fix(sse): send the sanitized error body on non-streaming provider failures - #19097
Reported privately by @serenity-X, who wrote the fix and the regression test. The commit keeps their authorship.
When the provider pipeline returned an error for a non-streaming chat request, the non-streaming leg spread the raw upstream Response over the sanitized error result. The client got the upstream body verbatim, including internal fields and file paths, plus every upstream header, including Set-Cookie. The streaming path already rebuilt the response from the sanitized body.
The leg no longer relays the raw response. The client gets the one createErrorResult built:
sanitized body and Content-Type;
Retry-After when the upstream sent it, parsed before the raw response is discarded.
Raw upstream headers stay on upstreamHeaders for server-side classification.
Combo routing reads this same sanitized body. The first version of this PR left the merge queue because tests/integration/combo-failover-e2e.test.ts failed: context_window_exceeded was not in the public identifier vocabulary, so the body said bad_request and a protected priority combo stopped falling back on context overflow. The same applied to billing_hard_limit_reached, credits_exhausted and authz_insufficient_balance. All four are fixed public identifiers and are now in SAFE_PUBLIC_ERROR_IDENTIFIERS. A new test pins every routing code against the sanitized body.
Known change in behaviour: a retry hint that an upstream sends only inside its error body no longer reaches combo pacing. The Retry-After header still does.

The whole tests/integration directory, run locally, reported 18 failures across 6 files (plugins, compression, OpenCode, MCP origin, CLI settings, combo breaker). Re-running those 6 files gave the same 16 failing test names with and without this diff; both runs hit my 15-minute limit, so that comparison is partial. CI is the authority for that suite.

The non-streaming leg relayed the raw upstream Response object to the
client when the provider execution pipeline returned an error: the client
received the upstream body verbatim (internal fields, file paths) plus
every upstream header, including Set-Cookie. The streaming path already
rebuilt the error response from the sanitized body; only the
non-streaming path differed, because the leg spread the raw
result.response from the pipeline outcome over the sanitized one
createErrorResult() had built.
Drop the raw relay. The sanitized response keeps the upstream Retry-After
hint (parsed from the raw response before it is discarded), and the raw
upstream headers stay available to server-side classification through the
separate upstreamHeaders field, so cooldown/fallback behaviour does not
change.

Bug Fixes
Non-streaming errors now return sanitized responses instead of exposing raw upstream error details and headers.
Error responses retain the upstream Retry-After hint when available.
The non-streaming provider error path now returns a sanitized error result instead of the raw upstream response. It preserves the parsed Retry-After delay. Unit tests cover sanitized 400 responses and the retry header on a 429 response.
Changes: Provider Error Handling
Layer / File(s)
Summary
Sanitized error result open-sse/handlers/chatCore/nonStreamingProviderLeg.ts, changelog.d/fixes/0000-nonstream-error-sanitize.md
The shared-pipeline error path passes the parsed upstream retry delay into the sanitized error result and no longer returns the raw upstream response. The changelog describes the sanitized response and header handling.
Tests check that 400 responses omit upstream body details and headers while retaining the expected error type. A non-streaming 429 test checks that the response retains Retry-After.
Some rate-limited accounts may use a local cooldown instead of the provider’s reset window. Address or accept this bounded retry risk before merging.
Security architecture risk: Low · up to 0c6f0
The fix strengthens isolation by preventing raw upstream error bodies and headers from reaching clients. However, some upstream reset information is also lost from downstream retry decisions, potentially allowing retries before a shared provider account is ready again.
Retained concerns
Medium · reliability · inferred: The sanitized client response also remains the metadata source for outer account-exhaustion decisions. When upstream retry hints are enabled and a failure supplies a future x-ratelimit-reset without a usable Retry-After or body hint, the base response exposed that reset to classification, while the head response drops it. Classification can therefore substitute local backoff for the provider reset window, weakening failure containment through premature retries against the affected shared connection or model. Preserving upstreamHeaders protects internal classification, but the outer callers do not consume that field.
Security review details
Security Blast Radius
inferred — The direct confidentiality boundary is upstream-provider error content reaching clients on the changed non-streaming failure path. The secondary availability concern follows shared provider connection or model cooldown state. Deployment-wide or cross-tenant impact was not quantified.
Security Findings and Attack Paths
inferred — The response replacement removes the inspected path by which an upstream error could supply arbitrary response fields and headers to a client. The remaining concern is conditional retry-policy drift, not an established new confidentiality exploit, privilege gain, or authentication bypass.
Trust Boundaries and Controls
observed — The error-result type distinguishes the public Response from rawMessage, upstreamErrorBody, and upstreamHeaders. The changed branch preserves that separation, and the immediate classification consumer deliberately reads the internal headers rather than relying on the public response.
Resilience and Maintainability Implications
observed — Retry-After survives the response replacement. The fallback classifier can also recover a retry delay from body-derived hints, and otherwise uses local backoff. Consequently, loss of x-ratelimit-reset does not imply that all cooldowns disappear; the concern is loss of the authoritative reset window under the stated conditions.

Explanation
The description is thorough and follows the template's main sections. It has a Summary that explains the change, the bug, and the fix. It names the changed test file and gives evidence for the test results. It includes a Reviewer Notes-style warning about a known behaviour change, where a retry hint sent only in the upstream error body no longer reaches combo pacing. It also cites the inherited base-branch failure (#18791). Some template items are not filled in. The Related Issues section has no issue link. The Target Branch checklist is not ticked, so the description does not confirm the base branch is release/v3.8.x. The Validation checklist is not ticked, and the change type is not chosen. The Tests Added Or Updated section lists no files by name in the template format, although the test file appears in the body. Coverage Notes are missing. The author should complete these items so the description matches the template. The required content for the change is present, so the check passes.

Pin the converted Retry-After value in the 429 test.
The test sends upstream retry-after: "7". The production path converts that to Retry-After: "7". The test checks only that the header is present, so a wrong conversion, such as "7000", would still pass. Assert the exact value.

Preserve the retry delay from status restatement.
When agentrouter returns a quota error as HTTP 403 without Retry-After, applyStatusRestatement changes it to 429 and supplies a 60-second retry delay. The pipeline does not carry that delay in outcome.result, so parseRetryAfterMs(outcome.result.response) returns null. The rebuilt 429 response then omits the retry hint used by clients and cooldown classification. Carry the restated retryAfterMs through ProviderExecutionOutcome, and use it before parsing the upstream header. (raw.githubusercontent.com)

Pass retained upstream headers to account exhaustion.
The non-streaming error result retains raw upstream headers, but both exhaustion calls pass only sanitized response headers. For an API-key 429 with a future x-ratelimit-reset and no usable Retry-After, fallback classification can use the local cooldown instead of the provider reset.
Apply the same change to the second exhaustion call.

The test's upstream answers 400 with error.code: "context_window_exceeded". Probable cause, not proven: once nonStreamingProviderLeg.ts stops returning the raw upstream Response, the combo layer can no longer read that structured code, so it does not fall back to the next model.
