Code Review
This pull request updates several documentation files to correct outdated file paths, migration names, and script locations. It also documents the "Doc Accuracy Gate" in the Copilot instructions and significantly expands the check-fabricated-docs.mjs script with extensive allowlists for planned environment variables, file references, and endpoints to prevent false positives in CI. Feedback on the changes includes addressing a bug in check-fabricated-docs.mjs where line numbers are incorrectly calculated due to character index shifts after stripping code blocks, removing a duplicate entry for OPENCODE_API_KEY in the environment variable allowlist, and cleaning up a redundant block of endpoints in the endpoint allowlist.

The lineOf(text, m.index) call uses m.index which is the match index from textNoCode (where fenced code blocks have been stripped). Because characters are removed from text to produce textNoCode, m.index is smaller than the actual index in text. This causes lineOf to calculate incorrect (earlier) line numbers for any findings that occur after a fenced code block.\n\nTo fix this, stripCodeBlocksAndFences should preserve the original character indices and newlines by replacing non-newline characters inside fenced code blocks with spaces instead of removing them entirely:\n\njavascript\nfunction stripCodeBlocksAndFences(text) {\n return text.replace(/[\s\S]*?/g, (match) => \n match.replace(/[^\\n]/g, " ")\n );\n}\n

Thanks @oyi77 — the slimming-down worked: no i18n dump, no personal But I can't, because of how it's made to pass. The script change adds ~438 entries to the allowlists, and a large part of that is self-defeating:
A doc-accuracy gate whose allowlist pre-exempts 400+ identifiers that don't exist anywhere is self-defeating: the only thing those entries can ever do is suppress a future "fabricated" flag when something starts documenting them. And those exact env-var families ( What the allowlist should contain (and what's genuinely needed to make it CI-wireable) is only true false-positives:
Could you drop the 417-entry "Planned features" env block (and the equivalent pre-blessed FILE_REF/ENDPOINT blocks), exclude the design-spec doc dirs from the scan instead, and keep only the genuine false-positive suppressions? Then the gate is honest and I'll merge it gladly — it's a genuinely useful gate. Leaving open. 🙏 |

Thanks @oyi77 — this is a big improvement over the earlier version, and you clearly acted on the round-2 feedback. 🙌 Concretely:

Thanks @oyi77 — the focused re-submit is much cleaner than #3528, and the idea of a fabricated-docs gate is genuinely valuable. But I rebased this onto current Running 1. Env-var indirection not recognized (5 FPs)

CRITICAL: Variable sub is undefined in the hook scanning loop. The hook detection loop (lines 872-887) defines name from the regex match (line 873), but line 879 incorrectly references sub which is only defined in the CLI command scanning loop (line 850). This will cause a ReferenceError at runtime or silent skip of the CLI_ALLOWLIST check.

…C+D) (cheaper-inference#4622) C — scripts/quality/validate-release-green.mjs (npm run check:release-green): reproduces the release-equivalent validation (typecheck, eslint, db-rules, public-creds, full unit, vitest, ratchets, optional --with-build package-artifact) against the current working tree and classifies each red as HARD (real defect, exit 1) vs DRIFT (ratchet — reported, never affects exit / never blocks). Pure helpers exported + orchestration behind a direct-run guard; unit-tested. D — .github/workflows/nightly-release-green.yml: runs C on the active release branch nightly (and on workflow_dispatch) and opens/updates a single tracking issue on HARD failures. Never a required check, never touches a contributor PR. Closes the gap where the full gate (ci.yml) only ran on the release PR, so reds accrued silently on release/** and surfaced in 40-min layers at release time. Non-blocking by construction; drift is the maintainer's to rebaseline at release. Co-authored-by: Diego Rodrigues de Sa e Souza <diego.souza@cdwasolutions.com.br>

Summary
Re-submits the doc-accuracy gate as a focused PR per @diegosouzapw's review on #3528. The previous PR mixed the gate with 140+ unrelated files (~42 i18n CHANGELOG dumps, personal gitignore entries, and conflicting release-bump commits). This PR is a clean 11-file change off current
