeps state = "pending" with its old nextAttemptAt. Return the circuit columns that claimDue already selects.

Lines 84 and 85 select circuitState and circuitOpenedAt, and Lines 93 to 98 discard both. runOnce in src/delivery/worker.ts (Lines 38 to 42) then issues a second query per delivery to read the same webhook row. For a batch of 20 this adds 20 avoidable queries per pass.

Add the two fields to DueDelivery and return them, then drop the per-item lookup in the worker.

Release the response body.

send reads response.status and returns. It never consumes or cancels response.body. The worker calls send on every pass for the lifetime of the process. An unread body keeps the underlying connection and buffers alive until garbage collection, which accumulates in a long-running worker.

Add coverage for the circuit breaker.

The suite exercises enqueue, retry, dead-lettering, replay, and the SSRF checks. It never drives environment_webhooks.consecutive_failures past CIRCUIT_FAILURE_THRESHOLD, so no test observes the circuit opening, blocking a pass, or admitting a probe. Two defects in that path are reported on src/delivery/worker.ts and would both pass this suite unnoticed.

Add tests that:

  • open the circuit through repeated failures and assert that the next pass attempts nothing;
  • advance the clock past the probe window, fail the probe, and assert that the circuit blocks again.

Cover revoked and expired keys, and scope refusal.

The comment on Lines 73 and 74 states that a revoked key must be indistinguishable from an unknown one, but the loop on Line 76 only presents unknown and malformed values. Pin the validated DNS address before sending webhooks.

The sender resolves and checks an address, then fetches the original hostname, allowing DNS rebinding to redirect the connection to a private or link-local address. Use a supported mechanism that binds the connection to the validated address while preserving the original hostname for Host and TLS SNI.

The serious ones first.

**Revoking consent was undone by the next claim.** hasStandingClaim counted
only granted and pending, so after a phone holder revoked a project the device
looked unclaimed, the next claim took the "new device" branch, and
grantImplicitConsent set the row back to granted. A revoked project could
re-grant itself, and a different project could take the same branch. Revoked
and denied are decisions, not absences; only expired means nobody decided. The
implicit grant no longer upserts, so it can never overwrite a refusal.

**The admin API was permitted in production.** It mints API keys and has no
authentication. Not-mounted-by-default is not enough for that; the config now
refuses to start.

**Tenant URLs reached gowa unvalidated.** /send/link is fetched server-side by
gowa (docs/12), so validating only bunwa's own outbound requests left the same
SSRF hole one hop further along. Link and media URLs are now checked against
the same policy before being passed on.

**Conformance skips were recorded as passes.** A callback that returns early is
a pass, so seven paired-device checks reported green for an engine that had
never run them — and I told you they were "visible, not hidden". They now use
test.skipIf against a declared capability: 51 pass, 7 skip.

Also fixed: the circuit breaker re-arms on every failure rather than opening
once and then letting everything through 60s later; a blocked environment's
deliveries are deferred rather than re-claimed every pass, which had one tenant
filling the batch and starving all others; logout and purge no longer swallow
gowa's error envelope and report success while the session is live; close()
wakes parked subscribe() consumers instead of hanging them for the life of the
process; a degraded device reports recovery; the delivery log returns recent
rows rather than the same fifty oldest; the breaker resets when the target URL
changes; claim, respondToConsent and revokeConsent are transactional; shutdown
awaits the delivery pass in flight and is idempotent.

Rejected with the reason recorded in code and in .coderabbit.yaml: pinning the
connection to the validated DNS answer. Bun's fetch cannot bind a connection to
an address while preserving Host and SNI, and Bun.dns exposes only lookup. What
is achievable is done; closing it needs an HTTP client over Bun.connect, now
tracked in docs/08 under stage 2.

§1.3 is finished: POST /v1/devices/claim returns the three outcomes over HTTP,
with the challenge token and the global device id both withheld from the
project.

**Delivery logs were scoped by project, not environment.** A key for
grande/staging could read and replay grande/production's deliveries. Smaller
than crossing projects, but the environment is the unit of isolation precisely
so staging cannot see production.

**A revoked binding was reactivated by a later consent grant.** activateBindings
updated every binding for the project, so one the phone holder had specifically
taken away came back when an unrelated environment's consent was answered. Only
bindings actually waiting on that answer are activated now.

**Three address-blocking gaps.** Link-local matched only the `fe80:` prefix, so
the rest of fe80::/10 — febf::1 and friends — went through. IPv4-compatible
`::7f00:1` was not decoded at all. A trailing dot evaded the local-name check,
so `localhost.` and `metadata.google.internal.` were accepted.

