fix(authz): read REQUIRE_API_KEY the same way in gate and warnings - #18550
The request gate and the two startup warnings parsed REQUIRE_API_KEY differently. The gate (isRequireApiKeyEnabled, which calls isFeatureFlagEnabled) accepted only the exact strings true, 1 and yes. The Next startup guard (src/lib/startup/nonLoopbackApiKeyGuard.ts) and the omniroute serve warning (bin/cli/utils/serverHost.mjs) trimmed and lower-cased the value first.
With TRUE, True, YES, " true" or "true\r", the gate was off and /v1 served requests without a key. Both warnings read the same value as on and printed nothing, so an operator reading the logs would think keys were required.
The cause is three separate hand-written comparisons for one setting.
Fix
New src/shared/utils/requireApiKeyValue.mjs exports isRequireApiKeyValueEnabled(value). It returns true for true, 1 or yes, ignoring case and surrounding whitespace. The file is plain .mjs so the CLI can import it without a TypeScript loader.
The gate, the Next startup guard and the CLI warning all call it. The gate applies it to the resolved value, so a DB override goes through the same parser as the env value.
.env.example and docs/reference/ENVIRONMENT.md list the accepted values.
Behavior change
Deployments that set REQUIRE_API_KEY to a case or whitespace variant of true, 1 or yes now require a key, and clients without one get 401. The operator asked for that, but clients will see it, so the changelog fragment says so.
No other value changes meaning. on, enabled, FALSE, 0, no, off, empty and unset are still off, and the warnings still fire for them. on stays off on purpose. The gate never accepted it, and accepting it now would go beyond fixing the mismatch.
Other boolean feature flags keep the exact-match parser. Changing isFeatureFlagEnabled for every flag would also turn on flags such as ALLOW_API_KEY_REVEAL and DISABLE_CONTEXT_WINDOW_CHECKS for values like TRUE. That needs its own discussion.

TDD. New test tests/unit/require-api-key-parse-consistency.test.ts. For 21 env values (unset, empty, true, TRUE, " true ", "true\r", "\tyes", on, FALSE and others) it sets the env and checks the gate result. With no DB override, it also checks that the Next guard and the CLI warning fire exactly when the gate is off. One more case covers a DB override (TRUE is on, on is off).
On the base commit with only the test added:
# tests 22
# pass 12
# fail 10
The 10 failures are the 9 case and whitespace variants plus the DB override case. For example:
not ok 6 - REQUIRE_API_KEY="TRUE": gate is on and, with no DB override, both warnings agree
With the fix:
# tests 22
# pass 22
# fail 0
Validation
Node 22.23.4, on top of the current release/v3.8.53.
Related unit tests: 122 existing files that touch the gate, the startup guard, serverHost.mjs, feature flags or auth policy, plus the new test. On the base, 1267 of 1269 pass. With this PR, 1289 of 1291 pass, and the 22 extra tests are the new file. The same two tests fail on both sides and are unrelated: client-bundle-no-server-only-10692.test.ts and material-symbols-removed.test.ts.
typecheck:core, check:changelog-integrity and check:ai-attribution pass. ESLint and Prettier on the changed files pass (serverHost.mjs is in the ESLint ignore list).
check:boundary:ratchet --base-ref passes at 777 edges, the same as the base. Without --base-ref it fails at 777 against the 774 baseline ceiling on the base too.
tsc -p tsconfig.typecheck-api.json reported the same 244 errors with and without this change. I ran that on an earlier base of this branch (5332fe88e), not on the current one.
On 5332fe88e I ran 78 combinations of an env line and a parser (the CLI .env parser, the Docker bootstrap-env parser, dotenv and Node util.parseEnv) and passed each resulting value to the gate and both warnings. In every row where the gate was off, both warnings fired on a non-loopback bind. Docker --env-file, systemd EnvironmentFile and Windows set were modeled from their documentation only.
Not tested: a production build, the standalone output, a Docker image, Electron, and a real server start. Nobody has read a real startup log or sent a real request that got 401. The warnings were checked by calling the functions the startup paths call. tests/unit/bootstrap-env.test.ts could not run because better-sqlite3 is not installed in my environment, and it fails the same way without this change.
The startup warnings still read only the env, not a DB override. If the env says true and the Feature Flags page turned it off, the gate is off and the warnings stay silent. That gap existed before and this PR does not change it. Fixing it means loading the DB in the guard, which I left out of this PR.
This PR leaves /api/settings/feature-flags (GET and PUT) alone. It still reports the raw value, and the dashboard compares it exactly. With env " TRUE ", the Feature Flags page shows REQUIRE_API_KEY as off while the gate requires a key. This affects only the display.
The changelog fragment still uses the 0000- placeholder and will be renamed to the PR number.
The request gate accepted only the exact strings "true", "1" and "yes".
The Next startup guard (nonLoopbackApiKeyGuard.ts) and the CLI serve
warning (serverHost.mjs) trimmed and lower-cased the value first. With
REQUIRE_API_KEY set to "TRUE", "True", "YES" or " true", the gate stayed
off and /v1 served requests without a key, while both warnings treated
the value as on and printed nothing.
All three now call isRequireApiKeyValueEnabled() from
src/shared/utils/requireApiKeyValue.mjs: true, 1 or yes, ignoring case
and surrounding whitespace. Every other value, including "on", is still
off, so no value that used to mean off and does not look like one of
those three is turned on. The gate applies the same parser to a DB
override. The two warnings still read only the env value, so a DB
override of "false" with env "true" leaves the gate off and both
warnings silent, as before.
Other boolean feature flags keep the exact-match parser.
The API key requirement now uses consistent parsing across request protection and startup exposure warnings. Case-insensitive true, 1, and yes values, including those with surrounding whitespace, enable the requirement; other values, including on, leave it disabled.
Documentation
Updated environment-variable guidance to clarify which REQUIRE_API_KEY values enable request protection and which do not.
The request gate and startup exposure warnings now use a shared parser for REQUIRE_API_KEY. The parser enables trimmed, case-insensitive true, 1, or yes values. Tests and documentation describe the accepted values and the existing /v1/models exception.

