fix(gemini): cool the connection on project-wide billing refusals - #18848
Gemini can reject an entire Google project with messages such as "free tier is not available for this project", "prepayment credits are depleted", or "project has exceeded its monthly spending cap".
Those errors are unrelated to the requested model. Today a passthrough connection can get a brief per-model lockout and be selected again immediately for other models from the same project.
Recognize those three specific responses and temporarily park the affected connection for six hours while keeping the credentials and active setting intact. Other 403s, ordinary quota 429s and transient 5xx errors follow the existing logic.
The patch is limited to one classifier, the account-unavailable path, and tests covering matching and non-matching cases.
Park the affected Gemini connection on documented project-wide free-tier, prepaid depletion or monthly spending-cap errors. Keep ordinary model quota errors and transient failures unaffected; cover classification cases.

Bug Fixes
Gemini project-wide billing and free-tier restrictions are now recognized separately from per-model quota limits. Affected connections are marked unavailable for six hours, with fallback enabled.
Ordinary rate limits, temporary service errors, and content restrictions continue to be handled separately.
The change adds a classifier for selected Gemini project-wide billing and free-tier failures. When a failure matches, account handling records its classification and HTTP status, resets backoff, and marks the connection unavailable for six hours.
Changes: Gemini Project-Block Handling
Layer / File(s)
Summary
Project-block classifications and tests src/sse/services/geminiProjectBlock.ts, tests/unit/geminiProjectBlock.test.ts
The classifier maps specified Gemini 403, 402, and 429 responses to project-block types. Unit tests cover these cases and responses that return no classification.
Before existing quota-scope handling, the account path checks for a project-block classification. On a match, it records the classification and status, sets backoff to zero, marks the connection unavailable for six hours, and returns with fallback enabled.
cheaper-inference/OmniRoute#18770: Both changes classify provider billing or credit exhaustion as an account failure. This PR adds Gemini project-block classifications; PR #18770 handles OpenAI's credit_balance_exhausted signal.

Add caller-level coverage for Gemini billing refusals.
The matching fixtures currently test only classifyGeminiProjectBlock. The existing Gemini 429 markAccountUnavailable test uses "too many requests", so it exercises the ordinary model-lockout path instead. Add a focused markAccountUnavailable test with a matching billing response, such as status 402 and "Your prepayment credits are depleted.". Assert the gemini_prepay_depleted error type, testStatus: "unavailable", a six-hour persisted cooldown, shouldFallback: true, and cooldownMs of 21600000. This test can detect removal of the new branch or an incorrect cooldown and fallback while the classifier tests still pass.

classifyGeminiProjectBlock so project-wide billing refusals are checked before
the existing-cooldown early return. When a project block is detected, retain the
later of the current cooldown expiry and the project-block expiry so a shorter
existing cooldown cannot reduce the block duration.

add regression coverage confirming a denied-access body that also reports
deactivation returns `null` from the classifier.