Also: re-opening a consent request kept the previous answer's timestamp and
evidence, so the trail read as though the customer had already replied to an
outstanding question; assertOk accepted a response with no code as success, and
a 5xx carrying a well-formed envelope was trusted; document sends dropped the
filename, which for the PDF requirement is the whole point; one environment
could still fill the worker's batch and starve the rest; a deleted webhook
retried forever instead of dead-lettering; replay could reset a delivery the
worker was mid-attempt on; expiresAt accepted unparseable input and stored NaN,
producing a key that never expires; ApiKeyStore.touch could raise an unhandled
rejection and take the process down over a timestamp; closeAll abandoned the
remaining engines if one threw; the project slug pattern was duplicated and
could mint keys prefixOf cannot parse; a webhook secret of any length was
accepted; degraded devices reported recovery while still logged out.

The .coderabbit.yaml tenancy rule targeted src/tenancy/**, which has never
existed — so the strictest rules in the project matched nothing while appearing
to cover the most sensitive code. Now src/stores/**.

Corrected in passing: I reported /health and /readyz returning 404 as a real
regression. It was my probe using a single-label hostname; both return 200.
The probes are now registered before the route plugins anyway, so they cannot
end up behind a plugin's auth.

251 tests, 7 skipped.

Keep degraded set until a usable observation.

Line 90 clears degraded for every successful poll. If that poll still reports an unusable device, no recovery event is emitted. A later reconnect can then emit only device.connected, particularly when disconnectedSince is null after an engine outage.

Keep the marker until isUsableNow is true. Emit device.recovered then, clear the marker afterwards, and suppress another threshold device.degraded event while the marker remains set.

Add the tenant scope to defer.

defer takes a deliveryId alone and updates the row without an environmentId or projectId predicate. Today the only caller is runOnce in src/delivery/worker.ts, which supplies ids returned by claimDue, so there is no cross-tenant path at present. The signature still allows a future caller to pass an id from a request without a tenant check.

70-83: 📐 Maintainability & Code Quality | 🔵 Trivial | ⚡ Quick win

Move the engine-kind choice out of the route.

Lines 72 and 75 name the engine kinds "gowa" and "fake" and implement a fallback order in the control plane. Two problems follow from this.

  1. The route decides engine policy. A new engine kind requires a change here rather than in src/engine.
  2. If a deployment ever registers a fake pool next to a gowa pool, a real claim silently pairs against the fake engine when the gowa pools are full. src/index.ts registers only a gowa pool today, so the path is inert in production, but the fallback encodes that risk in the route.

Move the ordered kind preference into EngineRegistry — for example a chooseAnyPool(preferredKinds, assigned) method — and let the route ask only for capacity.

As per path instructions: "Flag any conditional on engine kind outside this directory."

**The transactions did nothing.** A previous commit claimed claim(),
respondToConsent() and revokeConsent() were atomic. drizzle's bun-sqlite
transaction does not await an async callback, so writes either side of an await
both survive a rollback — verified directly, two inserts and a throw, both rows
still there. Code that looks transactional and is not is worse than none,
because it stops anyone looking. Replaced with explicit BEGIN IMMEDIATE /
COMMIT / ROLLBACK that awaits the callback, and pinned with a test that also
asserts drizzle's own behaviour, so a future release fixing it tells us the
workaround can go.

**The consent state machine is now enumerated, not patched.** This was the
fourth consecutive review finding a bug in it, and every previous fix handled
whichever combination had been surfaced — device state, then another project's
consent, then this project's pending request, then a revoked one. Each was
correct and each left the next case open. The branch is now exhaustive over
(this project's status x anyone else's standing claim) with the table written
out above it, and .coderabbit.yaml asks reviewers to name the wrong row rather
than propose another special case. This pass's bug was an expired row: it
counted as no claim, so the new-device branch ran, onConflictDoNothing wrote
nothing, and pairing proceeded with consent still reading expired and no audit
row at all.

**Bounded pool capacity had no effect.** Nothing ever wrote devices.enginePoolId,
so countByPool saw every device as unassigned and choosePool always read zero
usage. The pool is recorded after provision succeeds.

Also: GOWA_BASE_URL is redacted in describe(), which is written to the log at
every start and may carry engine credentials; shutdown stops accepting traffic
before tearing down what serves it, and engine cleanup can no longer prevent
the process exiting; and URLs handed to gowa are resolved and checked, since
gowa fetches them itself inside the container — the residual rebinding gap is
documented at both sites rather than claimed as closed.

256 tests, 7 skipped.

POST /v1/devices/:ref/messages covers the six v1 types behind one discriminated
route, and the response says `accepted`, never `sent`: the engine took the
message, WhatsApp has not acknowledged it, and for up to 203 seconds after a
silent drop the engine cannot tell the difference (docs/12). An outbound row is
kept so a later ack can be matched, and unacked sends can be swept to
message.undelivered rather than disappearing.

Idempotency is mandatory, not optional. The primary traffic is OTP, where a
duplicate is a second code in the customer's chat and a lost one is a failed
login — and optional idempotency is idempotency nobody uses. A replay returns
the original response byte for byte with an Idempotent-Replay header; the same
key with a different body is a 409, because replaying the first answer would
report the wrong message as sent.