Summary
Shared parser and consumers src/shared/utils/requireApiKeyValue.mjs, src/shared/utils/featureFlags.ts, src/lib/startup/nonLoopbackApiKeyGuard.ts, bin/cli/utils/serverHost.mjs
A shared parser enables only trimmed, case-insensitive true, 1, or yes values. The request gate and both exposure warnings use it.
Parsing checks and documentation tests/unit/require-api-key-parse-consistency.test.ts, .env.example, docs/reference/ENVIRONMENT.md, changelog.d/fixes/18550-require-api-key-consistent-parse.md
The test checks parsing, agreement between the gate and warnings, and database override values. The documentation and changelog describe accepted values and the existing /v1/models exception.
cheaper-inference/OmniRoute#18873 — It describes a WebSocket path that bypasses the API key gate. This parser change does not address that enforcement gap.
Merge Risk: Moderate · up to 9729e
Close the test database before deleting its data directory to keep the test reliable on Windows before merging.
Security architecture risk: Low · up to 9729e
The change strengthens authentication for enabled values containing uppercase letters or surrounding whitespace. Existing key validation and authenticated-session exceptions remain unchanged. Operators should expect some previously anonymous clients to receive authorization failures, and rollback can restore the old mismatch. No newly introduced bypass was established in the inspected paths, but coverage is incomplete.
Retained concerns
No architecture-level concerns identified.
Security review details
Security Blast Radius
inferred — The behavioral change extends beyond a single HTTP policy: dependent WebSocket bridge, A2A, and Cursor authorization paths inherit the normalized setting. For previously disabled case or whitespace variants, inspected callers select stricter authentication behavior rather than expanding anonymous access.
Trust Boundaries and Controls
observed — The central policy’s dashboard-session and WebSocket descriptor-handshake exceptions remain unchanged. WebSocket bridge authentication still validates a key or dashboard session when required; plain metadata handshakes remain distinct from bridge authorization. These are existing boundaries, not newly introduced bypasses.
observed — The startup checks remain advisory: non-loopback exposure with a disabled environment value produces a warning rather than blocking startup. Host selection and loopback checks were not changed by the parser replacement.

Explanation
The description covers the template's main parts: a Summary, Related Issues, Target Branch, Validation, Tests Added Or Updated, and Reviewer Notes. It explains the root cause, the fix, the behavior change, and the testing done. Some template items are not filled in as the template asks. The Coverage Notes section is missing, though the template asks for it when src/ or bin/ changes. The Validation checklist boxes are not ticked, and the Target Branch is set to release/v3.8.53 while the template asks for the active release/v3.8.x branch with a stated check. The Summary also does not say which checks were confirmed against the template's list. These gaps are minor, so the description is mostly complete.

Close the test database before removing TEST_DATA_DIR.
The flag reads and override writes open the database singleton. removeFeatureFlagOverride() does not close it. On Windows, fs.rmSync() can fail when the SQLite file remains open. Call closeDbInstance() before removing the directory. (raw.githubusercontent.com)
