Problem and behavior

Large request bodies can legitimately sit silent well past the flat 30s direct response-start window: prefill scales with input size, and a plain large-context request carries no reasoning_effort selector for the existing reasoning ceiling (REASONING_READINESS_CEILING_MS) to key on — so the fetch-layer guard cut healthy slow-prefill upstreams at exactly the floor and surfaced 504s the stream-readiness layer would have permitted.

Changes

  • resolveFlatDirectHeadersFloorMs raises the flat floor by payload tier: ≤512 KB unchanged (a small body overrunning the probe still smells like a dead connection, per fix(network): direct dispatcher stalls opencode-go and command-code until service restart #10214), >512 KB ×2, >2 MB ×3, capped at 180 s — composing with the existing effort/local raises via raise-only semantics.
  • New env OMNIROUTE_DIRECT_HEADERS_VOLUME_ADAPTIVE (0/false/off pins the flat behavior), registered in .env.example and docs/reference/ENVIRONMENT.md.

Validation

TDD — tests/unit/direct-response-start-timeout-volume-tier.test.ts failed before the fix on 4/7 (the resolver returned the flat 30000 for every tier), passes 7/7 after:

not ok 2 - 2. >512KB doubles the floor (expected 60000, got 30000)
not ok 3 - 3. >2MB triples the floor (expected 90000, got 30000)
not ok 4 - 4. tier raise is capped at the 180s volume ceiling (expected 180000, got 30000)
not ok 7 - 7. resolver binds the body once per request (expected 60000, got 30000)
  • Regressions: direct-response-start-timeout-reasoning-504.test.ts + direct-response-start-timeout-settled-guard-12861.test.ts — 17 pass.
  • npm run check:env-doc-sync in sync · eslint clean on the touched module.
  • Only the files the failing test proves need changing are touched; no test weakened or deleted.

Notes

  • Raise-only composition: the volume tier applies last as max(base, min(base * factor, 180s)), composes order-independently with the effort and local/LAN raises, and can never pull the window below an operator override or the local floor (test 4 pins the cap, test 5 pins the never-lower rule).
  • Byte length uses TextEncoder on the already-serialized body string the resolver binds once per request (same binding the reasoning-effort regex uses) — no extra serialization on the request path.
  • Default (OMNIROUTE_DIRECT_HEADERS_VOLUME_ADAPTIVE unset) = enabled tiering as described; the kill switch restores today's flat behavior exactly.
  • Base: release/v3.8.53 (active release branch); aware of the 3.8.55 v4-feature retarget policy.

Summary

Summary by CodeRabbit

  • New Features
    • Direct response-start timeouts now adapt to request size: payloads over 512 KB receive a 2× window, and those over 2 MB receive a 3× window, capped at 180 seconds. Existing higher timeout floors are preserved. Adaptive scaling can be disabled with the OMNIROUTE_DIRECT_HEADERS_VOLUME_ADAPTIVE setting.
  • Documentation
    • Added the new setting and its default, size thresholds, timeout cap, and options for disabling adaptive scaling to the environment-variable reference.

The direct response-start timeout now scales with request-body size when adaptive scaling is enabled. The change adds a setting to disable scaling, tests for size tiers and timeout floors, and documentation of the setting.

Changes: Direct response-start timeout

Layer / File(s) Summary
Volume-aware timeout calculation and validation
open-sse/utils/directResponseStartTimeout.ts, tests/unit/direct-response-start-timeout-volume-tier.test.ts, .env.example, docs/reference/ENVIRONMENT.md, changelog.d/fixes/18404-direct-headers-volume-tier.md
The timeout uses 2× above 512 KiB and 3× above 2 MiB, capped at 180 seconds without reducing a higher existing floor. Setting OMNIROUTE_DIRECT_HEADERS_VOLUME_ADAPTIVE to 0, false, or off keeps the flat window. Unit tests cover the thresholds, cap, floor, setting, and resolver behavior. The environment references and changelog describe the setting.