Writing the tests exposed a hole worth more than the feature: **nothing
transitioned a binding out of pending_pairing.** Engine events were never
connected to the control plane at all, so a device could pair and its binding
would stay pending for ever, and device.logged_out — the event this project
exists to deliver — was produced by the adapter and then dropped.

src/engine/consumer.ts is that bridge, and the only place engine events cause
writes. It activates bindings waiting on a scan, but never those waiting on a
person: a pending_consent binding is waiting on the phone holder, and
activating it on a socket event would hand a project access nobody agreed to.
Fan-out resolves from the device outward, so an event cannot reach a tenant
with no active binding, and the global device id is replaced by the binding's
before anything is queued.

265 tests, 7 skipped.

Reject credential-bearing file: database URLs before logging.

This branch accepts any file:// value and removes only the prefix. For example, file://user:pass@host/path becomes user:pass@host/path, which describe() returns as database at Line 202. Startup logging can expose pass. file://localhost/var/db.sqlite also becomes the relative path localhost/var/db.sqlite.

Parse the URL, reject userinfo and non-local hosts, and return the local pathname. Add regression tests for these cases.

This test does not put the binding into a disconnected state, so it does not test what its name says.

dropConnection and logout change the engine only. The setup at Lines 67-73 deliberately drives device.connected through handleEngineEvent so the stored binding reflects reality. This test skips that step, so virtualDevices.status stays active and the 503 comes solely from the engine rejecting the send. A regression that leaves a logged-out binding marked active still passes.

Drive the logout event through handleEngineEvent, as the setup does, then assert the response.

The test named "sends once" never asserts that only one send reached the engine.

It checks that the two responses carry the same messageId and that the replay header is set. Both hold even if the engine accepted two messages, because the second response could come from a fresh send that happened to be deduplicated afterwards. This is the property the module exists to guarantee, and it is the one property the test does not measure.

countSince joins a table it does not use and counts in JavaScript.

devices.enginePoolId is written with a possibly wrong engine kind and is never read back. This PR adds the pool-assignment column so bounded capacity has an effect, but the two halves of the contract do not meet: the pairing route records the assignment while collapsing every non-native kind to "gowa", and the send route selects a pool without consulting the recorded assignment at all.

  • src/api/routes/messages.ts#L103-L104: replace registry.list().find((p) => p.engine.kind !== undefined) with a lookup of the device's enginePoolId in the registry, and raise UnavailableError when that pool is not registered.
  • src/api/routes/devices.ts#L85-L87: pass pool.kind through to DeviceStore.assignPool instead of pool.kind === "native" ? "native" : "gowa", and widen the engineKind parameter so the fake pool is not recorded as gowa.

**The idempotency key was recorded after the send.** A crash between the engine
call and the write, or two concurrent requests both finding no key, produced a
second OTP — the exact thing the feature exists to prevent. The key is now
reserved *before* the side effect, with the unique primary key as the arbiter:
one caller inserts, the rest replay or are told a request is in flight. A failed
attempt releases its reservation, or the caller's retry would be blocked for
ever. Also made the request hash order-independent, since JSON.stringify
preserves insertion order and a client serialising the same payload differently
on retry would have got a 409.

**The `granted → active` row of my own consent table was wrong.** It read
status without expiry, and both grant paths write an expiresAt while nothing
transitions a granted row — so a grant older than 24 hours still read as
granted while its row claimed to have expired. Consent does not lapse; only a
*pending request* can. That is now explicit in one function, and the table
comment says so.

Concurrency: withTransaction spanned an await on a process-wide handle, so a
concurrent request's writes landed inside someone else's transaction and a
second BEGIN IMMEDIATE simply failed. Transactions are now serialised per
handle.

Also: a late `delivered` ack could overwrite `read`, making a message appear to
regress; recordAck and markUndelivered wrote from a bare id with no tenant
scope; outbound rows could pair one environment's binding with another's id,
now prevented by a composite foreign key; reading a message's state required an
active device, which is exactly when a caller cannot get one; sends went to the
first registered pool rather than the one holding the device; envelopes carried
an empty project slug and a hard-coded "gowa"; the engine consumer discarded
its stream task so shutdown could exit mid-write; and an SSRF test asserted the
rejection without asserting gowa was never called.

279 tests, 7 skipped.

**A failure after a successful send released the reservation.** The catch ran
for any error inside the try, including one raised after the engine had already
taken the message — WhatsApp holds it, the key is freed, and the caller's retry
sends a second OTP. Exactly the bug the reservation was introduced to prevent,
moved past the point the fix guarded. The route now tracks whether the engine
accepted: if it did, the reservation is *completed* with the failure so a retry
replays it and learns the send happened; only a failure before the engine
releases the key.

That is the second time a fix of mine has recreated the problem it addressed a
few lines away, so the test drives it directly — it breaks the bookkeeping that
runs after a successful send and asserts the key stays claimed.

**Transactions now get their own connection.** Serialising transaction callers
was not enough: a plain write on the shared handle, issued after an await
inside a callback, ran inside the open transaction and was committed or rolled
back with it. A dedicated connection makes that impossible rather than
unlikely, with SQLite's own locking arbitrating. In-memory databases cannot be
reopened, so those still serialise on one handle.

Also: an expired idempotency key reported in_flight for ever until an unrelated
sweep ran, and is now reclaimed in place; the engine consumer returns its
iterator before awaiting the task, so an idle stream cannot stall shutdown;
shutdown races the HTTP drain against a deadline, because Bun's stop(false)
waits indefinitely and one stuck handler would block every later step; and
cleanup uses allSettled so a consumer that fails to stop no longer skips the
worker, the engines and the exit.

281 tests, 7 skipped.

Limitation #2 from the assessment. gowa's entire automation surface is one
global static string: one reply, all devices, all senders, no conditions
(docs/01). This is the replacement, and it matches the brief directly — a
message from a certain number, to a certain number, containing a certain
format, triggers something.

Pattern safety is the substance, because rules are written by tenants and run
against every inbound message, so a catastrophically backtracking pattern is a
denial of service against every *other* tenant. Three mitigations: RE2
semantics only (no backreferences or lookaround — the constructs that
backtrack), compiled and length-bounded at save time while a human is watching
rather than at 3am against real traffic, and a time budget at execution because
JavaScript's engine is not RE2 and the first rule is enforced by inspection.
Nested quantifiers are refused outright.

Loop protection has two independent guards. A reply is itself a message, so a
rule that answers a message would otherwise answer its own answer for ever:
bunwa-originated events are excluded, and a chain depth cap holds even when
origin marking is lost across an engine boundary. A pattern that exceeds its
budget counts as *not* matching — firing actions on an unevaluated condition
would be worse than not firing.

Evaluation is pure — an event and rules in, a plan out, nothing sent or
written — which is what will make the dry run honest: it runs exactly this
code rather than an approximation of it.

Writing the test for the brief's own example found a real bug: I folded case

Completes stage 1. Rules are stored per virtual device rather than per device,
because two projects sharing one phone must automate it differently and neither
should see the other's — with the same composite foreign key as outbound
messages, so a rule cannot pair one environment's binding with another's id.

The dry run is the point of the surface. A rule engine that can only be tested
by messaging a real customer is one nobody will dare change, so evaluation is
pure and POST /rules/:id/test runs exactly the code the live path runs.
`actionsExecuted` is always empty and no code path could populate it: the
evaluator cannot send, write or reach an engine. The guarantee rests on the
shape of the code rather than on a flag being read correctly.

Rules are prepared afresh for a dry run rather than reusing a stored compile,
so a pattern that has become unsafe is reported there instead of at runtime. A
rule that no longer compiles is skipped on the inbound path rather than
throwing — one bad rule must not stop every other rule on the device.

Deliberately carved out and recorded in docs/08: **actions are planned, not
executed.** Executing a reply means sending on a rule match, which needs the
per-binding rate limiting, reply-rate cap and rule circuit breaker that belong
with stage 2 hardening. Shipping the trigger without them would let one
mis-written rule message a customer in a loop bounded only by the depth cap.

308 tests, 7 skipped.

Handle iterator.next() failures inside the consumer loop.

The try/catch at lines 182-190 only guards handleEngineEvent. await iterator.next() at line 179 is not guarded. If the engine's stream throws — for example a transient failure surfacing through the iterator — the exception propagates out of the for loop and rejects task.

Nobody observes that rejection until stop() runs await task.catch(() => undefined) at line 200, which discards the error with no log entry. If stop() is never called during normal operation, this becomes an unhandled promise rejection instead.

Either way, the consumer for that engine silently stops processing events with no retry and no visibility into why.

As per path instructions for src/engine/**, "unclassified failures must default to retryable, never fatal." An iterator failure here is currently fatal and silent.

🛡️ Proposed fix: log and stop cleanly instead of letting the failure propagate unseen

Document that timedOut reports an overrun, and that a match may be discarded.

If exec returns a successful match after the budget expires, Line 108 discards it and reports matched: false. The evaluator then adds the rule to timedOut and skips it. This is deliberate, but the TSDoc does not state that a valid match can be thrown away. Add that to the doc comment, because the caller uses timedOut to disable a rule.

Consider validating the action discriminant at save time.

Line 92 validates condition.op at runtime, because a rule definition can arrive from tenant input rather than from typed code. The action loop makes no equivalent check. An action with an unrecognised type passes validation, reaches PlannedAction, and then fails or is silently dropped in whichever component executes actions.

Reject unknown action types here, next to the operator check, so the failure happens while a human is present.

readPath still reads inherited properties.

The segment deny list stops __proto__, constructor and prototype. It does not stop other inherited members. A condition on the field toString resolves to Object.prototype.toString and passes exists, and asText then stringifies the function source into a contains test. No mutation is possible, but the rule matches for a reason the author did not intend.

Check own properties instead. That also makes the explicit deny list redundant for constructor and prototype, although keeping it does no harm.