Change: Bug fix

Merge Risk: Low · up to 44145

The change scales the direct response-start window by request size, and existing timeouts are preserved. Two small follow-ups are worth doing: make one unit test independent of environment variables, and count body bytes without allocating a copy of large request bodies. Neither is expected to block the merge.

Security architecture risk: Low · up to 44145

The larger timeout windows preserve existing caller cancellation and retry safeguards and grant no new access. The main residual risk is additional memory and CPU work for large payloads, including when adaptive timing is disabled; its production impact has not been measured.

Retained concerns
No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The incremental exposure is resource use in the serving process for eligible string-body direct requests: additional encoding work and, where the flat floor is effective, longer upstream waiting. It is not a new destination or privilege boundary. Bun and loopback requests bypass this bounded direct-fetch branch.

Trust Boundaries and Controls

  • observed — The inspected chat entrypoint invokes route-level authentication before body admission. The authentication guard accepts a valid API key or authenticated dashboard session, while anonymous access remains possible when API-key enforcement is disabled. These controls predate this PR; authentication cannot be assumed mandatory for every deployment.

Resilience and Maintainability Implications

  • observed — Each timeout-resolution call now encodes the entire bound string into a temporary Uint8Array merely to obtain its UTF-8 length. This occurs even when adaptation is disabled or the retry ceiling dominates the result, and it is repeated when the resolver is called again. The extra allocation is established, but process exhaustion or a failed admission guarantee is not.

Hardening Proposals

  • proposed — Use a non-copying UTF-8 byte count, skip counting when volume adaptation cannot affect the result, and cache the immutable bound body's size across attempts. This would reduce shared-process resource overhead and make the opt-out more complete without changing timeout semantics.

Full details: Title check

Explanation

The title "fix(sse): scale the direct response-start window by request payload size" follows the Conventional Commits format with the allowed type "fix" and scope "sse". It names the main change: the direct response-start window now scales with request body size. The changeset matches this: resolveFlatDirectHeadersFloorMs and the new resolveVolumeAwareDirectHeadersTimeoutMs apply payload-size tiers in open-sse/utils/directResponseStartTimeout.ts.

Full details: Description check

Explanation

The description covers the problem, the changes, validation, and notes. It explains why large bodies need a larger window, lists the code and env-var changes, and reports the TDD results and regression runs. It does not fill the template's Related Issues section, which has no issue numbers. It also omits the Target Branch checklist, the Tests Added Or Updated list, and the Coverage Notes section. It also does not list the changed test file under the Tests Added Or Updated heading, though the Validation section names it. The Reviewer Notes content is partly covered by the Notes section. The description is mostly complete, so the check passes, but the missing template sections should be added.

Full details: Tests For Production Code

Explanation

PASS — The PR changes production code in open-sse/utils/directResponseStartTimeout.ts and adds tests/unit/direct-response-start-timeout-volume-tier.test.ts. The added tests cover the new payload-size tiers, cap, raise-only behavior, kill switch, and resolver integration.

Full details: Docstring Coverage

Explanation

Docstring coverage is 0.00% which is insufficient. The required threshold is 80.00%. Docstring coverage is scoped to functions touched by this diff with cyclomatic complexity of at least 2. Documentation-only edits and deletions within surviving functions are included; unchanged diff context and fully deleted functions are excluded. Analyzed 4 functions across 2 files. (3 skipped: 3 unsupported.)

Resolution

Write docstrings for the functions missing them to satisfy the coverage threshold.

Avoid allocating an encoded copy of the body.

TextEncoder.encode() allocates a body-sized buffer on every resolver call, even when adaptation is disabled. Chat bodies can reach 50 MiB, so concurrent large requests add substantial memory pressure. Count UTF-8 bytes without allocating the copy, and skip the count when adaptation is disabled. adaptation is enabled. Preserve the existing byte-counting behavior for all
supported body content.

Minor · Functional Correctness · Quick win

Isolate the resolver test from environment overrides.

If OMNIROUTE_DIRECT_HEADERS_VOLUME_ADAPTIVE=0 is set, this resolver reads process.env and returns 30,000 ms, so the test suite fails. Set and restore the relevant environment values around this test.