♻️ Proposed refactor

**Pairing credentials were fanned out to every active binding.** device.qr
carries a code anyone who sees it can scan to take over the WhatsApp account,
and device.pair_code is the same secret in another form. fanOut ran for every
event, so both went to every *other* project sharing that phone — the precise
opposite of what the consent model is for. They are now withheld by name, held
in a set so a new pairing event cannot opt into fan-out by omission.

**The expired-key takeover was not atomic.** Checking expiry in code and then
updating let two callers both see the stale row, both take it over, and both
believe they held the reservation — a duplicate send, which is the failure that
table exists to prevent. The predicate is now in the WHERE clause.

**A lapsed consent counted as a standing claim for ever.** hasStandingClaim
compared the persisted status while the decision branch used effectiveStatus,
so the "expired + no other claim" row of my own table was unreachable: a device
nobody ever answered for stayed claimed permanently.

The nested-quantifier check is now a balanced-paren scanner rather than a regex
over regexes — it missed ((a+)b)+ entirely, because the quantifier sits inside
the nested group where no fixed pattern lines it up. Brace quantifiers count
too: (a{1,})+ backtracks exactly like (a+)+.

Rules were being evaluated against the raw engine event, so a rule written
against `data.text` — as the brief and docs/05 both describe — matched nothing.
They now see a documented subject, including the device's own JID, without
which "from a certain number, to a certain number" cannot be expressed at all.

Also: the dry run required no scope though it reveals what a project automates;
forward actions bypassed the SSRF policy the delivery path applies; recordAck,
countSince, RuleStore.prepared and disableForTimeout took ids without a tenant
scope; complete and release could clobber a newer reservation; conditions were
short-circuited so a later pattern's timeout went unreported; numeric strings
were parsed as dates, making every numeric comparison silently false; recovery
was emitted twice for a device that both disconnected and degraded; the fake
engine dropped events queued before close; a granted consent kept a live
challenge token; voice-note requests were silently downgraded to plain audio;
gowa received the raw URL rather than the normalised one that was validated;
resetDatabase leaked its handle; and updatedAt is now refreshed by Drizzle
rather than by every store remembering to.

316 tests, 7 skipped.

…d everything

Four findings from the 07:52 review that I never fetched: two were already
covered by the CLI batch (numeric-string comparison, and the non-atomic key
takeover), two were not.

**An empty condition array bypassed the check that exists to catch it.**
`{ match: { all: [] } }` produced one group containing no conditions, so a
group-count check passed — and `[].every(...)` is true, so the rule matched
every event and fired its actions. The validation reached the exact outcome it
was written to prevent. Counted by conditions now, not by groups.

**Quantified alternations were accepted.** `(a|aa)+` is the classic
overlapping-branch blowup and compiles perfectly; the scanner only looked for
quantifiers *inside* quantified groups. Deciding whether branches genuinely
overlap needs real analysis, so any quantified alternation is refused —
conservative on purpose, since refusing a safe pattern costs an error message
and accepting an unsafe one costs the node. `^(foo|bar)$` is unaffected because
it is not quantified.

The subject is also truncated to 4KB before matching, which bounds the damage
from a pattern the checks do not catch.

Stated plainly rather than papered over: static regex analysis cannot be
complete and JavaScript cannot pre-empt a running match, so the budget detects
a slow pattern rather than preventing it. Terminating one needs a worker that
can be killed, now tracked in docs/08 under stage 2 alongside the DNS pinning
gap.

320 tests, 7 skipped.

Move the reference resolution into DeviceStore.

bindingFor queries virtualDevices directly from a route and re-implements the tenancy predicate. The scoping here is correct. The concern is placement: every future route that accepts a device reference must repeat this and(environmentId, or(id, alias)) shape correctly, and one omission is a cross-tenant lookup.

DeviceStore already owns environment-scoped device reads. Add a resolveRef(environmentId, ref) there and call it from the route.

Add a cross-tenant write case.

The isolation test covers reads and the dry run covers a cross-tenant id with 404. No test attempts a mutation of another tenant's rule by id. Assert the consent row as well as the outcome.

The test proves the claim reaches the new-device branch. It does not prove what that branch wrote. grantImplicitConsent upserts over the expired row (src/stores/device-store.ts Lines 294-309), and that upsert replaced an earlier onConflictDoNothing that left the row expired while pairing proceeded. This test is the closest witness to that write, and it would still pass if the upsert regressed to doing nothing.

Type the actions array instead of accepting t.Any().

Action in src/rules/schema.ts Lines 36-41 is a five-member discriminated union. This schema accepts any array element, and definitionFrom then asserts the result with as RuleDefinition["actions"]. prepareRule inspects only reply and forward, so { "type": "nonsense" } passes validation, passes preparation, and is persisted as a rule action. The operator receives 201 for a rule that cannot do anything.

Describe the union in the schema so the route rejects it.

   }),
-  actions: t.Array(t.Any(), { minItems: 1 }),
+ actions: t.Array(actionSchema, { minItems: 1 }),

Consider narrowing op at Line 23 to a literal union over OPERATORS for the same reason. prepareRule already rejects an unknown operator, so that one is a clarity improvement rather than a gap.

As per path instructions: "Strict TypeScript: flag any use of any or unknown where a precise type is possible."

GET /devices/:ref/rules enforces no scope.

The three mutations call requireScope(auth, "manage:rules", path). This route does not. A key with scopes: [] can therefore read every rule on the binding, including all match conditions and all actions.

That contradicts the reasoning stated for the dry run at Lines 115-117: a dry run is gated because it "reveals a rule's conditions and its captures". This route returns the stored conditions and actions outright, which is strictly more.

Decide the intended read scope and apply it. If listing is meant to be readable by any key for the environment, record that decision here, because the adjacent comment currently argues the opposite.

+    requireScope(auth, "manage:rules", path);
+    return RuleStore.list(auth.environmentId, await bindingFor(auth, params.ref));

The integration test at src/api/__tests__/rules.integration.test.ts Lines 117-128 mints a zero-scope key and asserts 403 for POST only. Extend it to cover GET once the intended scope is settled.

Repeat the tenant predicate on the statement that reads or writes, not only on the preceding check. RuleStore proves ownership with a separate requireBinding or requireRule read, then issues the real statement keyed on a bare rules.id. The guard and the statement are separated by an await, and the statement carries no tenant scope of its own. Nothing is exploitable today, because every public method performs the check first. The pattern is one refactor away from a gap, and src/stores/** is the boundary where that matters most.

update lets a rename collide with an existing rule name.

create refuses a duplicate name for the same device (Line 39). update writes name with no equivalent check, so an operator can rename one rule onto the name of another rule on the same binding.

Names are not just labels. src/api/routes/rules.ts Line 133 decides the dry-run verdict with result.matched.includes(stored.name), so two rules sharing a name make that verdict ambiguous.

Check the name against the other rules on the binding before writing.

Disable timed-out rules in the consumer

When result.timedOut is non-empty, persist the disablement through RuleStore.disableForTimeout. evaluate currently returns rule names, but this method requires ruleId; return IDs or retain a name-to-ID mapping before calling it.

Hendro8899 and others added 5 commits August 23, 2026 22:07
docs/08 lists seven conditions for calling stage 1 finished. Five are now
executed as tests; two need a handset and a container topology and are marked
skipped rather than quietly counted — the conformance suite already showed how
easily a skip reads as a pass.

Written from the roadmap rather than from the implementation, which is the
point: every other test in this branch asks "does the code do what I wrote?",
and this one asks "does the system do what was promised?". Four of the five
failed on first run.

Two were real:

- **`GET /v1/devices/{ref}` did not exist.** docs/06 documents it, and it is
what an integrator polls to see whether the customer has replied to a consent
challenge. Without it the claim flow has no completion signal short of
waiting for a webhook. Added, with an environment-scoped lookup that accepts
either the id or the project's own alias.
- **The claim route returned 201 for every outcome.** docs/06 documents 202 for
`awaiting_confirmation`, and the distinction matters: 201 tells an integrator
a resource was created when in fact nothing happens until a human replies.

Two were my assertions rather than the code: the fake engine's completePairing
moves the engine but not the binding — that crossing is the consumer's job, as
it should be — and device.connected legitimately fans out alongside the message
being counted.

325 tests, 9 skipped.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>
The claim route now returns 202 for awaiting_confirmation, per docs/06. An
earlier test pinned 201 and I pushed the route change without re-running the
whole suite, so CI caught what I should have.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>
**GET /devices/:ref/rules enforced nothing.** The three mutations require
manage:rules; the read did not, so a key with no scopes could enumerate every
rule on a binding — its match conditions, the numbers it watches, and its
actions. That directly contradicted the reasoning I had written two functions
away to justify gating the dry run.

**A timed-out rule was never actually disabled.** evaluate returned rule
*names*, and a name cannot address a row, so the disable step could not be
written — the budget detected a pathological pattern and then left it running
on every subsequent message. Ids now travel with the prepared rule.

**The insecure-targets switch skipped address validation entirely.** It is
meant to relax the https requirement, which is a transport concern. Returning
before DNS resolution made a development config the route to the cloud
metadata endpoint through gowa.

**An ack selected by engine id alone.** That id is unique only within the
engine that issued it, so two devices could produce the same value and an ack
could mark another tenant's message delivered. Narrowed by device, and an
ambiguous match now refuses to guess.

Also: rule actions accepted t.Any(), so an unrecognised type was stored and
then silently ignored at evaluation — a rule that looks configured and does
nothing; RuleStore proved ownership with one read and then issued the real
statement on a bare id, with an await in between; and the subject-truncation
test asserted only `timedOut`, which is false whether or not truncation
happens, so it passed either way.

328 tests.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>
…a time

Five of fifteen findings landed in api-key-store, and one — the sender skipping
DNS validation under allowInsecure — was the identical bug I had fixed in the
gowa adapter one commit earlier. Patching the reported instance and not the
class is now the most reliable defect source in this branch, so this pass
started by scanning every .update() and .delete() in src/stores for a tenant
predicate rather than by reading the findings.

Fifteen unscoped mutations, of which these were real:

- **ApiKeyStore.revoke** (critical) proved ownership with a separate read and
then updated on a bare id, with an await between. It now scopes the statement
and requires revokedAt to be null, so revoking twice cannot move the
timestamp and lose when it actually happened.
- RuleStore.remove, DeliveryStore.recordAttempt, defer and replay had the same
  guard-then-unscoped-statement shape.

project-store is left unscoped deliberately — it is the tenant root — and the
idempotency sweep is a maintenance job over expired rows.

Two more that mattered:

- **Authentication had a timing oracle.** An unknown prefix returned without
hashing while a known one paid for Argon2id, which tells an attacker when a
prefix guess is right — the expensive half of finding a key. Both paths now
verify, and the candidate query is capped so a collision cannot turn one
  request into many hashes.
- **A granted consent expired after 24 hours.** The TTL belongs to the
question, not the answer, so every consent silently lapsed and the project
had to ask the customer again the next day. The column is nullable now and
only a pending challenge carries a deadline. I fixed the update branch first
and had to be told the insert branch of the same function still set one.

Also: the webhook payload is an allowlist rather than a spread, so a new engine
field cannot reach tenants by default; engine message ids are unique per
environment; the unacked sweep and the DNS lookup are both bounded; a rule is
disabled after three budget breaches rather than one; `?` counts as a
quantifier when nested; and a date no longer compares against a number.

330 tests.

Co-Authored-By: Claude Opus 5 (1M context) <noreply@anthropic.com>
Three findings, down from fifteen, and the shape says the class-level audit
worked where it reached. Nothing came back in the allowlist payload — fifty
lines of new, previously unreviewed code — and only one landed in src/stores.

That one was critical, and it was a `.select()`. My sweep scanned `.update()`
and `.delete()` for a tenant predicate and stopped there, which is the
too-narrow failure I had flagged as possible before running it.
EnvironmentStore.isServable decides whether a tenant may send and took only an
environment id.

Extending the audit to reads found thirteen unscoped ones, of which twelve are
deliberately cross-tenant: authentication itself, where the credential is what
establishes scope; the delivery worker and the ack sweep, which serve every
tenant by design; the operator fleet views; and the project store, which is the
tenant root. Rather than leave that judgement to be re-derived next time, each
is now marked `@crossTenant` with its reason, so the exceptions are a decision
on the record instead of an omission that looks identical to one.

Also: a cyclic cause chain — which a retry wrapper produces by re-wrapping the
error it caught — recursed until redaction's depth cap happened to stop it. It
now has its own lower cap, so the log says the wrapping looped rather than
emitting redaction's generic marker for an unrelated reason.

332 tests.

Disable rules after the configured consecutive timeout count.

This branch only logs timeout IDs. timeoutCounts and TIMEOUT_BREACHES_BEFORE_DISABLE are never used, so a rule that repeatedly exceeds its budget stays enabled and blocks evaluation on every inbound event.

Increment and reset the per-rule count here. When the count reaches the threshold, disable that rule through a tenant-scoped RuleStore operation.

Bound DNS resolution by the attempt budget.

AbortSignal.timeout starts only after resolve() completes. A stalled resolver can therefore keep send() and the sequential delivery worker blocked for longer than ATTEMPT_TIMEOUT_MS.

Apply the remaining attempt budget to the lookup as well. Return a failed SendOutcome when resolution exceeds that budget.

Enforce fairness before applying the global claim limit.

Over-claiming by four does not prevent starvation. If one environment owns the first batchSize * 4 due rows, the filter keeps only perEnvironment rows and the batch can contain fewer than batchSize deliveries. Other environments remain unselected, even when they have due work.

Make DeliveryStore.claimDue select a bounded number of rows per environment, and then fill the global batch. Extend that selection to the required virtual-device queue boundary.

…ched off

Three findings, all mine, all in code I had just written.

**Group prefixes were read as repetition.** Adding `?` to the nested-quantifier
scan caught `(a?)+` and also rejected `(?:foo)+` and `(?<year>\d{4})+` — a
non-capturing group and a named one, both entirely ordinary. Prefixes are now
recognised as syntax before `?` is tested as a quantifier.

Chasing that surfaced a worse version of the same problem. Blanket-refusing
quantified alternations rejected `(?:PAY|SEND)+`, which is a pattern from this
project's own brief. A check that blocks the obvious use case is one that gets
removed, and removing it unguards the dangerous patterns too — so the rule is
now the real condition rather than a proxy for it: branches overlap when one
can consume a prefix of what another consumes. `a|aa` overlaps; `POST|PUT`
shares a first letter and diverges at the second, so it cannot. Anything that
is not a plain literal is still refused, because it cannot be compared cheaply.

Fixed counts are allowed inside a quantified group for the same reason:
`(\d{4})+` consumes exactly four every time and is deterministic, while
`(\d{1,4})+` can split the same input many ways.

**A test asserted less than its name promised, for the second time.** "evaluate
reports the rule id" only checked that prepareRule preserved it — a regression
dropping the id in evaluate, which is the function that has to report it, would
have passed. evaluate now takes an injectable match budget so the timeout path
can actually be reached, since the compiler refuses to accept a pattern slow
enough to trigger it naturally.

Also closed the database handle the exit-criteria test creates directly.

337 tests.

233-233: 🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Reject numeric strings that overflow.

Line 233 accepts strings whose numeric conversion is Infinity. For example, "9".repeat(400) compared with gt against "1" + "0".repeat(309) becomes Infinity > Infinity and returns false, although the first value is larger.

Return null when the parsed number is not finite.

`((a|aa))+` is exactly as catastrophic as `(a|aa)+`, and a single extra pair of
parentheses hid the `|` from a top-level scan. Nesting is the cheapest possible
evasion of a check like this, so the analysis now follows groups to any depth —
while still accepting `((?:PAY|SEND))+`, so the recursion did not turn into a
blanket refusal of nested groups.

Third round running where the fix I had just written had a gap one level
deeper: `?` missed group prefixes, then the blanket alternation refusal
over-blocked, now the top-level scan missed nesting. Each narrower than the
last, which is what converging looks like, but pattern analysis is clearly
somewhere I under-generalise.

Also added the TSDoc `evaluate` was missing — it is the function the dry run
depends on being pure, and that is worth stating where someone changing it will
read it.

Ten references across nine files, all predating ADR-0005. The chaos plan named
Redis as a component to kill, the architecture diagram drew it as a box, the
requirements said a start needs it, and ADR-0004 listed it as a hard dependency
and a new failure domain. None of that is true — the delivery queue is a SQLite
table, chosen because one process owns the data and a durable queue that needs
no server is the smaller system.

Stale docs are worse than thin ones here. A chaos plan naming a component that
does not exist reads as covered, and an architecture diagram with a box nobody
deploys sends the next person looking for it.

Corrected throughout, with an inline comment at the chaos line recording what it
used to say and what the equivalent failure now is: a corrupt WAL, a full disk,
a handle held open by a crashed process. ADR-0004's "bad" list now names the
real trade instead — the queue shares the database's write lock, and the trigger
for splitting them is the same as the Postgres trigger.

Also added that Postgres trigger to stage 2 explicitly, since row-level security
is unavailable on SQLite and that leaves repository scoping as the only tenant
guard — worth stating where the hardening work is planned, not only in the ADR.

.coderabbit.yaml now tells the reviewer this, so a pass does not spend findings
suggesting Redis for queues or Postgres row-level security before the trigger is
met.

Replace the contradictory “resolve-then-pin” wording.

The src/delivery/sender.ts instruction at Lines 45-56 says not to request connection pinning. The delivery rule at Line 126 says “resolve-then-pin”. This can produce a false finding against the sender implementation. Require immediate resolution and validation of every returned address without requiring connection pinning.

Allow the one-time API-key response and secure verification.

The block permits plaintext to be shown once at creation, then says to flag any path that returns a plaintext key. It also says to flag any comparison, which can include verification of the presented key against its stored hash. Restrict the rule to logging, persistence, later responses, and direct plaintext comparisons.

Separate tenant checks for writes and reads.

The rule requires every Drizzle query to have a where clause binding a tenant key. INSERT statements do not have a where clause, so this wording can report valid inserts and does not require the inserted row to contain environmentId or projectId. Define separate checks for tenant values on inserts and tenant predicates on reads, updates, and deletes.

`indexOf("]")` closes a character class at an *escaped* bracket, so `[\]]` — a
class holding a literal `]` — ended early and everything after it was reparsed
as pattern structure. That rejected `([\](a|aa)])+`, where the alternation sits
inside the class and matches literal characters rather than branches.

The interesting part is how it happened. Two scanners in this file handle
escapes correctly; the two I added later, chasing the nested-alternation
finding, used indexOf. Same file, same session, same author, inconsistent on
the thing that file exists to get right. There is now one helper, so they
cannot disagree again.

Removing the special case for a leading `]` while doing it: `[]]` is a
bracket-containing class in legacy regex and an empty class plus a stray `]`
under the `u` flag these compile with. Carrying a rule for a dialect the
project does not use is how a scanner drifts from the language it is scanning.

Both are already in docs/08 under stage 2, but the roadmap says what a stage
contains, not what was learned while deciding to defer something. todo.txt
carries the context needed to act on these without re-deriving it.

The regex worker entry records why it matters more than its priority suggests:
four consecutive review passes each found a gap in the static analysis, every
one narrower than the last. That is convergence, and it is also evidence the
approach is hard to get exhaustively right — which is the argument for the
worker, since it makes all the static checks defence-in-depth rather than the
primary guard.

The DNS entry records something the roadmap line omits: the gap applies twice
over. Once to bunwa's own sender, and once to URLs handed to gowa, which
resolves them entirely outside our control — that second case may only be
closeable by fetching media ourselves and passing gowa a local reference.
